Low-Dose High-Resolution TOF-PET Using Ionization-activated Multi-State Low-Z Detector Media.

ABSTRACT: We propose PET scanners using low atomic number media that undergo a persistent local change of state along the paths of the Compton recoil electrons. Measurement of the individual scattering locations and angles, deposited energies, and recoil electron directions allows using the kinematical constraints of the 2-body Compton scattering process to perform a statistical time-ordering of the scatterings, with a high probability of precisely identifying where the gamma first interacted in the detector. In these cases the Line-of-Response is measured with high resolution, determined by the underlying physics processes and not the detector segmentation. There are multiple such media that act through different mechanisms. As an example in which the change of state is quantum-mechanical through a change in molecular configuration, rather than thermodynamic, as in a bubble chamber, we present simulations of a two-state photoswitchable organic dye, a 'Switchillator', that is activated to a fluorescent-capable state by the ionization of the recoil electrons. The activated state is persistent, and can be optically excited multiple times to image individual activated molecules. Energy resolution is provided by counting the activated molecules. Location along the LOR is implemented by large-area time-of-flight MCP-PMT photodetectors with single photon time resolution in the tens of ps and sub-mm spatial resolution. Simulations indicate a large reduction of dose.

Project description:BACKGROUND:Positron emission tomography (PET) is increasingly applied for in vivo brown adipose tissue (BAT) research in healthy volunteers. To limit the radiation exposure, the injected 18F-FDG tracer dose should be as low as possible. With simultaneous PET/MR imaging, the radiation exposure due to computed tomography (CT) can be avoided, but more importantly, the PET acquisition time can often be increased to match the more extensive magnetic resonance (MR) imaging protocol. The potential gain in detected coincidence counts, due to the longer acquisition time, can then be applied to decrease the injected tracer dose. The aim of this study was to investigate the minimal 18F-FDG dose for a 10-min time-of-flight (TOF) PET/MR acquisition that would still allow accurate quantification of supraclavicular BAT volume and activity. METHODS:Twenty datasets from 13 volunteers were retrospectively included from a prospective clinical study. PET emission datasets were modified to simulate step-wise reductions of the original 75 MBq injected dose. The resulting PET images were visually and quantitatively assessed and compared to a 4-min reference scan. For the visual assessment, the image quality and artifacts were scored using a 5-point and a 3-point Likert scale. For the quantitative analysis, image noise and artifacts, BAT metabolic activity, BAT metabolic volume (BMV), and total BAT glycolysis (TBG) were investigated. RESULTS:The visual assessment showed still good image quality for the 35%, 30%, and 25% activity reconstructions with no artifacts. Quantitatively, the background noise was similar to the reference for the 35% and 30% activity reconstructions and the artifacts started to increase significantly in the 25% and lower activity reconstructions. There was no significant difference in supraclavicular BAT metabolic activity, BMV, and TBG between the reference and the 35% to 20% activity reconstructions. CONCLUSIONS:This study indicates that when the PET acquisition time is matched to the 10-min MRI protocol, the injected 18F-FDG tracer dose can be reduced to approximately 19 MBq (25%) while maintaining image quality and accurate supraclavicular BAT quantification. This could decrease the effective dose from 1.4 mSv to 0.36 mSv.

Project description:Depth-encoding detectors with single-ended readout provide a practical, cost-effective approach for constructing high-resolution and high-sensitivity PET scanners. However, the current iteration of such detectors uses a uniform glass light-guide to achieve depth encoding, resulting in nonuniform performance throughout the detector array due to suboptimal intercrystal light sharing. We introduce Prism-PET, a single-ended-readout PET detector module with a segmented light-guide composed of an array of prismatoids that introduce enhanced, deterministic light sharing. Methods: High-resolution PET detector modules were fabricated with single-ended readout of polished multicrystal lutetium yttrium orthosilicate scintillator arrays directly coupled 4-to-1 and 9-to-1 to arrays of 3 × 3 mm silicon photomultiplier pixels. Each scintillator array was coupled at the nonreadout side to a light-guide (one 4-to-1 module with a uniform glass light-guide, one 4-to-1 Prism-PET module, and one 9-to-1 Prism-PET module) to introduce intercrystal light sharing, which closely mimics the behavior of dual-ended readout, with the additional benefit of improved crystal identification. Flood histogram data were acquired using a 3-MBq 22Na source to characterize crystal identification and energy resolution. Lead collimation was used to acquire data at specific depths to determine depth-of-interaction (DOI) resolution. Results: The flood histogram measurements showed excellent and uniform crystal separation throughout the Prism-PET modules, whereas the uniform glass light-guide module had performance degradation at the edges and corners. A DOI resolution of 5.0 mm full width at half maximum (FWHM) and an energy resolution of 13% FWHM were obtained in the uniform glass light-guide module. By comparison, the 4-to-1 coupled Prism-PET module achieved a DOI resolution of 2.5 mm FWHM and an energy resolution of 9% FWHM. Conclusion: PET scanners based on our Prism-PET modules with segmented prismatoid light-guide arrays can achieve high and uniform spatial resolution (9-to-1 coupling with ∼1-mm crystals), high sensitivity (20-mm-thick detectors and intercrystal Compton scatter recovery), good energy and timing resolutions (using polished crystals and after applying DOI correction), and compact size (depth encoding eliminates parallax error and permits smaller ring-diameter).

