Project description:In the present study, we examine the unique and interactive effects of race (African American or European American) and depression on suicidal ideation, controlling for poverty, within a representative sample of adolescent girls. A community sample of 2450 girls (43.9% African American) participating in the longitudinal Pittsburgh Girls Study (PGS) was interviewed annually about depression symptoms and suicidal ideation, from ages 10-15 years. Caregivers reported on the girls' racial/ethnic background and the family's receipt of public assistance. Race and depression scores explained unique variance in suicidal ideation; receipt of public assistance did not. Endorsement and recurrence of suicidal ideation was more likely for African American than European American girls: there was a nearly two-fold increase in the likelihood of reporting frequent thoughts of death or suicide as a function of race. Of the 255 girls reporting recurrent suicidal ideation, 65.9% were African American. An interaction effect between race and depression symptoms was observed, such that African American girls were more likely to report suicidal ideation at lower levels of depression severity. The findings indicate that race is a critical factor for understanding, preventing, and treating suicidal ideation in girls.

Project description:BackgroundGlobally, suicide is the third leading cause of death among girls aged 15-19 years. However, there is a growing concern that suicide research has paid little attention to pregnant adolescent girls, particularly in low-income and middle-income countries, including Ghana, where nearly one in seven adolescents experiences (unwanted) pregnancy.AimsTo assess the prevalence and correlates of suicidal ideation and suicide attempt and the self-reported reasons for attempted suicide among pregnant adolescent girls in Ghana.MethodsWe collected cross-sectional data (between August 2022 and December 2023) using structured anonymous questionnaires from a sample of 449 pregnant adolescent girls (aged 14-19 years) drawn from the three geographical zones of Ghana. We applied bivariable and multivariable analysis techniques to the data.ResultsOverall, 28.51% (95% confidence interval (CI) 24.37% to 32.93%) reported suicidal ideation, and 18.04% (95% CI 14.59% to 21.91%) reported suicide attempt during the current pregnancy. Participants who reported attempted suicide endorsed more interpersonal reasons (eg, to communicate distress, to seek help or to influence others) than intrapersonal reasons for their attempted suicide. In the final adjusted logistic regression models, food insecurity (adjusted odds ratio (aOR)=2.69; 95% CI 1.34 to 5.41; p=0.005) was uniquely associated with increased odds of suicidal ideation. Adverse childhood experiences (aOR=3.04; 95% CI 1.33 to 6.97; p=0.008), history of attempted suicide before current pregnancy (aOR=3.47; 95% CI 1.27 to 9.47; p=0.0.015) and depression (measured by the 5-item World Health Organization Well-Being Index; aOR=0.31; 95% CI 0.12 to 0.77; p=0.012) were uniquely associated with increased odds of suicide attempt. Five variables were commonly associated with increased odds of both suicidal ideation and suicide attempt: conflict with parents, alcohol use, pregnancy-related anxiety, history of attempted suicide and intimate partner violence. While being in junior high school was uniquely associated with reduced odds of suicidal ideation, intimate partner's acceptance of paternity was commonly associated with reduced odds of both suicidal ideation and suicide attempt.ConclusionsAlthough the prevalence estimates of suicidal ideation and suicide attempt in this study are comparable with known rates among non-pregnant adolescent girls in Ghana, additional research is needed to nuance our understanding of the correlates identified in this study. The evidence also highlights a need for both routine antepartum primary care mental health screening for suicidality and related risks and targeted prevention and intervention programmes.

Project description:Girls typically show much lower suicide rates than boys in most OECD countries. However, in South Korea, the suicide rate of girls almost reaches that of boys. Moreover, Korean girls' suicide rate is remarkable even among other advanced countries. One potential approach to explaining Korean girls' relatively high suicide rate is to investigate how their peer groups affect suicidal ideation, but this approach has rarely been explored in Korean adolescents. We tested how the gender heterogeneity of peer groups is associated with suicidal ideation by analyzing 2,990 adolescents from the 2018 Korean Children and Youth Well-Being Index Survey. For the analysis, logistic regression models with survey weights were used. The analysis revealed that adolescents with different-gender friends were associated with a higher likelihood of suicidal ideation than those with exclusively same-gender friends when adjusting for covariates. In addition, an analysis stratified by gender found that this association was significant only among girls. Furthermore, the protective power of having a mentor against suicidal ideation was significantly lower in girls with male and female friends than in girls with only female friends. The findings suggest a less protective role of different-gender peer groups for suicidal ideation among girls. During adolescent suicidality consultations, school counselors and practitioners should pay attention to the characteristics of adolescents' peers, particularly their gender.

