Data on alveolar mandibular bone thickness in Class I skeletal patient with bimaxillary protrusion.

ABSTRACT: The alveolar bone thickness influences both diagnosis and limitation of tooth movement, therefore significance retraction was commonly applied in treating patients with bimaxillary protrusion. This is a retrospective data collection of pre and post treatment lateral cephalographs from 18 to 40 years old patient treated with four premolars extraction. The alveolar mandibular bone thickness was identified in sagittal planes with Image-J software based on cephalometry lateral radiographs. Statistical analysis namely Wilcoxon test and Pearson correlation analysis coefficient were used to understand the correlation of alveolar mandibular bone thickness variables and mandibular incisors position to skeletal profile treated with first premolars extraction are presented. This data is essential for advancing in a further understanding of Class I skeletal patients with bimaxillary protrusion.

Relationship between alveolar-bone morphology at the mandibular incisors and their inclination in adults with low-angle, skeletal class III malocclusion-A retrospective CBCT study.

Project description:ObjectiveTo quantitatively study the effect of the labial inclination of the mandibular central incisors on the surrounding cortical and cancellous-bone morphology among patients with low-angle, skeletal class III malocclusion, by using cone-beam computed-tomography (CBCT) imaging.Materials and methodsThe CBCT images of 60 patients with low-angle, skeletal class III malocclusion were divided into lingual-inclination, upright, and labial-inclination groups. The height of the alveolar bone and the thickness and area of the cortical, cancellous, and total alveolar bone were measured separately on each side of the mandibular central incisors.ResultsThe thickness of the labial cortical bone from 6 mm below the cementoenamel junction (CEJ) to the root apex; the thickness of the labial cancellous bone at the root apex; the total thickness of the alveolar bone at the root apex; the area of labial cortical bone; the total area of labial alveolar bone; and the height of the labial alveolar bone were highest in the labial-inclination group (all P<0.05). All these variables were positively correlated with the labial inclination of the mandibular central incisors (all P<0.05). There were no statistical differences between the groups for any of the measurements on the lingual side of the teeth (P>0.05).ConclusionThe morphology of the alveolar bone on the labial but not the lingual side of the mandibular central incisors was statistically significantly correlated with the labial inclination of those teeth in patients with low-angle, skeletal class III malocclusion.

Variations in the buccal-lingual alveolar bone thickness of impacted mandibular third molar: our classification and treatment perspectives.

Project description:Selecting either buccal or lingual approach for the mandibular third molar surgical extraction has been an intense debate for years. The aim of this observational retrospective study was to classify the molar based on the proximity to the external cortical bone, and analyze the position of inferior alveolar canal (IAC) of each type. Cone-beam CT (CBCT) data of 110 deeply impacted mandibular third molars from 91 consecutive patients were analyzed. A new classification based on the mean deduction value (MD) of buccal-lingual alveolar bone thickness was proposed: MD≥1 mm was classified as buccal position, 1 mm>MD>-1 mm was classified as central position, MD≤-1 mm was classified as lingual position. The study samples were distributed as: buccal position (1.8%) in 2 subjects, central position (10.9%) in 12 and lingual position (87.3%) in 96. Ninety-six molars (87.3%) contacted the IAC. The buccal and inferior IAC course were the most common types in impacted third molar, especially in lingually positioned ones. Our study suggested that amongst deeply impacted mandibular third molars, lingual position occupies the largest proportion, followed by the central, and then the buccal type.

Variance of radiographical alveolar bone mineral density by the anatomical morphology of mandibular bone.

Project description:PurposeEvaluating the bone mineral density (BMD) of alveolar bone is useful for dental treatments. The DentalSCOPE is an image analysis system developed to evaluate the BMD of alveolar bone. The aim of this study was to evaluate the relationship between cross-sectional anatomical size and BMD value.Materials and methodsThirty-four subjects (adult dental patients and volunteers) participated in this study. Intraoral radiographs of the mandibular molar region were acquired. Using DentalSCOPE software, three to four line-shaped regions of interest (ROIs) were obtained in the alveolar septum region. Cross-sectional CT images of mandible at the same position to above mentioned line-shaped ROI was reconstructed from subject's dental CBCT images. The measurements were performed using cross-sectional CT images and compared with BMD value.Results and discussionFor stepwise multiple linear regression analysis, the buccal-lingual width of the mandibular body (mandible width) and the CT value of the cancellous bone were adopted as explanatory variables that affected the BMD of the mandible. The BMD value increased by 20 mg/mm2 when the mandible width increased by 1 mm, and the BMD value increased by 5 mg/mm2 when the CT value of the cancellous bone increased by 1%.ConclusionIn the clinical application of alveolar bone BMD, the effect of the anatomical morphology of alveolar bone should be taken into consideration.

Relationship between different skeletal facial types and anterior alveolar bone thickness with cone-beam computed tomography in an Asian population.

