Exploring the efficacy and safety of polyene phosphatidylcholine for treatment of drug-induced liver injury using the Roussel Uclaf causality assessment method: a propensity score matching comparison.

ABSTRACT: Objective In China, polyene phosphatidylcholine (PPC) is widely used to treat alanine aminotransferase (ALT) elevation associated with various liver diseases. Here, we assessed the efficacy and safety of PPC in treating drug-induced liver injury (DILI).Methods Data from a multicenter retrospective cohort study (DILI-R) were analyzed to compare PPC and magnesium isoglycyrrhizinate (MgIG) for treatment of DILI. We used the Roussel Uclaf causality assessment method (RUCAM) to evaluate patients with DILI. Patients with RUCAM scores ≥6 were included in the study, while those with RUCAM scores <6 were further evaluated by a panel of hepatologists. The primary outcome was the proportion of patients with ALT normalization at discharge. Propensity score matching was used to identify 183 matched pairs of patients (366 patients in total) from 25,927 patients with DILI.Results Among the DILI patients, 64 of 183 (34.97%) achieved normal ALT levels after treatment in both the PPC and the MgIG groups.Conclusion There were no significant differences in safety biomarkers including serum creatinine, blood urea nitrogen, white blood cells, platelets, hemoglobin, and albumin between patients treated with PPC or MgIG. The safety and efficacy of these two agents for treatment of DILI were comparable.

Project description:Currently, safety of laparoscopic pancreaticoduodenectomy (LPD) in patients with liver cirrhosis is unknown. The aim of this study was to explore postoperative morbidity and mortality and long-term outcomes of cirrhotic patients after LPD. The study was a one-center retrospective study comprising 353 patients who underwent LPD between October 2010 and December 2019. A total of 28 patients had liver cirrhosis and were paired with 56 non-cirrhotic counterparts through propensity score matching (PSM). Baseline data, intra-operative data, postoperative data, and survival data were collected. Postoperative morbidity was considered as primary outcome whereas postoperative mortality, surgical parameters (operative durations, intraoperative blood loss), and long-term overall survival were secondary outcomes. Cirrhotic patients showed postoperative complication rates of 82% compared with rates of patients in the control group (48%) (P = 0.003). Further, Clavien-Dindo ≥III complication rates of 14% and 11% (P = 0.634), Clavien-Dindo I-II complication rates of 68% and 38% (P = 0.009), hospital mortality of 4% and 2% (P = 0.613) were observed for cirrhotic patients and non-cirrhotic patients, respectively. In addition, an overall survival rate of 32 months and 34.5 months (P = 0.991), intraoperative blood loss of 300 (200-400) ml and 150 (100-250) ml (P<0.0001), drain amount of 2572.5 (1023.8-5275) ml and 1617.5 (907.5-2700) ml (P = 0.048) were observed in the cirrhotic group and control group, respectively. In conclusion, LPD is associated with increased risk of postoperative morbidity in patients with liver cirrhosis. However, the incidence of Clavien-Dindo ≥III complications and post-operative mortality showed no significant increase. In addition, liver cirrhosis showed no correlation with poor overall survival in patients who underwent LPD. These findings imply that liver cirrhosis patients can routinely be considered for LPD at high volume centers with rigorous selection and management.

Project description:BackgroundIn recent years, stenting has been widely used to treat patients with idiopathic intracranial hypertension (IIH) and venous sinus stenosis (VSS); however, research comparing stenting and medical treatment (MT) remains scarce. This study aimed to evaluate the effectiveness of stenting and MT in treating patients with IIH and VSS.MethodsIn this single-center, retrospective, cohort study, the clinical data of patients diagnosed with IIH and VSS at The First Affiliated Hospital of Zhengzhou University from January 2018 to June 2023 were collected for analysis. Based on the treatment approaches, the patients were divided into the following two groups: the stenting group (Group S), and the MT group (Group M). The patients underwent 1:1 propensity score matching (PSM) to compare the improvement in papilledema Frisén grade, lumbar puncture opening pressure (LPOP), and clinical symptoms after treatment.ResultsIn total, 128 participants were included in the study. The participants had an average age of 40.0±11.1 years (range, 18-61 years) and a body mass index (BMI) of 27.5±3.3 kg/m2 (range, 20.0-40.0 kg/m2), and 73.43% were female (68 in Group S and 60 in Group M). Compared with the patients in Group M, those in Group S had a longer median time from onset to treatment (2 vs. 1 month, P=0.026), a higher proportion of papilledema (85.3% vs. 68.3%, P=0.033), a higher median pretreatment stenosis rate (80% vs. 70%, P=0.005), and a larger median pretreatment trans-stenotic pressure gradient (15.5 vs. 11.0 mmHg, P=0.001), and a larger median pretreatment LPOP (391.1 vs. 350.5 mmH2O, P=0.006). Following 1:1 PSM, both groups comprised 28 patients each, and there were no statistically significant differences between the two groups in terms of the covariates (all P>0.05). Compared with the patients in Group M, those in Group S had a lower median papilledema Frisén grade (1 vs. 2, P=0.002) and average LPOP (213.0 vs. 259.8 mmHg, P=0.003) at discharge, and showed more pronounced symptom improvement at the time of discharge (P=0.019), and at 6 months (P=0.011) and 12 months (P<0.001) post-discharge.ConclusionsThe research indicated that stenting was quicker and more effective in alleviating papilledema, LPOP, and corresponding symptoms and signs than MT.

Project description:Purpose: Drug-induced liver injury (DILI) is a common adverse reaction in the clinic; however, there are relatively few reports of DILI in critically ill newborns and children. Making use of the Pediatric Intensive Care database (PIC), this study identifies which drugs are related to DILI in neonates and children in China. Methods: Using the PIC, we screened for patients whose liver was suspected of being injured by drugs during hospitalization. The medicine they used was then assessed by the Roussel Uclaf Causality Assessment Method (RUCAM). At the same time, we also collated drug combinations that may affect CYP (Cytochrome P) enzyme metabolism, which may cause DILI. Results: A total of 13,449 patients were assessed, of whom 77 newborns and 261 children were finally included. The main type of liver injury in neonates was mixed (83.1%), while the hepatic injury types of children were mostly distributed between hepatocellular (59.4%) and cholestatic (28.4%). In terms of the RUCAM assessment, the drugs that were most considered to cause or be associated with hepatic injury in newborns were medium and long chain fat emulsions (17%), sodium glycerophosphate (12%), and meropenem (9%); while omeprazole (11%), methylprednisolone sodium succinate (10%), and meropenem (8%) were the primary culprits of DILI in children. Drug combinations frequently seen in neonates that may affect CYP enzyme metabolism are omeprazole + budesonide (16.9%), dexamethasone + midazolam (10.4%), and midazolam + sildenafil (10.4%). In children, the commonly used drug combinations are fentanyl + midazolam (20.7%), ibuprofen + furosemide (18.4%), and diazepam + omeprazole (15.3%). Conclusions: In this study, medium and long chain fat emulsions and sodium glycerophosphate have been strongly associated with DILI in newborns, while omeprazole and methylprednisolone sodium succinate play an important role in the DILI of children. Also, attention should be paid to the effect on CYP enzymes when using multiple drugs at the same time. In future DILI cases, it is advisable to use the latest RUCAM for prospective study design so that complete case data and high RUCAM scores can be collected.
