Perceived racial discrimination and polysubstance use among African American and Afro-Caribbean adults: Results from the National Survey of American Life.

ABSTRACT: This study examined the relationship between perceived racial discrimination (PRD) and patterns of substance use. Data come from the 2001-2003 National Survey of American Life (N = 3,589). PRD was derived from the Major Experiences of Discrimination Scale. Multinomial logistic regression estimated the relationship between PRD and patterns of substance use (i.e., never/former, single-substance, dual-substance, and polysubstance [3+ substances]) based on six substances; effect modification by ethnicity and sex was assessed by stratification. Study findings indicated that PRD was associated with greater odds of lifetime and current polysubstance use. Results from the effect modification analyses suggested differential associations by ethnicity and sex.

Heterogeneity in Cardiovascular Disease Risk Factor Prevalence Among White, African American, African Immigrant, and Afro-Caribbean Adults: Insights From the 2010-2018 National Health Interview Survey.

Project description:Background In the United States, Black adults have higher rates of cardiovascular disease (CVD) risk factors than White adults. However, it is unclear how CVD risk factors compare between Black ethnic subgroups, including African Americans (AAs), African immigrants (AIs), and Afro-Caribbeans, and White people. Our objective was to examine trends in CVD risk factors among 3 Black ethnic subgroups and White adults between 2010 and 2018. Methods and Results A comparative analysis of the National Health Interview Survey was conducted among 452 997 participants, examining sociodemographic characteristics and trends in 4 self-reported CVD risk factors (hypertension, diabetes, overweight/obesity, and smoking). Generalized linear models with Poisson distribution were used to obtain predictive probabilities of the CVD risk factors. The sample included 82 635 Black (89% AAs, 5% AIs, and 6% Afro-Caribbeans) and 370 362 White adults. AIs were the youngest, most educated, and least insured group. AIs had the lowest age- and sex-adjusted prevalence of all 4 CVD risk factors. AAs had the highest prevalence of hypertension (2018: 41.9%) compared with the other groups. Overweight/obesity and diabetes prevalence increased in AAs and White adults from 2010 to 2018 (P values for trend <0.001). Smoking prevalence was highest among AAs and White adults, but decreased significantly in these groups between 2010 and 2018 (P values for trend <0.001), as compared with AIs and Afro-Caribbeans. Conclusions We observed significant heterogeneity in CVD risk factors among 3 Black ethnic subgroups compared with White adults. There were disparities (among AAs) and advantages (among AIs and Afro-Caribbeans) in CVD risk factors, suggesting that race alone does not account for disparities in CVD risk factors.

Perceived racial discrimination and healthy behavior among African Americans.

Project description:ObjectiveNumerous studies have found evidence of a link between perceived discrimination and unhealthy behavior, especially substance use. Within this body of literature, however, several studies have found unexpected evidence of a positive relation between perceived racial discrimination among African Americans-mostly women-and certain types of healthy behavior, primarily exercise and healthy eating. The current study further examined this positive relation, including an anticipated moderator: optimism. It also examined the relation between perceived racial discrimination and a correlate of unhealthy behavior: BMI.MethodSix waves of data were collected over 14 years in three related samples of African Americans from families participating in the Family and Community Health Study. Each family included an adolescent (Mage = 10.5 at Wave 1), the adolescent's primary caregiver (Mage = 37), and, in some cases, an older sibling of that adolescent (Mage = 13). Wave 1 Ns were 889, 889, and 295, respectively. Healthy behavior was defined as diet and exercise.ResultsThere was very little evidence of a long-term relation between perceived racial discrimination and BMI in any sample, and no evidence of a relation between discrimination and healthy behavior among the males. However, correlational analyses revealed a positive prospective relation between discrimination and healthy behavior among all three groups of females; structural equation modeling indicated that this relation was stronger among women who were high in optimism.ConclusionsPerceived racial discrimination does not appear to be related to BMI among African Americans, but it is related to healthy behavior among Black females who are high in dispositional optimism. African American emerging adults' experiences with racial discrimination and drinking habits: The moderating roles of perceived stress.

Project description:OBJECTIVES:Despite the abundance of research aimed at quantifying the impact of racism on the mental and physical health of African Americans, results remain inconclusive largely because of challenges with operationalization, as well as conflation with the concept of racial discrimination, which may be more readily assessed. The purpose of the current study was to: (a) determine whether racial discrimination had an impact on the degree of alcohol use and binge drinking among African American emerging adults, and if so, (b) determine whether perceived stress linked to racially discriminatory experiences moderated these associations. METHOD:We used a series of hierarchical regressions to examine associations among racial discrimination, perceived stress, and degree of alcohol consumption in a sample of African American emerging adults in the southeast (n = 235). RESULTS:We found that the association between racial discrimination and degree of alcohol consumption (alcohol use and binge drinking) was strongest among individuals who reported greater levels of perceived stress linked to racial discrimination experiences. This association, however, was not significant for individuals who reported lower levels of perceived stress in response to racial discrimination. CONCLUSIONS:African Americans who experience a high degree of perceived stress in response to experiences with racial discrimination may be at greater risk for problem drinking than their peers with less perceived stress. These findings highlight the need for novel intervention efforts aimed at mitigating the effects of stress and racial discrimination on health outcomes. Racial discrimination and relationship functioning among African American couples.

