Association of loss of spleen visualization on whole-body diffusion-weighted imaging with prognosis and tumor burden in patients with multiple myeloma.

ABSTRACT: This study investigated the clinical significance of loss of spleen visualization (LSV) on whole-body diffusion-weighted imaging (WB-DWI) in patients with multiple myeloma (MM). The WB-DWI of 96 patients with newly diagnosed MM (NDMM) and 15 patients with smoldering MM (sMM) were retrospectively reviewed. LSV was observed in 56 patients with NDMM (58.3%) and 1 patient with sMM (6.7%). Patients with NDMM with LSV had a higher median infiltration of bone marrow plasma cells (80.0% vs. 50.0%, p < 0.001) and median total diffusion volume (median; 540.2 vs. 137.0 mL, p = 0.003) than patients without LSV. Patients with LSV had a lower spleen-to-spinal cord ratio (0.36 vs. 0.96, p < 0.001) and worse 2-year overall survival (OS) (84.6% vs. 100%, p = 0.032). Patients who did not recover spleen visualization during treatment had a worse prognosis, even when they obtained very good partial response (median progression-free survival: 13.2 months). Spleen histopathological findings revealed higher cellularity and diffuse myeloma cell infiltration in a patient with LSV and splenic amyloidosis without extramedullary hematopoiesis in a patient without LSV. Therefore, LSV indicates worse prognosis for patients with MM, even when the patient responds to treatment. Further studies are warranted to clarify the immunological role of spleen in MM.

Project description:ObjectivesCytogenetic abnormalities are predictors of poor prognosis in multiple myeloma (MM). This paper aims to build and validate a multiparametric conventional and functional whole-body MRI-based prediction model for cytogenetic risk classification in newly diagnosed MM.MethodsPatients with newly diagnosed MM who underwent multiparametric conventional whole-body MRI, spinal dynamic contrast-enhanced (DCE-)MRI, spinal diffusion-weighted MRI (DWI) and had genetic analysis were retrospectively included (2011-2020/Ghent University Hospital/Belgium). Patients were stratified into standard versus intermediate/high cytogenetic risk groups. After segmentation, 303 MRI features were extracted. Univariate and model-based methods were evaluated for feature and model selection. Testing was performed using receiver operating characteristic (ROC) and precision-recall curves. Models comparing the performance for genetic risk classification of the entire MRI protocol and of all MRI sequences separately were evaluated, including all features. Four final models, including only the top three most predictive features, were evaluated.ResultsThirty-one patients were enrolled (mean age 66 ± 7 years, 15 men, 13 intermediate-/high-risk genetics). None of the univariate models and none of the models with all features included achieved good performance. The best performing model with only the three most predictive features and including all MRI sequences reached a ROC-area-under-the-curve of 0.80 and precision-recall-area-under-the-curve of 0.79. The highest statistical performance was reached when all three MRI sequences were combined (conventional whole-body MRI + DCE-MRI + DWI). Conventional MRI always outperformed the other sequences. DCE-MRI always outperformed DWI, except for specificity.ConclusionsA multiparametric MRI-based model has a better performance in the noninvasive prediction of high-risk cytogenetics in newly diagnosed MM than conventional MRI alone.Critical relevance statementAn elaborate multiparametric MRI-based model performs better than conventional MRI alone for the noninvasive prediction of high-risk cytogenetics in newly diagnosed multiple myeloma; this opens opportunities to assess genetic heterogeneity thus overcoming sampling bias.Key points• Standard genetic techniques in multiple myeloma patients suffer from sampling bias due to tumoral heterogeneity. • Multiparametric MRI noninvasively predicts genetic risk in multiple myeloma. • Combined conventional anatomical MRI, DCE-MRI, and DWI had the highest statistical performance to predict genetic risk. • Conventional MRI alone always outperformed DCE-MRI and DWI separately to predict genetic risk. Project description:BackgroundMarathon training can reverse bone marrow conversion; however, little is known about the normal bone marrow whole-body diffusion-weighted imaging (WB-DWI) signal characteristics of amateur marathon runners. If marathon training can cause diffuse hyperintensity of bone marrow on WB-DWI is essential for correctly interpreting the diffusion-weighted (DW) images. This study sought to evaluate the WB-DWI signal characteristics of normal bone marrow in amateur marathon runners.MethodsIn this prospective cross-sectional study, 30 amateur marathon runners who had trained for over 3 years for regular or half-marathon races and had a running frequency of more than 20 days a month at a distance of more than 100 km per month from the Chengde Marathon Outdoor Sports Association in Hebei, China, and 30 age- and gender-matched, healthy volunteers (the control group) who had no long-term heavy-load sports history were recruited between April 2021 to September 2021. All the subjects underwent WB-DWI (b-value: 0, 800 s/mm2) and lumbar vertebral transverse relaxation time (T2) mapping. The bone marrow WB-DWI signal characteristics were analyzed visually and statistically by chi-square (χ2) tests. The apparent diffusion coefficient (ADC), DWI signal intensity, and T2 values of the bone marrow were quantitatively and statistically analyzed by the independent sample t-test and Mann-Whitney U test.ResultsNo subjects were excluded from the study. The bone marrow of 30 of the 60 subjects (aged 30-50 years) showed diffuse hyperintensity in the DW images. However, in all 60 subjects, the humeral heads, femoral heads, and great trochanters had low signals. The frequency of diffuse bone marrow DWI hyperintensity was significantly higher in the male amateur marathon runners (50%) than the male controls (5%, P=0.003), but no such significant difference was found between the female amateur marathon runners (100%) and female controls (90%, P>0.99). The DW signal intensity ratios of bone marrow to muscle (SIRBM-muscle) were significantly higher in the male amateur marathon runners than the male controls in the thoracic vertebrae (4.68 vs. 3.57, P=0.021), lumbar vertebrae (4.49 vs. 3.01, P<0.001), sacrum (3.67 vs. 2.62, P=0.002), and hip (3.45 vs. 2.50, P=0.002), but were only significantly higher in the female amateur marathon runners than the female controls in the thoracic vertebrae (7.69 vs. 5.87, P=0.029) and hip (4.76 vs. 3.92, P=0.004). The mean T2 values of the lumbar vertebrae were significantly higher in the male amateur marathon runners than the male controls (116.76 vs. 97.63 ms, P=0.001), but no such significant difference was observed between the female amateur marathon runners and the corresponding controls (118.58 vs. 124.10 ms, P=0.386).ConclusionsMarathon training resulted in diffuse hyperintensity in the bone marrow based on WB-DWI in 50% of the male amateur marathon runners aged 30-50 years. Thus, when WB-DWI is used for bone marrow disease screening, marathon training history should be considered to avoid false-positive diagnoses.