Project description:IntroductionSuicidal thoughts and behaviors often onset during adolescence, constituting a major public health concern. Despite rising rates in youth, psychotherapeutic and pharmacological treatment outcomes remain meager, indicating a need to identify modifiable risk factors. Experts suggest sleep may serve as a promising clinical indicator of risk and treatment progress, yet few studies have examined acute temporal associations between sleep and suicidal ideation.MethodsParticipants were depressed and suicidal adolescents (age 13-19, n = 311) attending an intensive outpatient program (IOP). Patients completed weekly self-report assessments of sleep, depression, and suicidal ideation throughout IOP, with an average of 5 assessments over 30 days.ResultsGreater overall sleep difficulties, as well as within-person increases in sleep difficulties, were predictive of greater depression severity and suicidal ideation at subsequent assessments, above and beyond previous levels of symptoms. The reverse associations were not found. Inclusion of within-person changes in sleep difficulties significantly improved model fit compared to inclusion of overall sleep difficulties alone.DiscussionThis study provides additional evidence for a prospective relationship between worsening sleep difficulties and suicidal ideation in an IOP youth sample. Worsening sleep may be an important clinical indicator of subsequent depression and suicide risk among adolescents in treatment.

Project description:BackgroundResearch on the neural correlates associated with risk for suicidal ideation (SI) has been limited, particularly in one increasingly at-risk group-adolescents. Previous research with adolescents indicates that poor emotion regulation skills are linked with SI, but these studies have not previously examined neural activation in service of emotion regulation between those with and without SI histories.MethodsHere we examine whether SI is associated with neural responses during an emotion regulation functional magnetic resonance imaging task in a group of adolescents (N = 49) 13 to 20 years of age (mean = 16.95).ResultsWhile there were no differences between youths with and without SI in self-reported emotional responses to negative pictures, youths with SI activated the dorsolateral prefrontal cortex more than youths without SI on trials in which they attempted to regulate their emotional responses compared with trials in which they passively viewed negative pictures. In contrast, during passive viewing of negative stimuli, youths with SI activated the dorsolateral prefrontal cortex, temporoparietal junction, and cerebellum less than same-age control subjects.ConclusionsThese findings were robust to control subjects for depression and adversity exposure and are consistent with the idea that youths with SI have disrupted emotion regulation, potentially related to differences in recruitment of top-down control regions. In contrast, youths without SI activated regions implicated in emotion regulation even when not directed to effortfully control their emotional response. This is the first study to examine neural function during emotion regulation as a potential neural correlate of risk for SI in adolescents.

Project description:Although suicide remains a leading cause of death for adolescents, risk factors beyond diagnoses and suicide attempt history remain unclear. We examined whether cognitive style and temperament impact risk for an early, yet still clinically relevant and distressing, form of suicidality: active suicidal ideation. We used binary logistic regression to test whether brooding, inattention, and impulsivity predicted significantly increased risk for suicidal ideation in a sample of 134 twins, 46 of whom endorsed active suicidal ideation (i.e., probands), as well as probands' cotwins and matched controls. When comparing probands with controls and controlling for depression diagnoses, brooding (B = 0.73, Odds Ratio [OR] = 2.07, p = 0.021), inattention (B = 1.09, OR = 2.98, p < 0.001), and impulsivity (B = 0.91, OR = 2.47, p = 0.001) differentiated probands from controls, individually. We compared probands with their cotwins using the same approach, which allowed us to account for variance in suicidal ideation risk related to twins' shared, familial characteristics (e.g., prenatal environment, neighborhood); inattention was the only significant predictor of suicidal ideation risk (B = 0.66, OR = 1.93, p = 0.020). We then fit a logistic regression model that included all three predictors. Only inattention predicted significantly increased likelihood of suicidal ideation in proband versus controls and proband versus cotwin comparisons (B = 0.88, OR = 2.40, p = 0.024 and B = 0.67, OR = 1.96, p = 0.045, respectively). These results highlight the potential utility of examining novel, more proximal risk factors for suicidal ideation in addition to more established distal factors, like suicide attempt history and psychiatric diagnoses.

Project description:On average, one person dies by suicide every 40 s. However, extant studies have largely focused on the risk factors for suicidal behaviors, not so much on the formation of suicidal thoughts. Therefore, we attempt to explain how suicidal thoughts arise and persist inside one's mind using a multifiltering information mechanism called Mindsponge. Bayesian analysis with Hamiltonian Markov Chain Monte Carlo (MCMC) technique was run on a dataset of multinational students (N = 268) of an international university in Japan. Item 9 in the PHQ-9 was used to survey suicidal ideation. The associations among four main variables, namely, (i) suicidal ideation, (ii) help-seeking willingness (informal and formal sources), (iii) sense of connectedness, and (iv) information inaccessibility (represented by being international students), were tested in four models. Sense of connectedness is negatively associated with suicidal ideation, but its effect becomes less impactful when interacting with international students. The impact of a sense of connectedness on informal help-seeking willingness (toward family members) among international students is also lessened. Informal help-seeking is negatively associated with suicidal ideation, whereas formal help is positive. The findings support our assumption on three fundamental conditions for preventing suicidal thoughts: (i) a high degree of belongingness, (ii) accessibility to help-related information, and (iii) healthy perceived cultural responses towards mental health. Therefore, systematically coordinated programs are necessary to effectively tackle suicidal ideation.

