ABSTRACT: Chest pain in young adults is not always benign, and clinical suspicion should prompt further investigations. Multimodal imaging with computed tomography coronary angiography and cardiovascular magnetic resonance can be used to identify anomalous coronary arteries, determine adverse imaging features, and guide subsequent clinical decision making. Anomalous Double Right Coronary Artery Presenting as Inferior Wall ST-elevation Myocardial Infarction: A Rare Coronary Anomaly

Project description:Congenital anomalous origin of coronary arteries is rare and occurs in 0.2%–2% of patients undergoing coronary angiography (CAG). Most of the cases are benign but may present with life-threatening symptoms such as myocardial ischemia or sudden cardiac death. The prognosis depends on the site of origin of the anomalous artery, intramyocardial course, and relation to other great vessel and cardiac structures. Increased awareness and easy availability of noninvasive methods like computed tomography CAG have led to more reporting of such cases. Here, we report the case a 52-year-old male with a double right coronary artery having anomalous origin from a noncoronary aortic cusp detected during CAG which has not been reported in the literature before.

Project description:Anomalous origin of the left coronary artery from the pulmonary artery is rare and typically results in mitral regurgitation, ventricular arrhythmias, heart failure, and sudden death. The condition most often manifests itself in early childhood, but some individuals are diagnosed much later. We describe the case of a 75-year-old woman with heart failure in whom stepwise multimodal imaging revealed anomalous origin of the left coronary artery from the pulmonary artery.

Acute Myocardial Infarction Caused by an Anomalous Right Coronary Artery Occlusion Presenting with Precordial ST Elevation.

Project description:Acute coronary syndrome with precordial ST segment elevation is usually related to left anterior descending artery occlusion, although isolated right ventricular infarction has been described as a cause of ST elevation in V1-V3 leads. We present a case of a patient with previous inferior wall infarction and new acute ST elevation myocardial infarction (STEMI) due to proximal right coronary thrombotic occlusion resulting in right ventricular infarction with precordial ST elevation and sinus node dysfunction. The patient was treated with successful rescue angioplasty achieving resolution of acute symptoms and electrocardiographic abnormalities.

Anomalous origin of the left coronary artery from pulmonary artery mimicking antero-lateral ST-elevation myocardial infarction: a case report.

Project description:BackgroundAnomalous origin of the left coronary artery (LCA) from the pulmonary artery (PA) (ALCAPA) is a rare congenital abnormality. We present a case of an ALCAPA in a 25-year-old man.Case summaryA 25-year-old male with no past medical history was admitted to our intensive cardiac care unit after sudden cardiac arrest due to ventricular fibrillation and suspected acute coronary syndrome. After being treated with shock by automated external defibrillator, an electrocardiogram (ECG) demonstrated sinus tachycardia with antero-lateral ST-segment elevation. Initial transthoracic echocardiography showed severe and diffuse left ventricular dysfunction and dilatation. Coronary angiography revealed anomalous origin of the LCA from the PA and extensive collateral circulation from a giant RCA. An ECG-gated cardiac computed tomography confirmed the diagnosis of anomalous left main originating from the left PA. Cardiac magnetic resonance demonstrated an enlarged left ventricle with globally reduced function and extensive sub-endocardial scarring of the anterior, antero-lateral, and lateral walls. Following a multidisciplinary heart team discussion, the patient successfully underwent repair of aberrant LCA with direct LCA re-implantation to the aorta and subcutaneous implantable cardioverter defibrillator implantation. Optimal medical therapy for heart failure with reduced ejection fraction was initiated, and the patient was discharged home for a close clinical and echocardiographic follow-up.DiscussionIn conclusion, ALCAPA in the adulthood is a very rare congenital anomaly that clinicians should be aware of. The preferred treatment, when diagnosed in time, is direct re-implantation of the LCA to the aorta.

Catheter-induced aortocoronary dissection at the ostium of anomalous left coronary artery during percutaneous coronary intervention for acute inferior myocardial infarction.

Project description:We report a case of catheter-induced aortocoronary dissection at the ostium of anomalous left coronary artery (LCA) during percutaneous coronary intervention (PCI) for acute inferior myocardial infarction (MI). Urgent coronary angiography revealed the culprit lesion of MI was the proximal segment of the right coronary artery (RCA). The anomalous LCA arose from the right sinus of Valsalva the same as the RCA. Catheter-induced aortocoronary dissection at the ostium of RCA was extended to the ostium of anomalous LCA by contrast injection. The patient fell into hemodynamic collapse due to acute occlusion of the anomalous LCA. The patient underwent successful bailout stenting at the ostium of anomalous LCA under percutaneous cardiopulmonary support (PCPS). He was weaned from PCPS system five days after PCI and was discharged. This is the first report about bailout procedure for catheter-induced aortocoronary dissection at the ostium of anomalous LCA. <Learning objective: We present a case of catheter-induced aortocoronary dissection at the ostium of anomalous left coronary artery in a patient with acute inferior myocardial infarction. Anomalous origin of the coronary artery arising from the opposite sinus of Valsalva is a rare congenital anomaly. Aortocoronary dissection is also a rare but well-known complication during cardiac catheterization. Precise information on anatomy of anomalous coronary arteries is essential for percutaneous coronary intervention and attention should be paid to the potential risk of complications.>.

Project description:Anomalous left coronary artery arising from the noncoronary cusp (LCANCC) is a rare congenital disorder. We herein describe a 17-year-old female patient with sudden cardiac arrest followed by refractory cardiogenic shock. LCANCC-induced acute myocardial infarction with left main coronary artery involvement was subsequently diagnosed, and the patient required a durable left ventricular assist device. <Learning objective: Anomalous left coronary artery arising from the noncoronary cusp is extremely rare, but poses a life-threatening condition. Coronary computed tomography angiography plays a crucial role for the assessment of high-risk anatomic features and decision-making for surgical repair.>.