Project description:ObjectiveTo assess the test-retest reproducibility and intra/interobserver agreement of apparent diffusion coefficient (ADC) measurements of myeloma lesions using whole body diffusion-weighted MRI (WB-DW-MRI) at 3T MRI.MethodsFollowing ethical approval, 11 consenting patients with relapsed multiple myeloma were prospectively recruited and underwent baseline WB-DW-MRI. For a single bed position, axial DWI was repeated after a short interval to permit test-retest measurements.Mean ADC measurement was performed by two experienced observers. Intra- and interobserver agreement and test-retest reproducibility were assessed, using coefficient of variation (CV) and interclass correlation coefficient (ICC) measures, for diffuse and focal lesions (small ≤10 mm and large >10 mm).Results47 sites of disease were outlined (23 focal, 24 diffuse) in different bed positions (pelvis = 22, thorax = 20, head and neck = 5). For all lesions, there was excellent intraobserver agreement with ICC of 0.99 (0.98-0.99) and COV of 5%. For interobserver agreement, ICC was 0.89 (0.8-0.934) and COV was 17%. There was poor interobserver agreement for diffuse disease (ICC = 0.46) and small lesions (ICC = 0.54).For test-retest reproducibility, excellent ICC (0.916) and COV (14.5%) values for mean ADC measurements were observed. ICCs of test-retest were similar between focal lesions (0.83) and diffuse infiltration (0.80), while ICCs were higher in pelvic (0.95) compared to thoracic (0.81) region and in small (0.96) compared to large (0.8) lesions.ConclusionADC measurements of focal lesions in multiple myeloma are repeatable and reproducible, while there is more variation in ADC measurements of diffuse disease in patients with multiple myeloma.Advances in knowledgeMean ADC measurements are repeatable and reproducible in focal lesions in multiple myeloma, while the ADC measurements of diffuse disease in multiple myeloma are more subject to variation. The evidence supports the future potential role of ADC measurements as predictive quantitative biomarker in multiple myeloma.
