Project description:Abstract To study the efficacy of the public policy response to the COVID‐19 pandemic, we develop a model of the rich interactions between epidemiology and socioeconomic choices. Preferences feature a “fear of death” that lead individuals to reduce their social activity and work time in the face of the pandemic. The aggregate effect of these reductions is to slow the spread of the novel coronavirus. We calibrate the model, including public policies, to developments in Ontario in the spring of 2020. The model fits the epidemiological data quite well, including the second wave starting in late 2020. We find that socioeconomic interventions work well in the short term, resulting in a rapid drop‐off in new cases. The long run, however, is governed chiefly by health developments. Welfare cost calculations point to synergies between the health and socioeconomic measures.

Project description:The COVID-19 pandemic has led to shifts in healthcare utilization for patients both with and without COVID-19. We aimed to determine how trends in neurology service admissions during the pandemic may aid in departmental planning by predicting future clinician staffing and other needs. We examined all admissions to the general neurology, stroke, and neurocritical care services from January 31 to May 16, 2020 at our tertiary-care hospital using an electronic health record query, comparing these to analogous data in 2019. We trended admission rates and projected future censuses using logarithmic regression, tracked changes in length of stay (LOS), and quantified shifts in presentations of specific diagnoses. Daily rates of admissions declined sharply during the week of March 13, 2020 (the week after pandemic status was declared by the World Health Organization). On the censoring date, we projected a return to pre-pandemic censuses in the week of June 21 and used this information to make decisions regarding neurology resident schedules. There was a trend toward increased LOS for general neurology and stroke patients between March 27 and April 9, 2020 compared to in 2019, with subsequent decline coinciding with early hospital initiatives. Since March 13, 2020, there has been a trend toward reduced presentations of ischemic stroke, suggesting a need for community education on stroke awareness. Characterizing early trends in neurology admissions may allow physician administrators to plan local and community-level responses to the pandemic.

Project description:Unprecedented nationwide lockdowns were adopted because of the COVID-19 pandemic. Understanding the socioeconomic impact of the past and future restrictions while assessing the resilience of a local economy emerged as a worldwide necessity. To predict the economic and environmental effects of the lockdowns, we propose a methodology based on the well-established input-output inoperability model, using Italy as a case study. By reconstructing the 2020 restrictions, we analyzed the economic losses and greenhouse gas emissions reductions, identifying the most economically impacted sectors because of the restrictions and the sectoral interdependencies and those avoiding most air emissions. We constructed four partial-lockdown scenarios by minimizing the economic losses for increasing restrictions to highlight the model's utility as a tool for policymaking. By revealing the most interconnected and, thus, crucial sectors, the simulated scenarios showcase how the restrictions can be selected to avoid sudden and unpredicted economic damage.

Project description:Although phosphorus (P) recovery and management from sewage sludge are practiced in North America and Europe, such practices are not yet to be implemented in China. Here, we evaluated the environmental sustainability opportunity and socio-economic costs of recovering P from sewage sludge by replacing the current-day treatments (CT; sludge treatment and landfill) and P chemical fertilizer application (CF) in China using life cycle assessment and life cycle costing methods. Three potential P recovery scenarios (PR1‒PR3: struvite, vivianite, and treated sludge) and corresponding current-day scenarios (CT1‒CT3 and CF) were considered. Results indicated that PR1 and PR2 have smaller environmental impacts than the current-day scenarios, whereas PR3 has larger impacts in most categories. PR3 has the lowest net costs (sum of internal costs and benefits, 39.1-54.7 CNY per kg P), whereas PR2 has the lowest external costs (366.8 CNY per kg P). Societal costs for production and land use of 1 kg P by P recovery from sewage sludge (e.g., ∼527 CNY for PR1) are much higher than those of P chemical fertilizers (∼20 CNY for CF). However, considering the costs in the current-day treatments (e.g., ∼524 CNY for CT1), societal costs of P recovery scenarios are close to or slightly lower than those of current-day scenarios. Among the three P recovery scenarios, we found that recovering struvite as P fertilizer has the highest societal feasibility. This study will provide valuable information for improved sewage sludge management and will help promote the sustainable supply of P in China.

