Epidemiological Study of Violence against Children and Its Increase during the COVID-19 Pandemic.

ABSTRACT: The aim of this study was to identify the epidemiological profiles of violence against children, victims, and their aggressors, and their correlations between socioeconomic and demographic factors analyzed before and during the COVID-19 pandemic. This was a cross-sectional, retrospective observational study based on a review of Individual Notification Forms from the Information System for Notifiable Diseases, including child victims of violence, under 18 years, assisted by a pediatric emergency service in Brazil, from 2016-2020. Data were stratified, then statistical analysis was performed using the two-proportion equality test and the Chi-square test, with p < 0.05 and a 95% confidence interval. A total of 609 notifications were analyzed and a prevalence of sexual violence (63.2%) was reported. The prevalent profile of victim was female (76.7%), aged between 2-9 years (38.1%) and 14-18 years (35.6%). The violence occurs in the victim's home (58.9%). The prevalent profile of perpetrator was male (82.4%), young adolescent (59.2%), living as family (64%), mainly the parents (18.4%). No correlation was found between the classified socioeconomic and demographic variables and violence. There was an increase in notifications during the COVID-19 pandemic, compared to the same period in the previous year; self-harm was reported in 59.7% of physical violence in 2020. Prevalence of sexual violence was higher for females, aged between 2-9 and 14-18 years, victimized in their homes, by male offenders, living as family, mainly by their parents. No association was found between child violence and the socioeconomic and demographic.

Project description:ObjectivesThis scoping review aims to assess the situation of violence against women during the COVID-19 pandemic in Latin America, mainly in Brazil.MethodsThis study consists of a Scoping Review based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. To conduct this review, we mapped scientific evidence on domestic violence during the COVID-19 pandemic. The inclusion criteria were: journal articles on domestic violence set in Latin American countries, particularly Brazil, in the context of social isolation during the COVID-19 pandemic, published between March 2020 and June of 2021. Articles that were not published in journals, studies focusing on child violence or the elderly population, suicide-related approaches, editorials and letters to readers were excluded. The databases used were: National Library of Medicine (PubMed), Embase, COVID-19 Global literature on coronavirus disease, Scopus Preview, Web of Science, LILAC's, Database of the best Evidence-Based Health (Epistemonikos) with the following strategic keywords: "domestic violence" "female'/exp OR female" "domestic violence AND women" "SARS-CoV-19") AND ("domestic violence") SARS-CoV-2" OR "2020-nCoV". The databases above were accessed in July 2021. The articles selected from the databases were synthetized following the double-check procedure and the topics that were most relevant to the subject discussed were separated. The articles used were in English, Portuguese, and Spanish.ResultsWe discussed domestic violence against women in the context of social isolation, showing a significant increase and highlighting aggravating factors - financial instability, exacerbation of the female workload, increase in drug and alcohol consumption, lack of access to care services - and statistics that demonstrate the significant increase in countries such as Argentina, Ecuador, Paraguay and Brazil, especially in São Paulo, Rio de Janeiro, Paraná, among other states. Despite the meaningful increase in the number of cases and the aggravating factors for domestic violence against women in Latin America and Brazil during the COVID-19 pandemic, the reality of underreporting, resulting, among others, from limited legal, social and economic support and the lack of well-prepared intersectoral services may be a limitation in this scenario, since the situation can be even more acute. We presented measures adopted in Brazil and alternatives to confront the critical situation of domestic violence against women, aggravated by social isolation during the COVID-19 pandemic.The 18 most relevant articles were read twice by each author, following the "Double-Check" protocol. The articles used addressed important questions around the established goals: (a) characteristics of the current scenarios in Latin America and, specifically, in Brazil, with regard to violence against women; (b) factors that intensify this type of violence; (c) numbers that demonstrate an increase in cases; and finally (e) strategies to combat domestic violence against women. A total of 595 articles were found, then, based on the abstracts presented, 18 articles were used to write this scoping review.