Project description:Racial discrimination is a common stressor for African Americans, with negative consequences for mental and physical well-being. It is likely that these effects extend into the family, but little research has examined the association between racial discrimination and couple functioning. This study used dyadic data from 344 rural, predominantly low-income heterosexual African American couples with an early adolescent child to examine associations between self-reported racial discrimination, psychological and physical aggression, and relationship satisfaction and instability. Experiences of discrimination were common among men and women and were negatively associated with relationship functioning. Specifically, men reported higher levels of psychological aggression and relationship instability if they experienced higher levels of racial discrimination, and women reported higher levels of physical aggression if they experienced higher levels of racial discrimination. All results replicated when controlling for financial hardship, indicating unique effects for discrimination. Findings suggest that racial discrimination may be negatively associated with relationship functioning among African Americans and call for further research on the processes underlying these associations and their long-term consequences. Perceived Discrimination, Racial Identity, and Multisystem Stress Response to Social Evaluative Threat Among African American Men and Women.

Project description:Understanding individual differences in the psychobiology of the stress response is critical to grasping how psychosocial factors contribute to racial and ethnic health disparities. However, the ways in which environmentally sensitive biological systems coordinate in response to acute stress is not well understood. We used a social-evaluative stress task to investigate coordination among the autonomic nervous system, hypothalamic-pituitary-adrenal axis, and immune/inflammatory system in a community sample of 85 healthy African American men and women.Six saliva samples, 2 at each of baseline, event, and recovery phases of the stressor task, were assayed for cortisol, dehydroepiandrosterone-sulfate, salivary alpha-amylase, and salivary C-reactive protein. Individual differences in perceived discrimination and racial identity were also measured.Factor analysis demonstrated that stress systems were largely dissociated before stressor exposure but became aligned during event and recovery phases into functional biological stress responses (factor loadings ? .58). Coordinated responses were related to interactions of perceived discrimination and racial identity: when racial identity was strong, highly perceived discrimination was associated with low hypothalamic-pituitary-adrenal axis activity at baseline (B's = .68-.72, p < .001), low stress mobilization during the task (B's = .46-.62, p < .049), and a robust inflammatory response (salivary C-reactive protein) during recovery (B's = .72-.94, p < .002).Culturally relevant social perceptions may be linked to a specific pattern of changing alignment in biological components of the stress response. Better understanding these links may significantly advance understanding of stress-related illnesses and disparities.

Discrimination, racial bias, and telomere length in African-American men.

Project description:BackgroundLeukocyte telomere length (LTL) is an indicator of general systemic aging, with shorter LTL being associated with several chronic diseases of aging and earlier mortality. Identifying factors related to LTL among African Americans may yield insights into mechanisms underlying racial disparities in health.PurposeTo test whether the combination of more frequent reports of racial discrimination and holding a greater implicit anti-black racial bias is associated with shorter LTL among African-American men.MethodsCross-sectional study of a community sample of 92 African-American men aged between 30 and 50 years. Participants were recruited from February to May 2010. Ordinary least squares regressions were used to examine LTL in kilobase pairs in relation to racial discrimination and implicit racial bias. Data analysis was completed in July 2013.ResultsAfter controlling for chronologic age and socioeconomic and health-related characteristics, the interaction between racial discrimination and implicit racial bias was significantly associated with LTL (b=-0.10, SE=0.04, p=0.02). Those demonstrating a stronger implicit anti-black bias and reporting higher levels of racial discrimination had the shortest LTL. Household income-to-poverty threshold ratio was also associated with LTL (b=0.05, SE=0.02, p<0.01).ConclusionsResults suggest that multiple levels of racism, including interpersonal experiences of racial discrimination and the internalization of negative racial bias, operate jointly to accelerate biological aging among African-American men. Societal efforts to address racial discrimination in concert with efforts to promote positive in-group racial attitudes may protect against premature biological aging in this population.

Racial discrimination, educational attainment, and biological dysregulation among midlife African American women.