Project description:BackgroundTo investigate the relationship between different skeletal facial types and anterior alveolar bone thickness with cone-beam computed tomography (CBCT) in an Asian population.MethodsA total of 130 patients with 1,560 healthy anterior teeth were enrolled. On three-dimensional reconstructed images, Frankfurt-mandibular plane angle (FMA) value and angle formed by subspinale, nasion, and supramental (ANB) value were measured, and subjects were categorized into different groups based on their vertical skeletal patterns as well as sagittal jaw relationships. For each tooth, the thickness of alveolar bone was measured at three locations: 1, 3, and 5 mm apical to alveolar bone crest. Descriptive statistics were used. Kruskal-Wallis test, one-way ANOVA, and independent-samples t-test were used for further analysis.ResultsMen's maxillary anterior teeth's lingual alveolar bone thickness was significantly greater than women's (P<0.05). Strong correlations were found between vertical skeletal patterns and lingual alveolar bone thickness of maxillary/mandibular anterior teeth (R2=0.302, P<0.01 in the maxilla; R2=0.311, P<0.01 in the mandible). However, no significant difference was shown in the alveolar bone thickness among people with different sagittal bone profiles.ConclusionsThe lingual alveolar bone of the maxillary anterior teeth is thicker in males than in females. With the increase of FMA, the anterior alveolar bone gradually became thinner.

Correlation between objective and subjective evaluation of profile in bimaxillary protrusion patients after orthodontic treatment.

Project description:ObjectiveTo correlate the objective cephalometric measurements with subjective facial esthetics in patients with bimaxillary protrusion.Materials and methodsThe sample consisted of 60 Asian-Chinese patients with bimaxillary protrusion who met the inclusion criteria. The facial esthetics of posttreatment profile and the change of profile on standardized lateral photographs were rated by a panel of 10 orthodontists and a panel of 10 lay persons with bimaxillary protrusion. All of the pretreatment and posttreatment cephalograms were digitized and traced. Twenty-five cephalometric measurements were constructed and analyzed. Correlations between the subjective facial esthetic scores and each cephalometric measurement were evaluated.ResultsThe cephalometric measurements correlated with the facial esthetic scores of posttreatment profile given by the orthodontist and the lay persons were basically the same. For the evaluation of posttreatment profile in bimaxillary protrusion patients, the upper and lower lip to E-line, upper and lower incisor tip to AP plane, Pg-NB distance, mentolabial angle, and sulcus depth correlated significantly with the esthetic score. For the evaluation of profile change during orthodontic treatment, retraction of upper incisor relative to AP plane or the perpendicular line through sella (line Y), change of upper incisor inclination, change of mentolabial sulcus depth, and retraction of lips relative to E-line were correlated positively with the esthetic value.ConclusionsCephalometric measurements of lip position, incisor position, and chin morphology were key parameters correlated to facial esthetics.

Mapping the immune microenvironment for mandibular alveolar bone homeostasis at single-cell resolution.

Project description:Alveolar bone is the thickened ridge of jaw bone that supports teeth. It is subject to constant occlusal force and pathogens invasion, and is therefore under active bone remodeling and immunomodulation. Alveolar bone holds a distinct niche from long bone considering their different developmental origin and postnatal remodeling pattern. However, a systematic explanation of alveolar bone at single-cell level is still lacking. Here, we construct a single-cell atlas of mouse mandibular alveolar bone through single-cell RNA sequencing (scRNA-seq). A more active immune microenvironment is identified in alveolar bone, with a higher proportion of mature immune cells than in long bone. Among all immune cell populations, the monocyte/macrophage subpopulation most actively interacts with mesenchymal stem cells (MSCs) subpopulation. Alveolar bone monocytes/macrophages express a higher level of Oncostatin M (Osm) compared to long bone, which promotes osteogenic differentiation and inhibits adipogenic differentiation of MSCs. In summary, our study reveals a unique immune microenvironment of alveolar bone, which may provide a more precise immune-modulatory target for therapeutic treatment of oral diseases.

Assessment of dental, skeletal, and soft tissue changes following mandibular advancement with Invisalign in skeletal Class II.

Project description:ObjectiveThis study evaluated the dentoskeletal and soft tissue changes for Class II malocclusion patients treated with Invisalign clear aligners with mandibular wings (IAMW).MethodsThis retrospective study included 50 skeletal Class II patients treated with Invisalign clear aligner with mandibular wings. Records of 20 subjects were collected from the AAOF Legacy Collection (The Case Western Bolton Brush Growth Study) and were used as a control. The dental, skeletal, and facial soft tissue changes were assessed by digitizing and analyzing lateral cephalograms using Dolphin Imaging software (version 11.95 Premium; Dolphin Imaging & Management Solutions, Chatsworth, Calif). Paired t-tests and independent t-tests were used to assess the changes before and after and to compare between the IAMW and control groups.ResultsThe different measurements of the maxilla have shown that IAMW effect on the maxilla included minimal, non-significant retraction compared to the control group. The SNB and mandibular base position increased by 1.17° (±2.63) and 3.79 (±8.13), respectively. The mandible advanced significantly in the treatment group compared to the control group. Dentally, the lower incisors tipped slightly buccally, but the change was not significant (p > 0.05). The facial convexity angle decreased by 1.16° (±4.36).ConclusionInvisalign clear aligner with mandibular advancer wings was able to correct the Class II malocclusion. This correction was mainly skeletal with some dental changes. This device can be used to address the growth modification problem in Class II malocclusion at the same time as addressing the other occlusal problems.

