Smart Clothing as a Noninvasive Method to Measure the Physiological Cardiac Parameters.

ABSTRACT: In response to global aging, there have been improvements in healthcare, exercise therapy, health promotion, and other areas. There is a gradually increasing demand for such equipment for health purposes. The main purpose of smart clothing is to monitor the physical health status of the user and analyze the changes in physiological signals of the heart. Therefore, this study aimed to examine the factors that affect the measurement of the heart's physiological parameters and the users' comfort while wearing smart clothing as well as to validate the data obtained from smart clothing. This study examined the subjective feelings of users (aged 20-60 years) regarding smart clothing comfort (within 12 h); the median values were comfortable and above (3.4-4.5). The clothing was combined with elastic conductive fiber and spandex to decrease the relative movement of the fiber that acts as a sensor and increase the user's comfort. Future studies should focus on the optimization of the data obtained using smart clothing. In addition to its use in medical care and post-reconstructive surgery, smart clothing can be used for home care of older adults and infants.

Project description:BackgroundThe recent trends of technological innovation and widescale digitization as potential solutions to challenges in health care, sports, and emergency service operations have led to the conception of smart textile technology. In health care, these smart textile systems present the potential to aid preventative medicine and early diagnosis through continuous, noninvasive tracking of physical and mental health while promoting proactive involvement of patients in their medical management. In areas such as sports and emergency response, the potential to provide comprehensive and simultaneous physiological insights across multiple body systems is promising. However, it is currently unclear what type of evidence exists surrounding the use of smart textiles for the monitoring of physiological outcome measures across different settings.ObjectiveThis scoping review aimed to systematically survey the existing body of scientific literature surrounding smart textiles in their most prevalent form, the smart shirt, for monitoring physiological outcome measures.MethodsA total of 5 electronic bibliographic databases were systematically searched (Ovid Medical Literature Analysis and Retrieval System Online, Excerpta Medica database, Scopus, Cumulative Index to Nursing and Allied Health Literature, and SPORTDiscus). Publications from the inception of the database to June 24, 2019 were reviewed. Nonindexed literature relevant to this review was also systematically searched. The results were then collated, summarized, and reported.ResultsFollowing the removal of duplicates, 7871 citations were identified. On the basis of title and abstract screening, 7632 citations were excluded, whereas 239 were retrieved and assessed for eligibility. Of these, 101 citations were included in the final analysis. Included studies were categorized into four themes: (1) prototype design, (2) validation, (3) observational, and (4) reviews. Among the 101 analyzed studies, prototype design was the most prevalent theme (50/101, 49.5%), followed by validation (29/101, 28.7%), observational studies (21/101, 20.8%), and reviews (1/101, 0.1%). Presented prototype designs ranged from those capable of monitoring one physiological metric to those capable of monitoring several simultaneously. In 29 validation studies, 16 distinct smart shirts were validated against reference technology under various conditions and work rates, including rest, submaximal exercise, and maximal exercise. The identified observational studies used smart shirts in clinical, healthy, and occupational populations for aims such as early diagnosis and stress detection. One scoping review was identified, investigating the use of smart shirts for electrocardiograph signal monitoring in cardiac patients.ConclusionsAlthough smart shirts have been found to be valid and reliable in the monitoring of specific physiological metrics, results were variable for others, demonstrating the need for further systematic validation. Analysis of the results has also demonstrated gaps in knowledge, such as a considerable lag of validation and observational studies in comparison with prototype design and limited investigation using smart shirts in pediatric, elite sports, and emergency service populations.

Project description:Health care is one of the least funded sectors in Bangladesh and many other similar developing countries. People living in rural and remote areas do not have access to proper health care, and when they do, it is too expensive. This research aimed to develop a real-time health monitoring system that is cheap, easy to use, and accessible by doctors and patients. The system consists of several Internet of Things (IoT)-based sensors connected to an Arduino microprocessor, which thus measures the vital body signs of the patients. The measured readings are then transmitted to an Android application on a smartphone via a Bluetooth module. The sensors are connected to analog inputs. These sensors measure analog data, which is amplified by the microprocessor after being sorted. Doctors can also carry out the diagnosis of ailments using the data collected remotely from the patient. An Android-based mobile application that interfaces with a web-based application is implemented for efficient patients-doctors dual real-time communication. The Android application, which is connected to a MySQL database, updates the said database, which updates and displays the readings on a website accessible by both doctors and patients. Initially, the health monitor was tested using an Arduino Integrated Development Environment (IDE) monitor and a single user. Once initial simulations were successful, the proposed system was tested on five different real-human test subjects. The testing of the wireless health monitor produced successful results that measured patient vitals with a high level of accuracy. The proposed IoT-based system monitors vital signs such as the patient's body temperature, heart rate, ECG, SpO2 levels, blood pressure, and glucose levels. This system also includes a medical treatment plan by the doctors. The proposed system is novel as it integrates the IoT-based patient monitoring system with telemedicine. This proposed system has different sensors for real-time measuring the vital signs of the human body. A mobile and web application have also been integrated with this system for real-time remote patient monitoring and treatment plan. There are now systems available that only offer a telemedicine facility, where patients and doctors can have discussions, but do not have an IoT-based patient vital sign monitoring system integrated with telemedicine. The proposed system in this paper has the facility of IoT-based patient vital signs monitoring integrated with telemedicine, which makes this research work novel. The proposed system will increase the life expectancy of people throughout the world.

Project description:ObjectiveTo measure mechanical strain of the lamina cribrosa (LC) after intraocular pressure (IOP) change produced 1 week after a change in glaucoma medication.DesignCohort study.ParticipantsAdult glaucoma patients (23 eyes, 15 patients) prescribed a change in IOP-lowering medication.InterventionNoninvasive OCT imaging of the eye.Main outcome measuresDeformation calculated by digital volume correlation of OCT scans of the LC before and after IOP lowering by medication.ResultsAmong 23 eyes, 17 eyes of 12 persons had IOP lowering ≥ 3 mmHg (reduced IOP group) with tensile anterior-posterior Ezz strain = 1.0% ± 1.1% (P = 0.003) and compressive radial strain (Err) = -0.3% ± 0.5% (P = 0.012; random effects models accounting inclusion of both eyes in some persons). Maximum in-plane principal (tensile) strain and maximum shear strain in the reduced-IOP group were as follows: Emax = 1.7% ± 1.0% and Γmax = 1.4% ± 0.7%, respectively (both P < 0.0001 vs. zero). Reduced-IOP group strains Emax and Γmax were significantly larger with greater % IOP decrease (P < 0.0001 and P < 0.0001, respectively). The compliances of the Ezz, Emax, and Γmax strain responses, defined as strain normalized by the IOP decrease, were larger with more abnormal perimetric mean deviation or visual field index values (all P ≤ 0.02). Strains were unrelated to age (all P ≥ 0.088). In reduced-IOP eyes, mean LC anterior border posterior movement was only 2.05 μm posteriorly (P = 0.052) and not related to % IOP change (P = 0.94, random effects models). Only Err was significantly related to anterior lamina depth change, becoming more negative with greater posterior LC border change (P = 0.015).ConclusionsLamina cribrosa mechanical strains can be effectively measured by changes in eye drop medication using OCT and are related to degree of visual function loss in glaucoma.Financial disclosuresProprietary or commercial disclosure may be found in the Footnotes and Disclosures at the end of this article.