Project description:Creating opportunities for people to achieve socioeconomic mobility is a widely shared societal goal. Paradoxically, however, achieving this goal can pose a threat to high-socioeconomic-status (SES) people as they look to maintain their privileged positions in society for both them and their children. Two studies evaluate whether this threat manifests as "opportunity hoarding" in which high-SES parents adopt attitudes and behaviors aimed at shoring up their families' access to valuable educational and economic resources. The current paper provides converging evidence for this hypothesis across two studies conducted with 2,557 American parents. An initial correlational study demonstrated that believing that socioeconomic mobility is possible was associated with high-SES parents being more inclined to attempt to secure valuable educational and economic resources for their children, even when doing so came at the cost of low-SES families. Specifically, high-SES parents with stronger beliefs in socioeconomic mobility exhibited decreased support for redistributive policies and viewed engaging in discrete behaviors that would unfairly advantage their children (e.g., allowing them to misrepresent their identities on school and job applications) as more acceptable relative to both low-SES parents with similar beliefs and high-SES parents who were less optimistic about socioeconomic mobility. A subsequent experimental study established these relationships causally by comparing parents' responses to different types of socioeconomic mobility. Together, the current findings merge insights across psychology and economics to deepen understandings of the processes through which societal inequities emerge and persist, especially during times of apparently abundant opportunity.

Project description:The COVID-19 pandemic has caused (and continues to cause) severe disruption in global and local economies and has forced countries, societies, and individuals to adapt quickly to the unprecedented and unpredictable situations. Despite the obvious negative consequences of the pandemic, many have called for efforts to identify transformative opportunities for sustainable development throughout this disorderly time. In the present paper, we explore such potential opportunities in the context of an interdisciplinary, international research project, which is focusing on sustainable marine management in biosphere reserves and marine parks in Southeast Asia. During a virtual workshop conducted as part of the GCRF (Government's Global Challenges Research Fund) Blue Communities Project, future scenarios were developed depicting the potential effects of the COVID-19 pandemic on five case study sites. All of these sites are in areas of internationally recognized outstanding ecological value (Taka Bonerate Kepulauan-Selayar Biosphere Reserve, Indonesia; Tun Mustapha Park, Sabah, Malaysia; Palawan Biosphere Reserve, Philippines; North Devon Biosphere Reserve, United Kingdom; Cu Lao Cham-Hoi An Biosphere Reserve, Vietnam). At the macro-level, economies, governance structures, and societal norms are undergoing big changes. At the micro-level, the livelihoods, lifestyles, and backyards of local residents have to adapt. Collaboratively, we explored how COVID-19 posed challenges in our five case study sites, but we also focused on the potential COVID-19-related windows of opportunity for future sustainable development. Opportunities could be identified in all three pillars of sustainable development: the environment, the society, and the economy. Although remarkable similarities can be found across all five sites, we conclude that there cannot be a "one-size-fits-all" solution to turn the tide toward achieving sustainable development. Just as before the pandemic, sustainable development starts with engaging with and understanding local environments, challenges, and situations; building on local knowledge; and developing tailor-made solutions for the communities in situ.

Project description:Management of uncontrolled diabetic (DM) patients has always been a challenge across healthcare settings, but recently we noticed a surge in the number of uncontrolled DM patient in our clinic patients' population during the COVID19 pandemic. This in part a combination of disruptions to care, delayed or inaccessible care, and poor diet and physical activity. To address this issue, we formed a multidisciplinary Diabetes Initiative Team consisting of attending physicians, residents, clinical pharmacist, nurse manager, care manager, and coordinator, along with supportive staffs in our Internal Medicine Residency clinic. Our aim was to reengage diabetic patients in the outpatient setting and to overcome barriers limiting diabetic care.

