PriME-PGx: La Princesa University Hospital Multidisciplinary Initiative for the Implementation of Pharmacogenetics.

ABSTRACT: The implementation of clinical pharmacogenetics in daily practice is limited for various reasons. Today, however, it is a discipline in full expansion. Accordingly, in the recent times, several initiatives promoted its implementation, mainly in the United States but also in Europe. In this document, the genotyping results since the establishment of our Pharmacogenetics Unit in 2006 are described, as well as the historical implementation process that was carried out since then. Finally, this progress justified the constitution of La Princesa University Hospital Multidisciplinary Initiative for the Implementation of Pharmacogenetics (PriME-PGx), promoted by the Clinical Pharmacology Department of Hospital Universitario de La Princesa (Madrid, Spain). Here, we present the initiative along with the two first ongoing projects: the PROFILE project, which promotes modernization of pharmacogenetic reporting (i.e., from classic gene-drug pair reporting to complete pharmacogenetic reporting or the creation of pharmacogenetic profiles specific to the Hospital's departments) and the GENOTRIAL project, which promotes the communication of relevant pharmacogenetic findings to any healthy volunteer participating in any bioequivalence clinical trial at the Clinical Trials Unit of Hospital Universitario de La Princesa (UECHUP).

Project description:ObjectivesIn March 2020, the World Health Organization declared the coronavirus disease 2019 (COVID-19) a pandemic. In absence of official recommendations, implementing daily multidisciplinary team (MDT) COVID-19 meetings was urgently needed. Our aim was to describe our initial institutional standard operating procedures for implementing these meetings, and their impact on daily practice.MethodsAll consecutive patients who were hospitalized in our institution due to COVID 19, from March 31 to April 15, 2020, were included. Criteria to be presented at MDT meetings were defined as a proven COVID-19 by PCR or strongly suspected on CT scan, requiring hospitalization and treatment not included in the standard of care. Three investigators identified the patients who met the predefined criteria and compared the treatment and outcomes of patients with predefined criteria that were presented during MDT meeting with those not presented during MDT meeting. COVID-19 MDT meeting implementation and adhesion were also assessed by a hospital medical staff survey.ResultsIn all, 318 patients with confirmed or suspected COVID-19 were examined in our hospital. Of these, 230 (87%) were hospitalized in a COVID-19 unit, 91 (40%) of whom met predefined MDT meeting criteria. Fifty (55%) patients were presented at a MDT meeting versus 41 (45%) were not. Complementary exploration and inclusion in the CorImmuno cohort were higher in MDT meeting group (respectively 35 vs. 15%, P=0.03 and 80 versus 49%, P=0.0007). Prescription of hydrocortisone hemisuccinate was higher in group of patients not presented during MDT meeting (24 vs. 51%, P=0.007). Almost half of the patients fulfilling the inclusion criteria were not presented at MDT meeting, which can be partly explained by technical software issues.ConclusionsMultidisciplinary COVID-19 meetings helped implementing a single standard of care, avoided using treatments that were untested or currently being tested, and facilitated the inclusion of patients in prospective cohorts and therapeutic trials.

Project description:BackgroundThe University of California's Diabetes Prevention Program (UC DPP) Initiative was implemented systemwide to address diabetes and obesity risk on all 10 campuses. As little is known about implementing lifestyle change programs in university settings, we examined implementation partners' (i.e., UC DPP leaders and campus leads) perceptions of factors influencing program success on UC campuses.MethodsWe conducted qualitative interviews with UC DPP leaders and campus leads to examine challenges and opportunities with university-based DPP delivery models. Interviews were recorded, professionally transcribed, and reviewed in detail by the research team. Transcripts were analyzed using rapid qualitative analysis (RQA). The study was approved by the UCLA Institutional Review Board. All implementation partners provided verbal informed consent.ResultsTwenty-six implementation partners (8 UC DPP leaders and 18 campus leads) completed interviews in 2021. Seven themes were identified as critical for implementation, including (1) marketing and recruitment (i.e., market and recruit broadly through established channels as well as target at-risk populations); (2) enrollment (i.e., offer the program during convenient times and let participants know what to expect); (3) use an adaptable, evidence-based program; (4) secure funding for the program, participants, lifestyle coaches, and space; (5) hire experienced and dedicated staff and lifestyle coaches; (6) ensure leadership support; and (7) utilize campus linkages and resources. Perceptions of challenges faced with respect to these themes are also described.ConclusionsThis is one of the first studies to examine the challenges and opportunities of delivering an intensive lifestyle change program across 10 university sites. Understanding factors that enhance success of university-based diabetes prevention programs can facilitate UC DPP efforts and help inform delivery strategies of health and wellness programs across other university settings more broadly.

