Primary umbilical cutaneous endometriosis: The first case report in Syria.

ABSTRACT: Endometriosis in the umbilical region is a very rare case. Although it is possible to have migrating endometrial glands and stroma outside the uterus, for example, the ovaries, fallopian tubes, and other pelvic organs, the umbilicus is a unique place. In our case it was a 40-year- old female patient with no surgical history. She has been suffering for years from symptoms that are very consistent with endometriosis in terms of pain, mass and bleeding. but the diagnostic procedures from imaging and pathology have been taken completely up, then the case was managed while preserving the patient's reproductive function.

Project description:Introductionand importance: Leiomyosarcoma is a rare aggressive soft-tissue malignancy typically originating from embryonic mesoderm or mesenchymal cell lines in smooth muscles.Leiomyosarcoma of the skin is termed as "Dermal Leiomyosarcoma", and is categorized into two subdivisions; superficial cutaneous and deep subcutaneous.Both types begin either as primary lesions or metastatic lesions from distant sites.Case presentationWe report the case of a 60-year-old male patient with Primary Cutaneous Leiomyosarcoma (PCL) located in the left iliac region.His history is insignificant and he has no family or genetic history of leiomyosarcoma. The lesion was itchy without any other symptoms and existed 20 years before our evaluation.A biopsy from the nodule was performed and sent to the pathology department, where the section was stained with smooth muscle actin stain (SMA) and the result was positive.We referred the patient to a surgeon to excise the nodule. The lesion was excised with a 3cm safety margin, the eradication includes also the of the major iliac muscle.After one year of follow-up there was no metastasis nor recurrence.ConclusionPrimary Cutaneous Leiomyosarcoma is a very rare malignancy and it is hard to diagnose without biopsy and pathological examination.

Laparoscopic umbilical trocar port site endometriosis: A case report.

Project description:BACKGROUND:Abdominal wall endometriosis can occur secondary to gynecological and obstetric pelvic laparotomy; however, this is a rare clinical event. There are few cases of endometriosis involving the incision site of a laparoscopic surgery, especially for those of the endometrial nodule at the umbilical trocar port site where the camera is placed. CASE SUMMARY:We describe the case of a 37-year-old woman who presented with a 2-year history of a tough swelling below the umbilicus, which presented periodical pain during menstruation. The patient had undergone laparoscopic ovarian cystectomy 4 years prior, and we theorized that the umbilical nodule was a complication of that laparoscopic surgery. Histological analysis confirmed the diagnosis of abdominal umbilical scar endometriosis secondary to previous laparoscopic surgery. Surgical removal of the nodule followed by three cycles of leuprorelin was curative. CONCLUSION:Abdominal mass and pain in women of childbearing age with a previous history of pelvic surgery should support consideration of endometriosis at the surgical site.

A Strangulated Amyand's hernia:The first case report in Syria.

Project description:Amyand's hernia is a rare case of inguinal hernia where the hernia sac contains the appendix, and depending on the condition of the appendix, this type is classified into under types where the appendix can be normal, inflamed, or even perforated. Accordingly, the patient's condition can be good or bad. In our case, we have a 13-year-old child who suffers from abdominal pain and discomfort. He is referred to the emergency department with this complaint. By clinical and radiological examination, the presence of an inguinal hernia containing the appendix was found. Based on that, we performed emergency and investigative surgery in order to determine the condition of the appendix and whether there is suffocation or not. It was found by surgical opening that the hernia was incarcerated and this required excision of the appendix and repair of the hernia.

Allgrove syndrome: Case report of 18 years old male:the first case report from Syria.

Project description:Triple A syndrome 3A (Allgrove syndrome) is a rare autosomal recessive multiorgans dysfunction characterized by alacrima, achalasia which is the absence of esophageal muscle peristalsis and lower sphincter failure to relax and adrenal insufficiency. About third of patient additional features like neurological and autonomic manifestations reported (making the syndrome 4A), the spectrum of neurological symptoms varies including gait disturbances, parkinsonism, muscle wakeness, mental retardation, peripheral sensory and motor neuropathy. Here we reported A 18 years old male, who had postnatal recurrent conjunctivitis so alacrima was diagnosed, in the sventh years he developed achalasia signs; dysphagia and regurgitation and laparscopic surgical myotomy and fundoplication were done, when he became 16 he presented to our clinic for poor appetite, weight loss,and failure to thrive. Assessment of ACTH, cortisol, ACTH stimulation test confirmed he had adrenal insufficiency and physical examination showed he had foots deformity due to muscular atrophy caused by neuropathy.treatment performed by managing symptoms of the condition(replacement of glucocorticoids, surgical correction of achalasia, artificial tears).The follow-up was over a period of 6 months and we noted a great improvement of patient's condition.

Feline cutaneous histoplasmosis: The first case report from Thailand.

