Associations between Movement Behaviours and Obesity Markers among Preschoolers Compliant and Non-Compliant with Sleep Duration: A Latent Profile Analysis.

ABSTRACT: This study identifies physical activity (PA) and sedentary behaviour (SB) clusters in preschoolers compliant (C) or non-compliant (NC) with sleep recommendations; and associates these clusters with obesity markers. PA and SB were objectively assessed (Actigraph WGT3-X) in 272 preschoolers (4.4 ± 0.7 years old). Sleep duration was parent-reported, and preschoolers were classified as C (3-4 years old: 600-780 min/day; 5 years old: 540-660 min/day) or NC with sleep recommendations. Body mass index (BMI) and waist circumference (WC) were assessed according to international protocols. Moderate to vigorous physical activity (MVPA) and light physical activity (LPA) were categorized as low/high (<60 min/>60 min/day or <180 min/180 min/day, respectively). SB was defined according to mean values between clusters. Latent profile analysis was performed. Associations between the observed clusters and obesity markers were determined using linear regression (RStudio; 1.3.1073). Four cluster solutions for C and NC preschoolers were identified. A negative association between C/Low MVPA cluster and BMI, and a positive association between NC/Low MVPA and BMI (β = -0.8, 95%CI = -1.6;-0.1, and β = 0.9, 95%CI = 0.1;1.7, respectively) were observed. No association was seen for SB clusters. Adequate sleep duration may have a protective role for preschoolers' BMI, even if the children do not comply with MVPA recommendations.

Project description:BACKGROUND/OBJECTIVES:An element of obesity prevention is increasing total physical activity energy expenditure. However, this approach does not incorporate the balance of various movement behaviors-physical activity, sedentary behaviors and sleep-across domains of the day. We aimed to identify time-use profiles over work and leisure, termed 'movement behavior profiles' and to investigate their association with obesity. SUBJECTS/METHODS:Eight-hundred-and-seven workers completed (a) thigh accelerometry and diaries to determine their 24-h composition of behaviors (sedentary and standing, light physical activity and moderate-to-vigorous physical activity at work and leisure, and time in bed) and (b) obesity measurements. Movement behavior profiles were determined using latent profile analyses of isometric log-ratios of the 24-h composition, and labeled according to animal movement behavior traits. Linear models were applied to determine the association between profiles and obesity. RESULTS:Four profiles were identified, labeled as "Chimpanzees" (n?=?226), "Lions" (n?=?179), "Ants" (n?=?244), and "Koalas" (n?=?158). "Chimpanzees" work time was evenly distributed between behaviors while their leisure time was predominantly active. Compared to Chimpanzees, "Lions" were more active at work and sedentary during leisure and spent more time in bed; "Ants" were more active at work and during leisure; "Koalas" were more sedentary at work and leisure and spent similar time in bed. With "Chimpanzees" as reference, "Lions" had least favorable obesity indicators: +2.0 (95% confidence interval [CI] 0.6, 3.4) %body fat, +4.3?cm (1.4, 7.3) waist circumference and +1.0 (2.0, 0.0) Body Mass Index (BMI), followed by "Koalas" +2.0 (0.4, 3.7) %body fat, +3.1?cm (0.1, 6.0) waist circumference, and +0.8 (-0.30, 1.94) BMI. No significant differences were found between "Chimpanzees" and "Ants". CONCLUSIONS:Movement behavior profiles across work and leisure time-use compositions are associated with obesity. Achieving adequate balance between work and leisure movement behaviors should be further investigated as a potential obesity prevention strategy.

Project description:IntroductionThe relationship between 24-h movement behaviours (i.e. sleep, sedentary behaviour and physical activity) and adiposity in preschoolers remains unclear. Therefore, this study aims to investigate the associations between 24-h movement behaviours and adiposity in preschoolers making use of compositional data analysis (CoDA).MethodsAustralian preschoolers (3-5 years) from the Early Start Baseline Study wore an ActiGraph accelerometer to assess sedentary behaviour (SB), light physical activity (LPA) and moderate-to-vigorous physical activity (MVPA). Their weight and height were measured using standardized protocols and converted to Body Mass Index (BMI) z-scores using the World Health Organisation growth references. Their parents completed a questionnaire to assess their level of education and the child's sleep duration, age and sex. CoDA was employed to investigate the association between 24-h movement behaviours and adiposity in R.ResultsThis study included 169 preschoolers and their overall 24-h movement behaviour composition was associated with BMI z-scores (F = 5.02, p = 0.002). When examining the association between each movement behaviour relative to the others and BMI z-scores, we observed a statistically significant favourable association for sleep (p = 0.025) and unfavourable association for MVPA (p = 0.010), but not for the other behaviours. As such, reallocating 10 min from sleep or from MVPA, proportionally to all other behaviours was associated with a difference of + 0.031 (95%CI = 0.004,0.06) and -0.085 (95%CI = -0.15,-0.02) in BMI z-score, respectively.ConclusionDespite the association between more time spent in MVPA and higher BMI z-scores, promoting a balanced amount of time in each 24-h movement behaviour-more MVPA, less sedentary time, and sufficient sleep-remains important for overall health. Future studies should address methodological challenges (e.g. recruitment bias that may exist in the parents/children willing to participate versus the general population, recall bias in parent reported sleep duration, or other confounding variables such as diet), use larger and more diverse samples, and consider longitudinal designs. Additionally, focusing on other adiposity indicators, such as waist-to-height ratio or fat percentage, could enhance understanding of these relationships.
