Project description:BackgroundUpper eyelid retraction may develop due to various neurogenic, myogenic, and mechanistic causes. Although the development of lagophthalmus after rhinoplasty has been described in the literature, upper eyelid retraction after uncomplicated rhinoplasty has not been reported before.Case presentationThis report presents a case of unilateral upper eyelid retraction observed six days after uncomplicated rhinoplasty. A 40-year-old female patient presented with left upper eyelid retraction on the sixth day of uncomplicated closed-approach structural rhinoplasty. The patient was able to close her eyelids completely and eye movements were normal in all directions of gaze. Anterior and posterior segment examinations were in normal range. We prescribed artificial tear drop and night-time liquid tear gel. After four weeks of conservative treatment, the upper eyelid retraction was improved without any complication. No recurrence was observed at 6-month follow-up.ConclusionThis is the first case presenting with upper eyelid retraction without lagophthalmus after uncomplicated rhinoplasty. Ophthalmologists should be aware that such a complication may occur quite rarely and may regress without sequelae with conservative treatment.

Project description:Negatively impacting the aesthetics of the face, nasal deformities can be attributed to many congenital or acquired causes, of which muscular hypertrophy is uncustomary. In this article, we narrate a case of a 26-year-old woman with a prominently wide mid-vault, in addition to other abnormalities of the nose. It was thought to originate from soft tissue because it was thick and firm to palpation. Open rhinoplasty was done where hypertrophy of the paired transverse nasalis muscle was observed and resected. The patient enjoyed a safe postoperative period with excellent cosmetic results and minimal effect on the nasal function in terms of breathing and pronunciation. We recommend this approach to any patient with a similar disfigurement due to its simplicity and ability to correct other co-existing nasal disfigurements.

Project description:PurposeTo evaluate the effectiveness, tolerability, and safety of the Nictavi Tarsus Patch™ (NTP) in inducing temporary eyelid closure for the management of lagophthalmos in the pediatric and young adult population.MethodsWe prospectively enrolled 20 patients <21 years of age who had previously been managed for lagophthalmos to trial the NTP in clinic. Inter-palpebral fissure distance (IPFD) was compared before and after the placement of the NTP in the eyes-closed position using paired t-tests. Subjects then underwent a 3-night home trial with the NTP, and parent and subject perceptions of effectiveness, comfort, and complications with the patch were analyzed using Likert scale survey questions.ResultsTwenty subjects ages 2-20 years with paralytic (65%) and non-paralytic (35%) lagophthalmos were enrolled. The NTP improved lagophthalmos from a mean pre-placement IPFD of 3.3 mm to post-placement IPFD of 0.4 mm (p < 0.01). Overall, 80% of subjects achieved successful eyelid closure defined as ≤1 mm of post-placement IPFD. When stratified by subtype, 100% of subjects with paralytic lagophthalmos achieved successful eyelid closure compared to 71% of subjects with non-paralytic lagophthalmos. On a scale of 1 (worst) to 5 (best), parents rated the NTP at 4.3±0.7 for comfort while wearing, 4.3±1.0 for comfort in removing, 4.6±0.7 for ease of use, and 4.3±0.9 for effectiveness. Ninety-three percent of parents reported preferring NTP to other eyelid closure methods previously tried and indicated that they would use it again.ConclusionThe NTP is an effective, tolerable, and safe method of eyelid closure for children and young adults.

Project description:Metastatic malignant lesions of the eyelids are less than 1% of all eyelid tumors. Eyelid metastasis from the cervix, particularly the first sign of metastasis, has not been reported. A female presented to an ophthalmologist with a gradually increasing mass on the lateral edge of the lower eyelid for 2 months and was diagnosed with stage IB1 cervical cancer 3 years ago. We performed wide local excision of the eyelid mass. The mass was histologically and immunohistochemically similar to cervical cancer. Upon metastatic examination, no lesions involving other sites or lymph nodes were found. Subsequently, local radiation therapy yielded the desired results. In the present case, the possibility of metastasis from the cervix was not considered for the first time. More attention should be paid when identifying unexplained masses in patients with a history of malignant tumors.

Project description:Congenital eyelid imbrication syndrome (CEIS) is an extremely rare, benign, transient, self-limiting eyelid malposition disorder. The classic triad of signs in patients with a CEIS consists of bilateral upper eyelids overriding the lower eyelids when child was in sleep, bilateral medial and lateral canthal tendon laxity and tarsal conjunctival hyperemia. We report a third case of congenital combined eyelid imbrication and floppy eyelid syndrome in healthy neonate that was resolved within a week with conservative treatment.

Project description:PurposeTo report a case of lagophthalmos caused by cicatricial adhesion of orbital adipose tissue to the orbital roof.ObservationsA 23-year-old female was presented with right lagophthalmos. Five months prior to consult at our clinic, she suffered from a penetrating trauma to the frontal lobe of the brain through the right orbital roof with cerebrospinal fluid leakage. Decompressive craniectomy was performed immediately after the injury using a coronal incision, which was followed by reconstruction with an artificial bone 1 month later. On examination at our clinic, she showed right exposure keratopathy with best corrected visual acuity of 20/100 due to corneal opacity. The palpebral contracted scar was first elongated using Z-plasty technique but excursion of the upper eyelid under a finger force assistance was insufficient to eliminate lagophthalmos. However, complete eyelid closure under a finger force assistance was achieved after sharp dissection of the cicatrized adipose tissue from the orbital roof. An autogenous dermis-fat was grafted on the orbital roof and superior orbital rim to avoid adhesion of orbital adipose tissue onto the bone again.Conclusion and importanceCicatricial adhesion of an orbital adipose tissue to the orbital roof is one of the possible causes of posttraumatic lagophthalmos in patients with an orbital roof fracture. Surgeons need to be aware of this condition in planning of surgical repair when such a fracture is encountered.

Project description:Mucosal cysts are rare late complications of rhinoplasty, appearing anywhere from weeks to years post-surgery. The cyst's location can cause both cosmetic and functional issues. Definitive treatment requires complete surgical excision of the cyst and its capsule to prevent recurrence. A 32-year-old man presented with a 2 × 2-cm painless, soft nasal mass 3 years after open rhinoplasty. The mass had gradually enlarged over 6 months, leading to breathing difficulties. Physical examination revealed a pollybeak deformity. Imaging confirmed a well-defined cystic lesion. The cyst was excised through an open rhinoplasty approach, followed by reconstruction using a costal cartilage graft. The postoperative period was uneventful, with no recurrence or issues at 12 months follow-up. We highlight the importance of meticulous surgical techniques in rhinoplasty to reduce mucosal cyst formation. For cysts >1 cm, open rhinoplasty is preferred due to better reconstruction and lower recurrence rates.

This dynamic region plays a pivotal role in the expression of mood, emotion, and character, thus making it the most relevant component of the facial esthetic and functional unit. Any change in the periocular unit leads to facial imbalance and functional disharmony, leading both the young and the elderly to seek consultation, thus making blepharoplasty the surgical procedure of choice for both cosmetic and functional amelioration. The applied anatomy, indications of upper eyelid blepharoplasty, preoperative workup, surgical procedure, postoperative care, and complications would be discussed in detail in this review article.

Project description:In this report, we describe a rare case of a tick egg-laying infestation of the eyelid. A 7-year-old girl consulted our hospital after a trip in Thailand for an itchy and painful eyelid. The first examination at the slit lamp showed the tick's body attached to the left superior eyelid margin, surrounded by its eggs. The tick detached itself afterward and a tobramycin prophylactic treatment was introduced. The patient was put under surveillance and no signs of a tick-borne disease were described during the follow-up.