Project description:BackgroundGender-based violence (GBV) includes any physical, sexual, psychological, economic harms, and any suffering of women in the form of limiting their freedom in personal or social life. As a global crisis, COVID-19 has exposed women to more violence, which requires serious actions. This work aims to review the most critical dimensions of the GBV against women, effective factors on it, and strategies for combating it during the COVID-19 pandemic in order to provide recommendations for future pandemics.MethodsThis study was conducted based on PRISMA-ScR. First, PubMed, Embase, Scopus, Web of Science, ProQuest, and Google Scholar were searched in April 2021 with no time limitation and location using the related keywords to COVID-19 and GBV. The searched keywords were COVID-19, gender-based violence, domestic violence, sexual violence, women, violence, abuse, and their synonyms in MESH and EMTREE. Duplicates were removed, titles and abstracts were screened, and then the characteristics and main results of included studies were recorded in the data collection form in terms of thematic content analysis.ResultsA total of 6255 records were identified, of which 3433 were duplicates. Based on inclusion criteria 2822 titles and abstracts were screened. Finally, 14 studies were eligible for inclusion in this study. Most of these studies were conducted in the United States, the Netherlands, and Iran, mostly with interventional and qualitative methods.ConclusionsStrengthening ICT infrastructure, providing comprehensive government policies and planning, government economic support, social support by national and international organizations should be considered by countries worldwide. It is suggested that countries provide sufficient ICT infrastructure, comprehensive policies and planning, economic support, social support by collaboration between national and international organizations, and healthcare supporting to manage incidence of GBV against women in future pandemics.

Project description:IntroductionNorovirus infection is a common cause of acute gastroenteritis (AGE). Surveillance activities are important to aid investigation into effective norovirus control strategies, including vaccination. Here, we report ancillary findings related to the incidence, prevalence, and etiology of AGE caused by norovirus in Panama after adjustment of study methodology to comply with national coronavirus disease 2019 (COVID-19) mandates.MethodsIn January 2020, children aged <2 years began enrolling into an epidemiological study in Panama to estimate the burden of norovirus in preparation for evaluating upcoming prevention strategies. This included an observational, longitudinal, community-based AGE surveillance study and a hospital-based AGE surveillance study. For the longitudinal study, healthy children aged 5-18 months were enrolled from January 6 through March 23, 2020, with a follow-up of approximately 6 months. The last participant was contacted on September 23, 2020. For the hospital-based study, starting on January 21, 2020, children aged <2 years who were admitted to the Hospital del Niño Dr. José Renán Esquivel in Panama City due to AGE were evaluated. The last sample was collected on September 29, 2020. Collected stool samples were tested for norovirus as well as astrovirus, sapovirus, and various enteropathogens. Unfortunately, this study was disrupted by the subsequent implementation of disease transmission control procedures for the COVID-19 pandemic, and the study methodology was revised to comply with COVID-19 mandates.ResultsIn the longitudinal surveillance cohort [N = 400 (Chiriquí, n = 239; Panama, n = 161)], a total of 185 AGE episodes were documented (Chiriquí, n = 85; Panama, n = 100) resulting in an overall AGE incidence of 11.6 (95% CI: 9.99-13.4) episodes per 100 child-months. The norovirus-related AGE incidence was 0.3 (95% CI: 0.10-0.73) episodes per 100 child-months (5/185 AGE episodes) and the prevalence of norovirus was 4.6% (13/282 stool samples collected). In the hospital-based surveillance cohort, at least one pathogen was detected in 50% of samples (44/88 stool samples collected) and norovirus prevalence was 6.8% (6/88 stool samples collected).DiscussionThis report demonstrates how the occurrence of the COVID-19 pandemic hindered the conduct of clinical trials. However, this also created unique research opportunities to investigate the potential impact of pandemic control measures on the etiology of infectious diarrheal disease.
