Impact of COVID-19-related knowledge on protective behaviors: The moderating role of primary sources of information.

ABSTRACT: This study assessed the modifying role of primary source of COVID-19 information in the association between knowledge and protective behaviors related to COVID-19 among adults living in the United States (US). Data was collected from 6,518 US adults through an online cross-sectional self-administered survey via social media platforms in April 2020. Linear regression was performed on COVID-19 knowledge and behavior scores, adjusted for sociodemographic factors. An interaction term between knowledge score and primary information source was included to observe effect modification by primary information source. Higher levels of knowledge were associated with increased self-reported engagement with protective behaviors against COVID-19. The primary information source significantly moderated the association between knowledge and behavior, and analyses of simple slopes revealed significant differences by primary information source. This study shows the important role of COVID-19 information sources in affecting people's engagement in recommended protective behaviors. Governments and health agencies should monitor the use of various information sources to effectively engage the public and translate knowledge into behavior change during an evolving public health crisis like COVID-19.

Project description:BackgroundIn the digital media era, the prevalence of algorithm-driven content push has intensified the health information echo chambers phenomenon, characterized by excessive homogenization and overload of information. This phenomenon may negatively impact the older adults, a "digitally vulnerable" group, by limiting their access to diverse health information and potentially inducing health information avoidance behaviors. However, the psychological mechanisms within this impact process remain unclear, and this study aims to explore them. Additionally, current study introduces trait mindfulness as a potential intervention tool in reducing health information avoidance behavior among the older adults.MethodsThis study constructs an impact model based on the Stress-Coping Theory and the Stress-Strain-Outcome (SSO) framework. It identifies health information similarity, relevance, and overload as characteristics of the echo chamber, constituting the Stress part; information fatigue as the Strain; and health information avoidance as the Outcome, with trait mindfulness serving as a moderating factor between Strain and Outcome. A cross-sectional survey of 236 older adults individuals aged 60 and above was conducted, and the collected data were analyzed using SmartPLS and SPSS.ResultsThe study found that health information similarity and overload significantly increased health information fatigue among the older adults, thereby promoting avoidance behavior, but no significant correlation was found between relevance and fatigue. While there was no significant correlation between trait mindfulness and health information fatigue, it effectively moderated the avoidance behavior induced by fatigue.ConclusionCurrent study reveals the impact of the health information echo chamber phenomenon on health information avoidance behavior among the older adults and the moderating role of trait mindfulness. It emphasizes the importance of breaking the health information echo chamber and promoting diverse information dissemination to improve the health information behavior of the older adults. Furthermore, the moderating role of trait mindfulness demonstrates positive intervention potential, providing the older adults with a psychological adjustment mechanism to accept and process health information with a more open attitude, thereby reducing unproductive avoidance behavior.

Project description:BackgroundConflicting messages and misleading information related to the coronavirus (COVID-19) pandemic (SARS-CoV-2) have hindered mitigation efforts. It is important that trust in evidence-based public health information be maintained to effectively continue pandemic mitigation strategies. Officials, researchers, and the public can benefit from exploring how people receive information they believe and trust, and how their beliefs influence their behaviors.MethodsTo gain insight and inform effective evidence-based public health messaging, we distributed an anonymous online cross-sectional survey from May to July, 2020 to Virginia residents, 18 years of age or older. Participants were surveyed about their perceptions of COVID-19, risk mitigation behaviors, messages and events they felt influenced their beliefs and behaviors, and where they obtained information that they trust. The survey also collected socio-demographic information, including gender, age, race, ethnicity, level of education, income, employment status, occupation, changes in employment due to the pandemic, political affiliation, sexual orientation, and zip code. Analyses included specific focus on the most effective behavioral measures: wearing a face mask and distancing in public.ResultsAmong 3,488 respondents, systematic differences were observed in information sources that people trust, events that impacted beliefs and behaviors, and how behaviors changed by socio-demographics, political identity, and geography within Virginia. Characteristics significantly associated (p < 0.025) with not wearing a mask in public included identifying as non-Hispanic white, male, Republican political identity, younger age, lower income, not trusting national science and health organizations, believing one or more non-evidence-based messages, and residing in Southwest Virginia in logistic regression. Similar, lesser in magnitude correlations, were observed for distancing in public.ConclusionsThis study describes how information sources considered trustworthy vary across different populations and identities, and how these differentially correspond to beliefs and behaviors. This study can assist decision makers and the public to improve and effectively target public health messaging related to the ongoing COVID-19 pandemic and future public health challenges in Virginia and similar jurisdictions.
