Optimization of collimator angles in dual-arc volumetric modulated arc therapy planning for whole-brain radiotherapy with hippocampus and inner ear sparing.

ABSTRACT: To optimize the collimator angles in dual-arc volumetric modulated arc therapy (VMAT) plans for whole-brain radiotherapy with hippocampus and inner ear sparing (HIS-WBRT). Two sets of dual-arc VMAT plans were generated for 13 small-cell lung cancer patients: (1) The collimator angles of arcs 1 and 2 (θ₁/θ₂) were 350°/10°, 350°/30°, 350°/45°, 350°/60°, and 350°/80°, i.e., the intersection angle of θ₁ and θ₂ (Δθ) increased. (2) θ₁/θ₂ were 280°/10°, 300°/30°, 315°/45°, 330°/60°, and 350°/80°, i.e., Δθ = 90°. The conformity index (CI), homogeneity index (HI), monitor units (MUs), and dosimetric parameters of organs-at-risk were analyzed. Quality assurance for Δθ = 90° plans was performed. With Δθ increasing towards 90°, a significant improvement was observed for most parameters. In 350°/80° plans compared with 350°/10° ones, CI and HI were improved by 1.1% and 25.2%, respectively; MUs were reduced by 16.2%; minimum, maximum, and mean doses (D_100%, D_max, and D_mean, respectively) to the hippocampus were reduced by 5.5%, 6.3%, and 5.4%, respectively; D_mean to the inner ear and eye were reduced by 0.7% and 5.1%, respectively. With Δθ kept at 90°, the plan quality was not significantly affected by θ₁/θ₂ combinations. The gamma-index passing rates in 280°/10° and 350°/80° plans were relatively lower compared with the other Δθ = 90° plans. Δθ showed a significant effect on dual-arc VMAT plans for HIS-WBRT. With Δθ approaching 90°, the plan quality exhibited a nearly continuous improvement, whereas with Δθ = 90°, the effect of θ₁/θ₂ combination was insignificant.

Project description:PurposeIn the field of radiation therapy for brain metastases, whole-brain hippocampus-avoidance treatment is commonly employed. this study aims to examine the impact of different head tilt angles on the dose distribution in the whole-brain target area and organs at risk. It also aims to determine the head tilt angle to achieve optimal radiation therapy outcomes.MethodsCT images were collected from 8 brain metastases patients at 5 different groups of head tilt angle. The treatment plans were designed using the volumetric modulated arc therapy (VMAT) technique. The 5 groups of tilt angle were as follows: [0°,10°), [10°,20°), [20°,30°), [30°,40°), and [40°,45°]. The analysis involved assessing parameters such as the uniformity index, conformity index, average dose delivered to the target, dose coverage of the target, hot spots within the target area, maximum dose, and average dose received by organs at risk. Additionally, the study evaluated the correlation between hippocampal dose and other factors, and established linear regression models.ResultsSignificant differences in dosimetric results were observed between the [40°,45°] and [0°,10°) head tilt angles. The [40°,45°] angle showed significant differences compared to the [0°,10°) angle in the average dose in the target area (31.49 ± 0.29 Gy vs. 31.99 ± 0.29 Gy, p=0.016), dose uniformity (1.20 ± 0.03 vs. 1.24 ± 0.03, p=0.016), hotspots in the target area (33.64 ± 0.35 Gy vs. 34.42 ± 0.49 Gy, p=0.016), maximum hippocampal dose (10.73 ± 0.36 Gy vs. 11.66 ± 0.59 Gy, p=0.008), maximum dose in the lens (2.82 ± 1.10 Gy vs. 4.99 ± 0.16 Gy, p=0.016), and average dose in the lens (1.93 ± 0.29 Gy vs. 4.22 ± 0.26 Gy, p=0.008). There is a moderate correlation between the maximum dose in the hippocampi and the PTV length (r=0.49, p=0.001). Likewise, the mean dose in the hippocampi is significantly correlated with the hippocampi length (r=0.34, p=0.04).ConclusionThe VMAT plan with a head tilt angle of [40°,45°] met all dose constraints and demonstrated improved uniformity of the target area while reducing the dose to organs at risk. Furthermore, the linear regression models suggest that increasing the head tilt angle within the current range of [0°,45°] is likely to lead to a decrease in the average hippocampal dose.

