Nanoscopic Medial Meniscus Repair.

ABSTRACT: Arthroscopic meniscus repair is one of the commonly used arthroscopic surgical procedures. Open treatment was the standard for meniscus repairs and recently arthroscopic all-inside treatment has become popular. Novel and more minimally invasive techniques to common arthroscopic procedures are paving the foundation to better patient outcomes. With the use of the NanoScope and nanoinstruments, we continue to develop new minimally invasive diagnostic and treatment techniques that do not require standard portals. The nanoscopic medial meniscus repair technique described here uses a less-invasive approach to a meniscus repair.

Knotless Medial Meniscus Posterior Root Repair.

Project description:Medial meniscus posterior root tears are common injuries, especially in the Asian world. This injury must be repaired where indicated to restore knee biomechanics and prevent arthritis. Suturing the meniscus using suture tapes provides good hold of the tissue. The use of a 70° arthroscope and creation of a transseptal portal improve visualization of the posterior compartment. Creation of a high posteromedial portal achieves the correct trajectory for the suture anchor insertion. A knotless suture anchor can be used to fix the posterior root at its anatomic attachment site. This obliterates the need for transtibial drilling for a suture pull-out repair or for knot tying and suture shuttling as for a conventional suture anchor.

Project description:Repair of medial meniscus posterior root tear is important in preventing rapid progression of knee osteoarthritis. There are many repair techniques for medial meniscus posterior root tears, and good clinical results have been reported. Conversely, in terms of improving extrusion and healing meniscal injuries, sufficient results have not been obtained. Reconstruction of the medial meniscus posterior root can restore meniscal hoop action and normal knee biomechanics. Moreover, pull-out repair technique provides strong traction. We describe arthroscopic medial meniscus posterior root reconstruction and pull-out repair technique combined technique for medial meniscus root tear.

Technique for Medial Meniscus Ramp Repair: An Anatomic Approach.

Project description:Medial meniscus ramp lesions are commonly associated with anterior cruciate ligament injury. They are one of the causes of positive pivot shift and can lead to failure to obtain rotary stability if missed during the surgical procedure. Several repair methods use a suture lasso from 2 posteromedial portals to attach the posteromedial capsule to the meniscus. Because it was concluded at the consensus meeting of the European Society for Sports Traumatology, Knee Surgery and Arthroscopy in 2019 that ramp lesions are not considered a meniscal tear and the lesion is essentially in either the articular capsule, meniscal capsular ligament, or meniscotibial ligament (MTL), it should be considered as a peel-off injury of the posteromedial capsule. This lesion is a meniscocapsular separation with or without MTL injury. Thus, reattaching the capsule and MTL to the bone on the posteromedial aspect would provide adequate anatomical repair. The prevailing repair methods suture the capsule to the meniscus, ignoring the MTL attachment. This technique addresses the refixation of the meniscal capsular ligament and MTL to the bone where it is attached instead of suturing it to the meniscus, thus providing sounder anatomical repair.

Project description:Meniscal root tears have become increasingly recognized in the past decade. Lateral meniscus root tears typically occur in active, younger patients who sustain acute anterior cruciate ligament tears. Conversely, medial meniscus root tears typically occur in isolation and affect middle-aged patients. However, recent reports have described the incidence of meniscal root tears occurring in pediatric populations who are skeletally immature. The purpose of this technical note is to describe the authors' surgical technique for medial meniscal root repair for patients with open physes.

Tips and Pearls for All-Inside Medial Meniscus Repair.

Project description:The medial meniscus is one of the most commonly injured structures in the knee. When the importance of its proper function is well understood, an adequate management in meniscus tear is a key issue for whole knee joint well-being. Although it has been proven that meniscal repairs have better long-term results than meniscectomy, there is still no consensus as to which suturing technique is the best. An all-inside technique seems to allow for the most anatomic repairs with the greatest preservation of surrounding soft tissues. Our aim is to show that there are no technical limits for all-inside meniscal repairs with nonabsorbable sutures. We describe the technique and some tricks for medial meniscus repair in this Technical Note.

Medial Meniscus Posterior Root Repair Using a Transtibial Technique.

Project description:The meniscal roots are critical in maintaining the normal shock absorbing function of the meniscus. If a meniscal root tear is left untreated, meniscal extrusion can occur rendering the meniscus nonfunctional resulting in degenerative arthritis. Two main repair techniques are described: (1) suture anchors (direct fixation) and (2) sutures pulled through a tibial tunnel (indirect fixation). Meniscal root repair using a suture anchor technique is technically challenging requiring a posterior portal and a curved suture passing device that can be difficult to manipulate within the knee. We present a technique for posterior medial meniscus root repair using 3 sutures (1 leader, 2 cinch), standard arthroscopy portals, and transtibial fixation. Overall, this technique simplifies a challenging procedure and allows for familiarity and efficiency.

Project description:Concomitant meniscal injuries are common in patients with anterior cruciate ligament (ACL) insufficiency. Along with ACL reconstruction, meniscal repair offers better outcomes. This requires multiple incisions for both procedures. We present a novel technique of medial meniscus inside out repair using only the tibial tunnel wound without making an additional posteromedial incision. This is the first such description of this novel technique in a 21 year old male who underwent ACL reconstruction along with bucket handle medial meniscal repair.

Although the clinical outcomes of MMPRT repair have demonstrated improvements, subsequent second-look arthroscopy shows poor healing postrepair. Consequently, numerous repair techniques have been reported. Herein, we introduce a simple locking-suture method, the "delta-grip" stitch, for MMPRT repair. This technique exhibited sufficient pullout strength compared with conventional suturing methods, offering promising prospects for enhancing the outcomes of MMPRT repair.

Arthroscopic Saucerization and Repair of a Torn Medial Discoid Meniscus.

Project description:A discoid meniscus is a congenital abnormality that usually affects the lateral meniscus, leading to instability and increased risk of tearing. A discoid medial meniscus is an extremely rare pathology that is seldom described in literature. In this report, we present the technique of operative treatment of a symptomatic, torn discoid medial meniscus. The meniscus is saucerized to 6-8 mm of stable rim, and the inside-out technique is used as the modality of meniscal fixation. Although a discoid medial meniscus is an uncommon finding, all treating surgeons should be aware of the possibility during surgical intervention.