Project description:PurposeIn this study, we examine three-dimensional (3D) proctoring tools (i.e., semitransparent ghost tools overlaid on the surgeon's field of view) on realistic surgical tasks. Additionally, we develop novel, quantitative measures of whether proctors exploit the additional capabilities offered by ghost tools.MethodsSeven proctor-trainee pairs completed realistic surgical tasks such as tissue dissection and suturing in a live porcine model using 3D ghost tools on the da Vinci Xi Surgical System. The usability and effectiveness of 3D ghost tools were evaluated using objective measures of proctor performance based on proctor hand movements and button presses, as well as post-study questionnaires.ResultsProctors exploited the capabilities of ghost tools, such as 3D hand movement (p < 0.001), wristedness (p < 0.001), finger pinch gestures (p < 0.001), and bimanual hand motions (p < 0.001). The median ghost tool excursion distances across proctors in the x-, y-, and z-directions were 57.6, 31.9, and 50.7, respectively. Proctors and trainees consistently evaluated the ghost tools as effective across multiple categories of mentoring. Trainees found ghost tools more helpful than proctors across all categories (p < 0.05).ConclusionsProctors exploit the augmented capabilities of 3D ghost tools during clinical-like training scenarios. Additionally, both proctors and trainees evaluated ghost tools as effective mentoring tools, thereby confirming previous studies on simple, inanimate tasks. Based on this preliminary work, advanced mentoring technologies, such as 3D ghost tools, stand to improve current telementoring and training technologies in robot-assisted minimally invasive surgery.

Project description:Rationale, aims, and objectivesAddressing wellbeing among learners, faculty, and staff during the COVID-19 pandemic is a challenge for many clinical departments. Continued and systemic supports are needed to combat the pandemic's impact on mental health and wellbeing. This article describes an iterative approach to conducting a needs assessment and implementing a COVID-19-related wellness initiative in a psychiatry department.MethodsDevelopment of the initiative followed the Plan-Do-Study-Act (PDSA) quality improvement cycle and was informed by Shanafelt and colleagues' framework for supporting healthcare workers during the COVID-19 pandemic. Key features included the establishment of a Wellness Working Group, the curation of relevant resources on the Department's website, and the deployment of regular, monthly surveys that informed the creation of further supports, such as a weekly online drop-in support group.ResultsSurvey response rates ranged from 22% to 32% (n = 90-127) throughout our initiative. Across multiple surveys, approximately 80% of respondents reported feeling supported or very supported by the Department, and 90% were satisfied or very satisfied with the quantity and quality of information provided. Our support group and resources page were accessed by nearly one-quarter and one-third of respondents, respectively, with satisfaction rates of 81% or higher. Consistent with the Department's mandate, ensuring equity was a key focus of the Working Group throughout its operations.ConclusionsThere is potential for this model to be scaled to create a faculty-wide, institution-wide, or regional approach to addressing wellbeing. Other departments may also wish to adopt similar approaches to supporting their members during this challenging time.

Project description:Climate change adaptation is paramount, but increasing evidence suggests that adaptation action is subject to a range of constraints. For a realistic assessment of future adaptation prospects, it is crucial to understand the timescales needed to overcome these constraints. Here, we combine data on documented adaptation from the Global Adaptation Mapping Initiative with national macro indicators and assess future changes in adaptation constraints alongside the Shared Socioeconomic Pathways, spanning a wide range of future socio-economic development scenarios. We find that even in the most optimistic scenario, it will take until well after 2050 to overcome key constraints, which will limit adaptation for decades to come particularly in vulnerable countries. The persistence of adaptation constraints calls for stringent mitigation, improved adaptation along with dedicated finance and increasing efforts to address loss and damage. Our approach allows to ground truth indicators that can be further used in climate modelling efforts, improving the representation of adaptation and its risk reduction potential.