Total Mandibular Subapical Alveolar Osteotomy to Correct Class II Division I Dentofacial Deformity.

Project description:Introduction:Class II division I malocclusions are the most common dentofacial deformities seen in clinical practice. Severe cases or cases in which growth has ceased may require full correction combining orthodontic and surgical treatment. We report a case of a total mandibular subapical alveolar osteotomy, performed to correct a class II division I dentofacial deformity. Case Report:A 19-year-old female patient was referred to the oral and maxillofacial surgery department at the Federal University of Paraná with chin aesthetic complaints as well as class II malocclusion. The proposed treatment was total mandibular subapical alveolar osteotomy, retaining the chin position and eliminating the need for genioplasty, since, although the patient presented with a class II dentofacial deformity, the chin was well positioned. Under general anesthesia, a "V-shaped" incision was conducted from the right retromolar region to the left retromolar region. A ring of cortical bone was removed around the mental foramen, with the aim to create a space around the mental nerve. Fixation was conducted with plates and screws of the 2.0 system. The patient on six-year follow-up showed osteotomy stability, a better overall occlusion, and outcome satisfaction.

Finite element analysis of a new alveolar bone splitting technique in the mandibular posterior region.

Project description:BackgroundFinite element analysis was used to predict the risk of bone plate fracture and the expected bone augmentation effect of a new alveolar bone splitting technique in the mandibular posterior region for different alveolar crest widths, different alveolar bone densities, different root incision widths, and different insertion depths of bone expansion instrumentation.MethodsThe jaw models of the mandibular posterior region were constructed by computer-aided software and surgical incisions and bone expansion instruments were prepared on the models, after which the alveolar bone splitting procedure was simulated by finite element analysis software, and the equivalent stress-strain distribution characteristics of the jaw models of each group, as well as the maximal force and the maximal displacement of the bone plate when it was fractured, were recorded.ResultsThe distribution of equivalent stress and strain was mainly concentrated in the cancellous bone area at the root incision and the lower 1/3 of the buccal cortical bone plate, and there was no significant difference in the stress-strain distribution characteristics of the jaw models of each group. The wider of the alveolar crest, the higher the force required to fracture the bone plate, but the smaller the maximum displacement; the plastic deformation capacity of type IV bone jaws was more excellent; the wider the width of the root incision, the shallower the depth of instrument insertion, and the larger the maximum displacement.ConclusionFinite element analysis can effectively simulate the surgical criticality index of the new alveolar bone splitting procedure. Alveolar crest width, alveolar bone density, root incision width, and instrument insertion depth had a clear correlation with the maximum displacement of the bone plate at fracture. The alveolar crest width and alveolar bone density also had a significant effect on the maximum force required to fracture the bone plate.

Comparison of invisalign mandibular advancement and twin-block on upper airway and hyoid bone position improvements for skeletal class II children: a retrospective study.

Project description:BackgroundThis study is to evaluate and compare the improvement of upper airway morphology and hyoid bone position in children with Class II mandibular retrusion treated with Invisalign mandibular advancement (MA) and Twin-Block (TB) appliances, utilizing cone beam computed tomography (CBCT).Methods32 children aged between 8 and 11.5 years old were included in this study, with an average age of 10.2 years old. These children were divided into two groups, MA and TB, with 16 children in each group. Changes in upper airway morphology and hyoid bone position before and after treatment were analyzed using CBCT.Results(1) Changes in upper airway before and after treatment: the oropharynx volume (Or-V), the oropharynx minimum cross-sectional area (Or-mCSA), the hypopharynx volume (Hy-V), and the hypopharynx minimum cross-sectional area (Hy-mCSA) in both the MA and TB groups increased after treatment, and the differences were statistically significant (P < 0.05) compared to pre-treatment status. (2) Changes in hyoid bone position before and after treatment: The distances between H point and third cervical vertebra (H-C3), H point and pogonion (H-RGN), H point and mandibular plane (H-MP), H point and Frankfort horizontal plane (H-FH), H and S point (H-S), and H point and palatal plane (H-PP) in both the MA and TB groups increased after treatment, and the differences were statistically significant (P < 0.05).ConclusionBoth MA and TB appliances effectively improved the structural narrowness of the upper airway and reduced respiratory resistance, thus improving breath quality. However, MA showed more effectiveness in improving the narrowest part of the hypopharynx compared to TB. Both appliances also promoted anterior downward movement of the hyoid bone, which opens the upper airway of the oropharynx and hypopharynx and helps the upper airway morphology return to normal range.
