Surveillance and prevalence of antimicrobial resistant bacteria from public settings within urban built environments: Challenges and opportunities for hygiene and infection control.

ABSTRACT: Antimicrobial resistant (AMR) bacteria present one of the biggest threats to public health; this must not be forgotten while global attention is focussed on the COVID-19 pandemic. Resistant bacteria have been demonstrated to be transmittable to humans in many different environments, including public settings in urban built environments where high-density human activity can be found, including public transport, sports arenas and schools. However, in comparison to healthcare settings and agriculture, there is very little surveillance of AMR in the built environment outside of healthcare settings and wastewater. In this review, we analyse the existing literature to aid our understanding of what surveillance has been conducted within different public settings and identify what this tells us about the prevalence of AMR. We highlight the challenges that have been reported; and make recommendations for future studies that will help to fill knowledge gaps present in the literature.

Project description:BackgroundEffective public health surveillance systems are crucial for early detection and response to outbreaks. In 2016, Kenya transitioned its surveillance system from a standalone web-based surveillance system to the more sustainable and integrated District Health Information System 2 (DHIS2). As part of Global Health Security Agenda (GHSA) initiatives in Kenya, training on use of the new system was conducted among surveillance officers. We evaluated the surveillance indicators during the transition period in order to assess the impact of this training on surveillance metrics and identify challenges affecting reporting rates.MethodsFrom February to May 2017, we analysed surveillance data for 13 intervention and 13 comparison counties. An intervention county was defined as one that had received refresher training on DHIS2 while a comparison county was one that had not received training. We evaluated the impact of the training by analysing completeness and timeliness of reporting 15 weeks before and 12 weeks after the training. A chi-square test of independence was used to compare the reporting rates between the two groups. A structured questionnaire was administered to the training participants to assess the challenges affecting surveillance reporting.ResultsThe average completeness of reporting for the intervention counties increased from 45 to 62%, i.e. by 17 percentage points (95% CI 16.14-17.86) compared to an increase from 49 to 52% for the comparison group, i.e. by 3 percentage points (95% CI 2.23-3.77). The timeliness of reporting increased from 30 to 51%, i.e. by 21 percentage points (95% CI 20.16-21.84) for the intervention group, compared to an increase from 31 to 38% for the comparison group, i.e.by 7 percentage points (95% CI 6.27-7.73). Major challenges for the low reporting rates included lack of budget support from government, lack of airtime for reporting, health workers strike, health facilities not sending surveillance data, use of wrong denominator to calculate reporting rates and surveillance officers having other competing tasks.ConclusionsTraining plays an important role in improving public health surveillance reporting. However, to improve surveillance reporting rates to the desired national targets, other challenges affecting reporting must be identified and addressed accordingly.

Project description:BackgroundAedes aegypti is the main vector of the dengue, Zika and several other arboviruses. It is highly adapted to urbanized environments and can be found worldwide. Mosquito population control is considered the best strategy for fighting mosquito-borne diseases, making an understanding of their population dynamics vital for the development of more effective vector control programs. This study therefore sought to investigate how different levels of urbanization affect Aedes aegypti populations and modulate population structure in this species with the aid of wing geometric morphometrics.MethodsSpecimens were collected from eleven locations in three areas with distinct levels of urbanization in the city of São Paulo, Brazil: conserved, intermediate and urbanized. The right wings of female mosquitoes collected were removed, and photographed and digitized. Canonical variate analysis and Mahalanobis distance were used to investigate the degree of wing-shape dissimilarity among populations. Thin-plate splines were calculated by regression analysis of Canonical Variation Analysis scores against wing-shape variation, and a cross-validated reclassification was performed for each individual; a neighbor-joining tree was then constructed.ResultsMetapopulation and individual population analysis showed a clear segregation pattern in the Canonical Variation Analysis. Pairwise cross-validated reclassification yielded relatively high scores considering the microgeographical scale of the study and the fact that the study populations belong to the same species. The neighbor-joining tree showed that mosquitoes in the intermediate urban area segregated in the metapopulation and individual population analyses. Our findings show significant population structuring in Aedes aegypti mosquitoes in the areas studied. This is related to the different degrees of urbanization in the areas where the specimens were collected along with their geographical location.ConclusionsUrbanization processes in the study areas appear to play an important role in microevolutionary processes triggered by man-made modifications in the environment, resulting in a previously unknown population structuring pattern of major epidemiological importance.

Project description:Multisectoral actions for health, defined as actions undertaken by non-health sectors to protect the health of the population, are essential in the context of inter-linkages between three dimensions of sustainable development: economic, social, and environmental. These multisectoral actions can address the social and economic factors that influence the health of a population at the local, national, and global levels. This editorial identifies the challenges, opportunities and capacity development for effective multisectoral actions for health in a complex policy environment. The root causes of the challenges lie in poor governance such as entrenched political and administrative corruption, widespread clientelism, lack of citizen voice, weak social capital, lack of trust and lack of respect for human rights. This is further complicated by the lack of government effectiveness caused by poor capacity for strong public financial management and low levels of transparency and accountability which leads to corruption. The absence of or rapid changes in government policies, and low salary in relation to living standards result in migration out of qualified staff. Tobacco, alcohol and sugary drink industries are major risk factors for non-communicable diseases (NCDs) and had interfered with health policy through regulatory capture and potential law suits against the government. Opportunities still exist. Some World Health Assembly (WHA) and United Nations General Assembly (UNGA) resolutions are both considered as external driving forces for intersectoral actions for health. In addition, Thailand National Health Assembly under the National Health Act is another tool providing opportunity to form trust among stakeholders from different sectors. Capacity development at individual, institutional and system level to generate evidence and ensure it is used by multisectoral agencies is as critical as strengthening the health literacy of people and the overall good governance of a country.
