Case Report: Dual-lumen microcatheter-facilitated wiring technique to correctly access a protruded aorto-ostial stent: a case series.

Project description:BackgroundPercutaneous coronary intervention (PCI) through the aorto-ostial coronary stent that is protruding into the aorta remains a technical challenge because of the poor coaxial alignment of the guiding catheter and the inability to advance the guidewire into the distal vessel through the stent's central lumen. In this article, we introduce a dual-lumen microcatheter-facilitated wiring technique performed on two patients to overcome this difficulty.Case summaryThe first case was a 75-year-old man who presented with chest pain. He was diagnosed with an unstable angina, and coronary angiography showed near-total in-stent occlusion of the previously placed stent protruding into the aorta. Despite several attempts, the guidewire passed through the side strut of the stent instead of the central stent lumen. Thus, we placed the tip of the microcatheter proximally to the side strut, outside the stent. Then, a second wire was passed through the central lumen successfully. After confirming the wire's position via intravascular ultrasound, we inflated a drug-eluting balloon, subsequently obtaining a successful angiographic result. The second case was a 78-year-old woman diagnosed with non-ST segment elevation myocardial infarction. Coronary angiography revealed tight stenosis at the ostial left anterior descending artery with a previous stent deployed from the left main to the circumflex artery. Owing to the excessive overhanging stent into the aorta, the wire could not be advanced into the stent's central lumen. However, with the facilitation of a dual-lumen microcatheter, a second wire successfully passed through the stent's central lumen. Finally, the patient received a successful PCI with a stent.ConclusionA dual-lumen microcatheter-facilitated wiring technique may be useful in overcoming wiring difficulty caused by the excessive protrusion of an aorto-ostial stent into the aorta.

Parent Vessel Occlusion via the Balloon-Assisted, Dual Microcatheter Technique.

Project description:Endovascular parent vessel sacrifice is an established technique used to treat vascular pathology and tumor infiltration of blood vessels. In high-flow vessels, such as the carotid artery or vertebral artery, distal coil migration and embolization are concerns. A method to mitigate this risk would improve the safety profile of the procedure. Five patients undergoing parent vessel sacrifice were retrospectively identified (from June 2018 to May 2019) for the purpose of illustrating this high-flow parent vessel occlusion technique. The technique utilizes a proximal dual-lumen balloon microcatheter inflated for blood flow arrest. The balloon-assisted, dual microcatheter technique is useful for occlusion of high-flow parent arteries. Because it utilizes both flow arrest and a tethered coil backstop, precise occlusion of a vessel can be achieved.

Project description:Background and aimsSerial stent placement may be necessary during endoscopic interventions, but the passage of a guidewire alongside an initial stent can be challenging, time-consuming, and sometimes unsuccessful. We describe a modification of a cytology brush catheter to allow simultaneous placement of 2 guidewires to facilitate serial stent placement and demonstrate its application in different scenarios.MethodsThis is a retrospective series of 3 patients with different conditions (acute cholecystitis, pancreas pseudocyst, and severe biliary stricture) in whom placement of a second guidewire facilitated serial stent placement. A step-by-step demonstration of the technique is provided.ResultsSerial stent placement was successful in all patients without adverse events.ConclusionsA modified cytology brush catheter can be used to deliver 2 guidewires simultaneously during ERCP and EUS procedures. This technique may improve procedural efficiency, maintain a safety track, and augment therapy in certain situations.

A Simple and Effective Phosphine-Doping Technique for Solution-Processed Nanocrystal Solar Cells.

Project description:Solution-processed cadmium telluride (CdTe) nanocrystal (NC) solar cells offer the advantages of low cost, low consumption of materials and large-scale production via a roll-to-roll manufacture process. Undecorated CdTe NC solar cells, however, tend to show inferior performance due to the abundant crystal boundaries within the active CdTe NC layer. The introduction of hole transport layer (HTL) is effective for promoting the performance of CdTe NC solar cells. Although high-performance CdTe NC solar cells have been realized by adopting organic HTLs, the contact resistance between active layer and the electrode is still a large problem due to the parasitic resistance of HTLs. Here, we developed a simple phosphine-doping technique via a solution process under ambient conditions using triphenylphosphine (TPP) as a phosphine source. This doping technique effectively promoted the power conversion efficiency (PCE) of devices to 5.41% and enabled the device to have extraordinary stability, showing a superior performance compared with the control device. Characterizations suggested that the introduction of the phosphine dopant led to higher carrier concentration, hole mobility and a longer lifetime of the carriers. Our work presents a new and simple phosphine-doping strategy for further improving the performance of CdTe NC solar cells.

Surface Alloying in Silver-Cobalt through a Second Wave Solution Combustion Synthesis Technique.

Project description:Herein, we report the synthesis of silver-cobalt nanopowders using three different modes of solution combustion synthesis, and we present the effects of the synthesis conditions on particle morphology. The synthesized nanoparticles were characterized using X-ray diffraction (XRD), Scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR), UV-Visible spectrophotometer (UV-vis), Transmission electron microscopy (TEM), and X-Ray Photoelectron Spectroscopy (XPS) to understand the structural and elemental properties. When Co is synthesized over Ag in a second wave of combustion, peak shifts observed in XRD and XPS show a change in the cell parameters and prove the existence of a strong electronic interaction between Ag and Co. Better control of mixing and alloying through the second wave combustion synthesis mode (SWCS) was evident. The sequence of combustion affects the structure and composition of the material. SWCS reduces the amount of carbon content, as compared to single-stage combustion, and the combustion of carbon is followed by a rearrangement of atoms.