Project description:PurposeDigital PET involving silicon photomultipliers (SiPM) provides an enhanced time-of-flight (TOF) resolution as compared with photomultiplier (PMT)-based PET, but also a better prevention of the count-related rises in dead time and pile-up effects mainly due to smaller trigger domains (i.e., the detection surfaces associated with each trigger circuit). This study aimed to determine whether this latter property could help prevent against deteriorations in TOF resolution and TOF image quality in the wide range of PET count rates documented in clinical routine.MethodsVariations, according to count rates, in timing resolution and in TOF-related enhancement of the quality of phantom images were compared between the first fully digital PET (Vereos) and a PMT-based PET (Ingenuity). Single-count rate values were additionally extracted from the list-mode data of routine analog- and digital-PET exams at each 500-ms interval, in order to determine the ranges of routine PET count rates.ResultsRoutine PET count rates were lower for the Vereos than for the Ingenuity. For Ingenuity, the upper limits were estimated at approximately 21.7 and 33.2 Mcps after injection of respectively 3 and 5 MBq.kg-1 of current 18F-labeled tracers. At 5.8 Mcps, corresponding to the lower limit of the routine count rates documented with the Ingenuity, timing resolutions provided by the scatter phantom were 326 and 621 ps for Vereos and Ingenuity, respectively. At higher count rates, timing resolution was remarkably stable for Vereos but exhibited a progressive deterioration for Ingenuity, respectively reaching 732 and 847 ps at the upper limits of 21.7 and 33.2 Mcps. The averaged TOF-related gain in signal/noise ratio was stable at approximately 2 for Vereos but decreased from 1.36 at 5.8 Mcps to 1.14 and 1.00 at respectively 21.7 and 33.2 Mcps for Ingenuity.ConclusionContrary to the Ingenuity PMT-based PET, the Vereos fully digital PET is unaffected by any deterioration in TOF resolution and consequently, in the quality of TOF images, in the wide range of routine PET count rates. This advantage is even more striking with higher count-rates for which the preferential use of digital PET should be further recommended (i.e., dynamic PET recording, higher injected activities).

Project description:BackgroundPositron emission tomography (PET) has had a transformative impact on oncological and neurological applications. However, still much of PET's potential remains untapped with limitations primarily driven by low spatial resolution, which severely hampers accurate quantitative PET imaging via the partial volume effect (PVE).PurposeWe present experimental results of a practical and cost-effective ultra-high resolution brain-dedicated PET scanner, using our depth-encoding Prism-PET detectors arranged along a compact and conformal gantry, showing substantial reduction in PVE and accurate radiotracer uptake quantification in small regions.MethodsThe decagon-shaped prototype scanner has a long diameter of 38.5 cm, a short diameter of 29.1 cm, and an axial field-of-view (FOV) of 25.5 mm with a single ring of 40 Prism-PET detector modules. Each module comprises a 16 × 16 array of 1.5 × 1.5 × 20-mm3 lutetium yttrium oxyorthosillicate (LYSO) scintillator crystals coupled 4-to-1 to an 8 × 8 array of silicon photomultiplier (SiPM) pixels on one end and to a prismatoid light guide array on the opposite end. The scanner's performance was evaluated by measuring depth-of-interaction (DOI) resolution, energy resolution, timing resolution, spatial resolution, sensitivity, and image quality of ultra-micro Derenzo and three-dimensional (3D) Hoffman brain phantoms.ResultsThe full width at half maximum (FWHM) DOI, energy, and timing resolutions of the scanner are 2.85 mm, 12.6%, and 271 ps, respectively. Not considering artifacts due to mechanical misalignment of detector blocks, the intrinsic spatial resolution is 0.89-mm FWHM. Point source images reconstructed with 3D filtered back-projection (FBP) show an average spatial resolution of 1.53-mm FWHM across the entire FOV. The peak absolute sensitivity is 1.2% for an energy window of 400-650 keV. The ultra-micro Derenzo phantom study demonstrates the highest reported spatial resolution performance for a human brain PET scanner with perfect reconstruction of 1.00-mm diameter hot-rods. Reconstructed images of customized Hoffman brain phantoms prove that Prism-PET enables accurate radiotracer uptake quantification in small brain regions (2-3 mm).ConclusionsPrism-PET will substantially strengthen the utility of quantitative PET in neurology for early diagnosis of neurodegenerative diseases, and in neuro-oncology for improved management of both primary and metastatic brain tumors.
