A pilot study evaluating the effect of early physical therapy on pain and disabilities after breast cancer surgery: Prospective randomized control trail.

ABSTRACT:

Background

Morbidity of the shoulders after breast cancer (BC) surgery is a common side effect that includes; persistent pain, function limitation, and decreased range of motion (ROM). This study examines the effect of early physical therapy (PT) and patient's education on these morbidities.

Methods

A prospective, randomized clinical trial was conducted at a single medical center from October 2018 until April 2019. Women scheduled for breast cancer surgery were divided into intervention or control as standard care. The intervention included a PT treatment that included exercise instructions from the first postoperative day. Pain levels, upper limb function, ROM, and complications were measured.

Results

The study includes 157 women (mean age, 52.2 ± 12.9). Early PT reduced pain levels at the first month (NPRS 1.5 ± 1.2) and six months (NPRS 0.5 ± 0.8), compared with control (NPRS 2.1 ± 1.4, 1.0 ± 1.2), p = 0.019 and p = 0.011, respectively. Subdivision of the sample into small and extensive surgeries revealed additional positive effect for the intervention six months postoperatively on functional disabilities, p = 0.004 and p = 0.032 respectively. No complications attributable to the intervention were recorded.

Conclusions

Early PT and patient education reduces pain levels, and may improve function disabilities, without causing postoperative complications, although a larger study is needed to achieve unequivocal results.

Project description:Background: Physical activity (PA) interventions are limited in number and reach for youth with physical disabilities (YPD) who experience systemic barriers that may preclude their in-person participation. Further, a lack of theory in the development and evaluation of PA interventions impedes our understanding and replication of active components of behavior change. These limitations pose challenges in the effective promotion of PA in YPD. Theory-based and more inclusive methods of PA intervention delivery must be explored in our efforts to promote PA and overall health in YPD. Methods: A pilot study was conducted to evaluate the feasibility and outcomes of an online, 4-week social cognitive theory-based PA intervention for YPD. Intervention feasibility (implementation fidelity, intervention compliance, and intervention acceptability) was evaluated through manual documentation, weekly feedback questionnaires, and open-ended feedback at 1-month post-intervention. Targeted social cognitive (outcome expectations, self-efficacy [task, self-regulatory, barrier] and self-regulation) and PA behavior outcomes were self-reported at baseline and 1-week and 1-month post-intervention. Results: Sixteen YPD (Mage = 17.4 ± 2.7 years, 69% female) completed the study. Intervention feasibility was supported by high implementation fidelity (100%), high intervention compliance (>90%), and positive ratings on indicators of acceptability for all weeks of the intervention (weekly feedback questionnaire means ranging from 5.74 to 6.19 out of 7). Through open-ended feedback, participants indicated the intervention was easy to use and understand, favorably shifted their self-awareness and personal meaning of PA, and provided value and potential for future use pertaining to the learned self-regulation skills and strategies. Participants also provided formatting and content recommendations for intervention improvement. Repeated measures ANOVAs showed significant and large effect sizes for changes in participants' task (p = 0.01, n 2 p = 0.28) and barrier (p = 0.02, n 2 p = 0.24) self-efficacy, goal-setting and planning and scheduling behaviors (ps < 0.001, n 2 ps = 0.42), and self-reported PA behavior (p = 0.02, n 2 p = 0.26). Conclusions: An online PA intervention for YPD is feasible and may offer potential benefit through the enhancement of self-efficacy, self-regulation, and PA behavior. Continued research is necessary to understand the efficacy and longer-term outcomes of online, theory-based interventions for YPD as a PA promotion strategy.

Project description:BackgroundThe daily life of children with a physical disability is organized around interventions and care, which is coordinated by a multidisciplinary team. Little is known about the incidence of care-related pain in pediatric rehabilitation centers and health facilities for children.AimTo determine the incidence and intensity of care-related pain in children with physical disabilities, identify risk factors for pain and practices used to prevent care-related pain in pediatric rehabilitation centers and health facilities for children in France.DesignNon-interventional observational study.SettingSixteen pediatric rehabilitation and special education centers in 4 departments of Brittany (France).PopulationA number of 280 children with physical disabilities randomly selected (mean age: 12±4 years). Predominant medical diagnosis was nervous system diseases (68%; e.g., cerebral palsy 33%).MethodsThe FLACC-r scale was used to evaluate pain during each care activity or intervention that required physical contact with the child for five consecutive days and one night.ResultsThe recorded interventions were 7689. Pain was induced by 6% of physical acts, and 48% of children experienced at least one painful act during the study period. Acts that were more frequently associated with pain and had the highest pain intensity were standing frame use, feeding, gentle mobilizations and bladder catheterization. Age, level of dependency and type of act were all risk factors for care-related pain (P<0.01). Pain prevention was used for only 26.5% of acts.ConclusionsCare-related pain is frequent and under-recognized in pediatric rehabilitation and health facilities for children. All acts that involve direct physical contact can cause pain. Young and severely dependent children are most at risk of pain.Clinical rehabilitation impactAll professionals who are involved in the care of children with a physical disability and significant limitations in activity and participation must be aware of the issue of pain and that pain can be induced by even the most routine physical act. The management of care-related pain requires a benefit-risk analysis, a prevention and pain assessment, and a family-professional partnership. A multidimensional approach is needed for more individualized pain management and to evaluate the impact of pain on children's participation.

Project description:ObjectiveDesign and pilot test a new decision making tool for women with physical disabilities (impairment of physical function due to chronic conditions) considering pregnancy.Data sources and study settingQuantitative surveys and qualitative interviews were collected from participants living in the community.Study designClinical guidelines and survey and focus group data about pregnancy informational and decisional needs guided content development. The tool was pilot tested in a 12-week trial with participants with physical disabilities considering or actively planning a pregnancy. Feasibility outcomes were acceptability, implementation, and demand (collected at end of the trial); preliminary efficacy focused on decisional conflict and readiness (baseline, 6 weeks, and end of trial).Data collectionSurvey data were collected using an online form. One-on-one interviews were conducted to learn more about experience using the tool.Principal findingsThirty eight participants with mild, moderate, or severe physical disabilities participated. Feasibility outcomes indicated that the tool provided participants with information, guiding questions, and helped them to consider multiple aspects of the decision about pregnancy. Most participants responded positively to the new decision making tool, finding it easy to use and the information balanced. Feedback highlighted opportunity for improvement, such as more specific information, peer stories, and the limitations of a paper format. There was significant linear effect of time, with increased decisional certainty and readiness, values clarity, and decisional support (partial η2 [90% CI] = 0.310 [0.08, 0.46], 0.435 [0.19, 0.60], 0.134 [0, 0.29], 0.178 [0.01, 0.35], respectively). Decisional certainty and readiness had high observed power (96.7% and 99.3%, respectively) with lower observed power for clarity and support (60.6% and 75.1%, respectively).ConclusionsThe new tool shows promise for supporting women with physical disabilities in navigating pregnancy decision making. Future development of complementary strategies to support health care providers will help improve shared decision making and patient-centered care.
