An Uncommon Cause of Dyspnea: Usefulness of Multimodality Cardiac Imaging.

ABSTRACT: Pericardial lipomas are very uncommon benign primary cardiac tumors. We describe the case of a patient with symptomatic large pericardial mass who presented with heart failure. Multimodality cardiac imaging helped us in the diagnosis of this unusual entity. (Level of Difficulty: Intermediate.).

Project description:Atrial myxomas are the most common primary tumors of the heart. Their presentation will vary depending on their anatomical site and many times remain asymptomatic. Local obstruction to atrial outflow can present with dyspnea, orthopnea, syncope, and rarely sudden death. A differential diagnosis of tumor in the left atrium includes thrombus. Echocardiography usually suffices in making a diagnosis, but in difficult cases, cardiac computed tomography and magnetic resonance imaging may be needed. We report a case of left atrial myxoma in an elderly female which was asymptomatic and presented with rapid deterioration of symptoms.

Multimodality Imaging Findings in Cardiac Plasmacytoma.

Project description:Extramedullary plasmacytoma involving the heart is a rare occurrence. We present the case of a 74-year-old man who received a diagnosis of cardiac plasmacytoma in the context of relapsed multiple myeloma. The tumor primarily presented as a solitary extramedullary plasmacytoma with extensive infiltration into the right ventricle, leading to mild symptoms such as dyspnea. There was no evidence of systemic involvement. A treatment with DRd (daratumumab, lenalidomide, and dexamethasone) was promptly initiated. This case demonstrates the effective use of multimodal imaging techniques to characterize cardiac masses and to determine the most appropriate management plan.

Mitral-aortic Intervalvular Fibrosa Aneurysm with Rupture into Left Atrium: An Uncommon Cause of Acute Dyspnea.

Project description:Aneurysm of the mitral-aortic intervalvular fibrosa (MAIF) is an exceptionally rare but a potentially catastrophic complication, commonly following aortic valve endocarditis. We present a 24-year-old male presenting with acute onset dyspnea secondary to MAIF aneurysm rupturing into a left atrium causing large shunt which was diagnosed on echocardiography. The MAIF aneurysm in the absence of infective endocarditis rupturing into left atrium is extremely rare.

Multimodality Imaging of a Silent Cardiac Hemangioma.

Project description:A 74-year-old man underwent echocardiographic exam for hypertension screening. A fixed plurilobulated mass originating from the right ventricular lateral wall and occupying half of the cavity was incidentally diagnosed. On cardiac magnetic resonance (CMR) it appeared homogeneous, intermediate-to-high signal on T1-weighted, and diffusely hyperintense on T2-weighted images. First pass enhancement was late and heterogeneous and no late gadolinium enhancement was present. Computed tomography (CT) showed no extracardiac infiltration, the feeding artery was a branch of therightcoronary artery. The tumor was excised and histological examination demonstrated a hemangioma of the cavernous type. The postoperative course was uneventful. From 1998 to 2014, four cardiac hemangiomas have been diagnosed in our Department, accounting for 8.7% of all primary cardiac tumors and for 9.5% of all benign forms; estimated population prevalence was 0.11/100.000 inhabitants/year. The hemodynamic consequences of unoperated cardiac hemangiomas cannot be predicted and therefore, resection is recommended.