Project description:BackgroundThe purpose of the study was to evaluate the dosimetry of the Halcyon in prophylactic cranial irradiation (PCI) with volumetric modulated arc therapy (VMAT) and hippocampal-sparing for small cell lung cancer (SCLC).MethodsFive VMAT plans were designed on CT images of 15 patients diagnosed with SCLC and received PCI. Three plans with two full arcs were generated on the Trilogy and the TrueBeam accelerators, and flattening filter (FF) and flattening filter free (FFF) modes were used on TrueBeam. Two Halcyon plans with two and three full arcs were generated, referred to as H-2A and H-3A, respectively. The prescription dose was 25 Gy in 2.5-Gy fractions. The dose limit for hippocampus were D100 ≤ 9Gy and Dmax ≤ 16Gy. The Wilcoxon matched-paired signed-rank test was used to evaluate the significance of the observed differences between the five plans.ResultsH-2A plans significantly increased the D2 of PTV, and H-3A plans showed comparable or even better target dosimetry (better conformity) compared to the three plans on C-arm accelerators. Compared to T and TB plans, the two Halcyon plans significantly reduced the D100 and mean doses of bilateral hippocampus, the mean doses of eyeballs, and the maximum doses of lenses. D100 of hippocampus was reduced in TrueBeam plans comparing to Trilogy plans. The FFF plans on TrueBeam also represented advantages in Dmean and D100 of hippocampas, Dmean and Dmax of eyeballs, and the Dmax of lenses compared to FF plans. Halcyon plans and TrueBeam plans with FFF mode increased the MUs compared to FF plans. Comparing to H-2A, the H-3A plans exhibited additional dosimetric advantages, including D2, CI and HI of PTV, as well as the maximum and mean doses of hippocampus and eyeballs, and the maximum doses of optic nerves and brainstem. The two Halcyon plans significantly reduced the delivery time and showed the higher gamma passing rate than the three plans of C-arm accelerators.ConclusionsCompared with the C-arm accelerators, the dose of hippocampus and the delivery times on Halcyon are relatively significantly reduced for hippocampal-sparing PCI. Three arcs are recommended for VMAT plans with the Halcyon in hippocampal-sparing PCI.

Project description:PurposeTo optimize collimator setting to improve dosimetric quality of pancreas volumetric modulated arc therapy plan for stereotactic body radiation therapy.Materials and methodsFifty-five volumetric modulated arc therapy cases in stereotactic body radiation therapy of pancreas were retrospectively included in this study with internal review board approval. Different from the routine practice of initializing collimator settings with a template, the proposed algorithm simultaneously optimizes the collimator angles and jaw positions that are customized to the patient geometry. Specifically, this algorithm includes 2 key steps: (1) an iterative optimization algorithm via simulated annealing that generates a set of potential collimator settings from 39 cases with pancreas stereotactic body radiation therapy, and (2) a multi-leaf collimator modulation scoring system that makes the final decision of the optimal collimator settings (collimator angles and jaw positions) based on organs at risk sparing criteria. For validation, the other 16 cases with pancreas stereotactic body radiation therapy were analyzed. Two plans were generated for each validation case, with one plan optimized using the proposed algorithm (Planopt) and the other plan with the template setting (Planconv). Each plan was optimized with 2 full arcs and the same set of constraints for the same case. Dosimetric results were analyzed and compared, including target dose coverage, conformity, organs at risk maximum dose, and modulation complexity score. All results were tested by Wilcoxon signed rank tests, and the statistical significance level was set to .05.ResultsBoth plan groups had comparable target dose coverage and mean doses of all organs at risk. However, organs at risk (stomach, duodenum, large/small bowel) maximum dose sparing (D0.1 cc and D0.03 cc) was improved in Planopt compared to Planconv. Planopt also showed lower modulation complexity score, which suggests better capability of handling complex shape and sparing organs at risk .ConclusionsThe proposed collimator settings optimization algorithm successfully improved dosimetric performance for dual-arc pancreas volumetric modulated arc therapy plans in stereotactic body radiation therapy of pancreas. This algorithm has the capability of immediate clinical application.

Project description:PurposeTo create and to validate knowledge-based volumetric modulated arc therapy (VMAT) models for breast cancer treatments without lymph node irradiation.Materials and methodsOne hundred VMAT-based breast plans (manual plans [MP]) were selected to create two knowledge-based VMAT models (breast left and breast right) using RapidPlan™. The plans were generated on Eclipse v15.5 (Varian Medical Systems, Palo Alto, CA) with 6 MV of a Novalis Tx equipped with a high-resolution multileaf collimator. The models were verified based on goodness-of-fit statistics using the coefficients of determination (R 2) and Chi-square (χ2), and the goodness-of-estimation statistics through the mean square error (MSE). Geometrical and dosimetrical constraints were identified and removed from the RP models using statistical evaluation metrics and plots. For validation, 20 plans that integrate the models and 20 plans that do not were reoptimized with RP (closed and opened validation). Dosimetrical parameters of interest were used to compare MP versus RP plans for the Heart, Homolateral_Lung, Contralateral_Lung, and Contralateral_Breast. Optimization planning time and user independency were also analyzed.ResultsThe most unfavorable results of R2 in both models for the organs at risk were as follows: for Contralateral_Lung 0.51 in RP right breast (RP_RB) and for Heart 0.60 in RP left breast (RP_LB). The most unfavorable results of χ2 test were: for Contralateral_Breast 1.02 in RP_RB and for Heart 1.03 in RP_LB. These goodness-of-fit results show that no overfitting occurred in either of the models. There were no unfavorable results of mean square error (MSE, all < 0.05) in any of the two models. These goodness-of-estimation results show that the models have good estimation power. For closed validation, significant differences were found in RP_RB for Homolateral_Lung (all P ≤ 0.001), and in the RP_LB differences were found for the heart (all P ≤ 0.04) and for Homolateral_Lung (all P ≤ 0.022). For open validation, no statistically significant differences were obtained in either of the models. RP models had little impact on reducing optimization planning times for expert planners; nevertheless, the result showed a 30% reduction time for beginner planners. The use of RP models generates high-quality plans, without differences from the planner experience.ConclusionTwo RP models for breast cancer treatment using VMAT were successfully implemented. The use of RP models for breast cancer reduces the optimization planning time and improves the efficiency of the treatment planning process while ensuring high-quality plans.