Project description:During the immediate period following psychiatric hospitalization, adolescents are at increased risk for suicide attempts and rehospitalization. Because most adolescents return to school during this time, school-related experiences are important considerations during the transition from inpatient hospitalization. This study sought to understand how adolescent perceptions of school may change following hospitalization, and how these changes may predict recovery from a suicide-related crisis. Participants included 155 adolescents (Mage = 15.2 years; 68.6% female, 65.4% White; 14.7% Hispanic/Latinx; grades 7-12) hospitalized for a suicide-related crisis assessed 3 weeks and 6 months following discharge. Results from Latent Change Score models indicated a worsening of perceptions of teacher relationships (mean change (Δ) = 1.52), connectedness to learning (Δ = 1.55), parent involvement (Δ = 1.82), and academic satisfaction (Δ = 1.34), as well as higher frequency of perceived bullying victimization (Δ = 0.71) following hospitalization. Poorer perceptions of teacher relationships and higher frequency of perceived bullying victimization during hospitalization (β = 0.31 and 0.34), as well as worsening changes of teacher relationships and increased frequency of perceived bullying victimization following hospitalization (β = 0.48 and 0.41) were associated with higher levels of suicidal ideation severity 3 weeks following discharge. Poorer and worsening perceptions of teacher relationships were associated with higher levels of suicidal ideation intensity at 3 weeks (β = 0.37 and 0.54). Poorer perceptions of connectedness to learning during hospitalization emerged as a significant predictor of higher levels of suicidal ideation intensity 6 months following hospitalization (β = 0.20). Results reinforce the importance of fostering positive adult relationships and preventing bullying both prior to and immediately following psychiatric hospitalization. Project description:BackgroundIt is well-established that offspring of depressed mothers are at increased risk for suicidal ideation. However, pathways involved in the transmission of risk for suicidal ideation from depressed mothers to offspring are poorly understood. The aim of this study was to examine the contribution of potential mediators of this association, including maternal suicide attempt, offspring psychiatric disorder and the parent-child relationship.MethodData were utilized from a population-based birth cohort (ALSPAC). Three distinct classes of maternal depression symptoms across the first 11 years of the child's life had already been identified (minimal, moderate, chronic-severe). Offspring suicidal ideation was assessed at age 16 years. Data were analysed using structural equation modelling.ResultsThere was evidence for increased risk of suicidal ideation in offspring of mothers with chronic-severe depression symptoms compared to offspring of mothers with minimal symptoms (odds ratio 3.04, 95% confidence interval 2.19-4.21). The majority of this association was explained through maternal suicide attempt and offspring psychiatric disorder. There was also evidence for an independent indirect effect via the parent-child relationship in middle childhood. There was no longer evidence of a direct effect of maternal depression on offspring suicidal ideation after accounting for all three mediators. The pattern of results was similar when examining mechanisms for maternal moderate depression symptoms.ConclusionsFindings highlight that suicide prevention efforts in offspring of depressed mothers should be particularly targeted at both offspring with a psychiatric disorder and offspring whose mothers have made a suicide attempt. Interventions aimed at improving the parent-child relationship may also be beneficial.

Project description:ObjectiveMultidimensional Treatment Foster Care (MTFC) has been found to reduce delinquency among girls in juvenile justice through 2-year follow-up. Given that such girls are at elevated risk for suicide and depression into adulthood, we tested MTFC effects on long-term trajectories of suicidal ideation and depressive symptoms.MethodGirls (N = 166; mean [SD] age = 15.3 [1.2] years; 68% White) with a recent criminal referral who were mandated to out-of-home care were enrolled in 2 sequential cohorts. Girls were randomized to receive MTFC (n = 81) or group care (GC) treatment as usual (TAU; n = 85); the second MTFC cohort also received modules targeting substance use and risky sexual behavior. Depressive symptoms and suicidal ideation were assessed repeatedly through early adulthood (mean [SD] follow-up = 8.8 [2.9] years). Suicide attempt history was assessed in early adulthood.ResultsGirls assigned to MTFC showed significantly greater decreases in depressive symptoms across the long-term follow-up than GC girls (π = -.86, p < .05). Decreases in suicidal ideation rates were slightly stronger in MTFC than in GC as indicated by a marginal main effect (odds ratio [OR] = .92, p < .10) and a significant interaction that favored MTFC in the second cohort relative to the first (OR = .88, p < .01). There were no significant MTFC effects on suicide attempt.ConclusionsMTFC decreased depressive symptoms and suicidal thinking beyond the decreases attributable to time and TAU. Thus, MTFC has further impact on girls' lives than originally anticipated.