Project description:ObjectiveTo examine the association between self-reported racial discrimination and allostatic load, and whether the association differs by socioeconomic position.MethodsWe recruited a purposive cross-section of midlife (ages 30-50) African American women residing in four San Francisco Bay area counties (n = 208). Racial discrimination was measured using the Experience of Discrimination scale. Allostatic load was measured as a composite of 15 biomarkers assessing cardiometabolic, neuroendocrine, and inflammatory activity. We calculated four composite measures of allostatic load and three system-specific measures of biological dysregulation. Multivariable regression was used to examine associations, while adjusting for relevant confounders.ResultsIn the high education group, reporting low (b = -1.09, P = .02, 95% CI = -1.99, -0.18) and very high (b = -1.88, P = .003, 95% CI = -3.11, -0.65) discrimination was associated with lower allostatic load (reference=moderate). Among those with lower education, reporting low (b = 2.05, P = .008, 95% CI = 0.55,3.56) discrimination was associated with higher allostatic load. Similar but less consistent associations were found for poverty status. Associations were similar for cardiometabolic functioning, but not for neuroendocrine or inflammatory activity.ConclusionsRacial discrimination may be an important predictor of cumulative physiologic dysregulation. Factors associated with educational attainment may mitigate this association for African American women and other groups experiencing chronic social stress.

Examining changes in African American mothers' racial socialization patterns during adolescence: Racial discrimination as a predictor.

Project description:Racial socialization is a culturally relevant parenting strategy known to combat the detrimental consequences of racial discrimination for African American youth. Three limitations hinder our developmental understanding of the racial socialization process. Few studies have accounted for the combination of messages that primary caregivers convey, examined how these messages change over time, or investigated how caregivers and adolescents experiences with racial discrimination predict change in the combination of messages conveyed. Given that African American mothers are often the primary socializers in families, the current study used data from a community sample of 497 African American adolescents (52% Female; Time 1 Mage = 15.69; Time 2 Mage = 18.74) and their mothers (Time 1 Mage = 40.43; Time 2 Mage = 43.39) to identify patterns in mothers' racial socialization messages, identify how mothers' racial socialization patterns change from middle to late adolescence, and investigate whether mother- and adolescent-reported racial discrimination contribute to changes in mothers' racial socialization patterns. Latent profile analysis and latent transition analysis were used to examine these questions. Findings revealed three racial socialization patterns: balanced socializers who mistrust, cultural socialization and preparation for bias emphasizers, and low racial socializers. Most mothers were in the low racial socializers group, and most provided similar messages in middle and late adolescence. Mothers' reports of their own racial discrimination influenced the racial socialization messages mothers delivered; however, adolescent-reported racial discrimination did not. Longitudinal associations between experienced racial discrimination and depressive symptoms in African American adolescents.

Project description:While recent evidence has indicated that experienced racial discrimination is associated with increased depressive symptoms for African American adolescents, most studies rely on cross-sectional and short-term longitudinal research designs. As a result, the direction and persistence of this association across time remains unclear. This article examines longitudinal associations between experienced racial discrimination and depressive symptoms among a community sample of African American adolescents (N = 504) from Grade 7 to Grade 10, while controlling for multiple alternative causal pathways. Sex was tested as a moderator of the link between experienced racial discrimination and later depressive symptoms. Structural equation modeling revealed that experienced racial discrimination was positively associated with depressive symptoms 1 year later across all waves of measurement. The link between experienced racial discrimination at Grade 7 and depressive symptoms at Grade 8 was stronger for females than males. Findings highlight the role of experienced racial discrimination in the etiology of depressive symptoms for African Americans across early adolescence.

Perceived Discrimination and Heavy Episodic Drinking Among African-American Youth: Differences by Age and Reason for Discrimination.

Project description:PurposeThe purpose of this study was to examine whether associations between perceived discrimination and heavy episodic drinking (HED) vary by age and by discrimination type (e.g., racial, age, physical appearance) among African-American youth.MethodsNational data from the Panel Study of Income Dynamics Transition to Adulthood Study were analyzed. Youth participated in up to four interviews (2005, 2007, 2009, 2011; n = 657) between ages 18 and 25 years. Respondents reported past-year engagement in HED (four or more drinks for females, five or more drinks for males) and frequency of discriminatory acts experienced (e.g., receiving poor service, being treated with less courtesy). Categorical latent growth curve models, including perceived discrimination types (racial, age, and physical appearance) as a time-varying predictors of HED, were run. Controls for gender, birth cohort, living arrangement in adolescence, familial wealth, parental alcohol use, and college attendance were explored.ResultsThe average HED trajectory was curvilinear (increasing followed by flattening), whereas perceived discrimination remained flat with age. In models including controls, odds of HED were significantly higher than average around ages 20-21 years with greater frequency of perceived racial discrimination; associations were not significant at other ages. Discrimination attributed to age or physical appearance was not associated with HED at any age.ConclusionsPerceived racial discrimination may be a particularly salient risk factor for HED around the ages of transition to legal access to alcohol among African-American youth. Interventions to reduce discrimination or its impact could be targeted before this transition to ameliorate the negative outcomes associated with HED.