Project description:BACKGROUND:Men die earlier than women in Germany. Men also have impaired access to cancer screening compared to women. OBJECTIVES:Our Movember campaign 2019 at University Hospital Frankfurt (UKF) aimed at improving health care awareness in the context of prostate cancer checkup. MATERIALS AND METHODS:In November 2019, every male employee of the UKF with a minimum age of 45 yrs (or 40 yrs with a first degree relative with prostate cancer) was offered a free prostate cancer checkup. This checkup contained digital rectal examination (DRE), transrectal ultrasound and PSA (prostata-specific antigen) testing. RESULTS:Overall, 121/840 employees (14.4%) participated in the Movember campaign. A first degree relative with prostate cancer was reported in overall by 14% of the participants (n?=?17). At least one prior prostate cancer check up had 33%. A total of 2.5% (n?=?3) had one prior negative prostate biopsy. Median age was 54 yrs (interquartile range 50-58). Median PSA level was 0.9?ng/ml and median free-PSA 0.3?ng/ml. A suspicious DRE was found in 5% (n?=?6). After stratification according to age (??50 yrs vs. >?50 yrs), participants over 50 yrs had a significantly higher PSA level (1.0?ng/ml vs. 0.7?ng/ml, p?<?0.01) and had more frequently at least one prior prostate cancer checkup in the past (42.0 vs. 12.1%, p?<?0.01). All suspicious DREs were in the cohort >?50 yrs. Overall, 32.2% (n?=?39) had at least a suspicious checkup. A total of 3.3% (n?=?4) had suspicious PSA levels. 17.4% (n?=?21) of the participants had a suspicious PSA ratio (<?20%) only. During follow-up, 6 prostate biopsies were performed, with the detection of one case of intermediate-risk prostate cancer (Gleason 3?+?4, pT3a, pPn1, pNx, R0). CONCLUSION:Overall, 121 employees participated in our Movember Prostate cancer checkup campaign with measurement of the PSA level. Suspicious results were recorded in 32.2%. One employee was diagnosed and successfully treated with an intermediate-risk prostate cancer.

Project description:BackgroundThe lack of a multidisciplinary approach to the management of cancer patients in most parts of Pakistan is a long-standing and major concern. To overcome this disparity, we started a student-run initiative to facilitate the establishment of multidisciplinary tumor boards (MTBs) in oncology, titled "Tumor Board Establishment Facilitation Forum (TEFF)". The objectives of this study were to evaluate the clinical and academic impact of TEFF on cancer care for patients and student education, respectively.MethodsThe formation of TEFF was based on the Theory of Change model. We conducted a needs assessment based on existing literature, physical evaluation of wards, and consultation with senior academic faculty members. The logic model was refined through multiple meetings between stakeholders. All engagements of TEFF described in this manuscript are limited to the Dow Medical College and its affiliated tertiary care hospital, Dr. Ruth K. M. Pfau Civil Hospital in Karachi, Pakistan. To gauge TEFF's impact, we used administrative data generated between October 2021 to March 2024 to evaluate predefined outcomes (number of MTBs, cases, educational interventions, and research).ResultsThe organizational structure comprised of 6 specialized departments: Communications, Operations, Media, Integrated Development, Research and Records, and Finance. We conducted 18 educational sessions for medical students about career guidance, research, cancer awareness; and 4 cancer awareness campaigns. TEFF facilitated the formation of 4 MTBs: breast, head and neck, gynecology, and pediatrics. Across these, 105 cases were discussed in 50 meetings. TEFF provided leadership opportunities, allowed familiarization with oncology, raised awareness of challenges associated with cancer care, allowed networking, and inculcated research-related skills and educational value through its MTBs.ConclusionMedical students can contribute significantly to clinical care at the undergraduate level. For countries/regions struggling to provide multidisciplinary cancer care, TEFF's model serves as a blueprint for a viable solution.