Project description:A 2-year-old, neutered female domestic shorthaired cat presented with a history of multiple papules and nodules on pinnae, nodules on the nose, and chronic wound at the lateral surface of left radial area for four months. Skin biopsy demonstrated moderate numbers of small, oval-to-round, single-walled yeasts inside the macrophages. In addition, PCR confirmed the sequence of Histoplasma capsulatum. This is the first case report of feline cutaneous histoplasmosis in Thailand.

Emphysematous pyelonephritis: A rare case report from Syria.

Project description:IntroductionEmphysematous pyelonephritis is a rare necrotizing infection of the kidney. It should be diagnosed early because it has a bad prognosis. Treatments range from intravenous antibiotics to nephrectomy in severe cases.Presentation of caseA 67 years old female with uncontrolled diabetes mellitus presented with fever, right flank pain, and dysuria. Computed tomography for abdominal and pelvis showed gas inside the right kidney. After administration of intravenous antibiotics, she underwent right nephrectomy. Next, she was admitted to intensive care unit for monitoring. After three days, she was discharged with no complains. Follow-up for three months showed full recovery.Clinical discussionEmphysematous pyelonephritis is an emergent condition. A history of uncontrolled diabetes mellitus with current flank pain with fever raises the suspicion. Computed tomography could be performed to confirm the diagnosis.ConclusionSevere cases of emphysematous pyelonephritis are an emergent condition that needs nephrectomy.

Multiple Primary Cutaneous Anaplastic Large Cell Lymphoma: A Case Report.

Project description:IntroductionAnaplastic large cell lymphomas (ALCL) are a subtype of non-Hodgkin lymphoma categorized into systemic and cutaneous (cALCL) subtypes. cALCL are frequently characterized by their lack of ALK receptor expression, which differentiates them from their systemic counterparts. cALCLs present as solitary or grouped skin lesions. Due to their rarity, cALCLs are frequently misdiagnosed as other skin conditions.Case presentationWe report the case of a female with a 30-year history of expanding skin lesions on the upper and lower limbs, abdomen, and back. During her initial presentation, the lesions were thought to be due to an arthropod bite. However, lesion biopsy revealed ALK-negative cALCL and methicillin-resistant Staphylococcus aureus. A full workup excluded systemic ALCL and, therefore, confirmed the diagnosis of primary cALCL. The patient was placed on methotrexate to treat the lesions and sulfamethoxazole/trimethoprim to treat the infection.ConclusionOur case highlights the need to consider cALCL in the differential diagnosis of patients with persistent skin lesions.

Primary cutaneous infection due to Microascus cirrosus: a case report.

Project description:BackgroundMicroascus cirrosus, the teleomorph of Scopulariopsis spp., is a saprobic species with a worldwide distribution and rarely causes human infection. In the present paper, we present the first case of primary cutaneous M. cirrosus infection in a Chinese female.Case presentationA 17-year-old female presented with tender ulceration on her left ankle for three months. Histology revealed multiple branching, septate hyphae and moniliform fungal elements in the dermis. Tissue culture grew M. cirrosus, the teleomorph of Scopulariopsis spp., characterized by intercalary and ballooned, chlamydospore-like structures, annellidic and ampulliform conidiogenous cells along with truncated, bullet-shaped, smooth conidia and globose perithecial ascomata with cylindrical necks. Further molecular sequencing confirmed the identification. A diagnosis of primary cutaneous infection due to M. cirrosus was made. Treatment with itraconazole 200 mg per day for 10 weeks achieved significant improvement of the skin lesions.ConclusionsThis case of uncommon mycotic cutaneous infection highlights the importance of mycological examination that help to recognize rare pathogenic fungi.

Renal primitive neuroectodermal tumor. The first case series from Syria

Project description:Primitive neuroectodermal tumor (PNET) mainly arises from soft tissues of the extremities such as humerus, femur, C tibia. It rarely arises from kidney; less than 200 cases have been reported in the literature. The clinical presentation and radiography findings are not specific. Here we first report two cases of renal primitive neuroectodermal tumor in Syria. the first patient was 26-year-old- female that presented to urology clinic complaining of right flank pain. Ultrasonography of the abdomen showed a large mixed heterogeneous mass in the right kidney with no hemorrhage or calcification and MSCT of abdomen and pelvis demonstrate a mixed well-demarcated heterogeneous mass measuring (74*117) mm in the right kidney right radical nephrectomy was performed. The second patient 19-year-old-male presented with left flank pain. Ultrasonography of the abdomen showed mixed large mass involving the left kidney, with unmarked border. The CT of the abdomen and pelvis demonstrating a (30*110*90) mm left renal mass and periaortic lymphadenopathy measuring (45*28) mm. The patient underwent Left radical nephrectomy with periaortic lymphadenectomy dissection. The final diagnosis for both cases was Renal PNET based on microscopic and immunohistochemistry examination. In patient with suspected renal mass in the radiographic images, the diagnosis of renal primitive neuroectodermal tumor should be kept in the mind despite its rarity. The final diagnosis is done by histopathological study in association with immunohistochemical examination.
