Analyzing the national fire incident reporting system to identify carbon monoxide incidents in the U.S. lodging industry.

ABSTRACT: Carbon Monoxide (CO) poisonings in the U.S. lodging industry have become a regular occurrence, however there is no current mandatory national reporting, tracking, or surveillance mechanism for CO incidents in the U.S. lodging industry. As such, the problem is largely invisible. The objective of this study was to utilize the National Fire Incident Reporting System (NFIRS) data to better understand the public health risk from Carbon Monoxide (CO) incidents in the U.S. lodging industry. The NFIRS datasets for years 1999 through 2018 were assessed to identify CO incidents occurring at U.S. hotels and motels. The results of the analysis identified 3405 incidents. Incidents were strongly correlated with increased fire department participation in NFIRS (R = 0.82). The number and frequency of CO incidents in the U.S. lodging industry are underreported. Previous efforts relying on news media identified only 10% of the incidents reported in the NFIRS data. This indicates a greater public health risk associated with CO exposure in the U.S. lodging industry than previously realized.

Project description:BackgroundThe study objectives were to describe the incidence and the nature of patient safety incidents (PSIs) in primary care general practice settings, and to explore the association between these incidents and practice or organizational characteristics.MethodsGPs, randomly selected from a national influenza surveillance network (n = 800) across France, prospectively reported any incidents observed each day over a one-week period between May and July 2013. An incident was an event or circumstance that could have resulted, or did result, in harm to a patient, which the GP would not wish to recur. Primary outcome was the incidence of PSIs which was determined by counting reports per total number of patient encounters. Reports were categorized using existing taxonomies. The association with practice and organizational characteristics was calculated using a negative binomial regression model.Results127 GPs (participation rate 79%) reported 317 incidents of which 270 were deemed to be a posteriori judged preventable, among 12,348 encounters. 77% had no consequences for the patient. The incidence of reported PSIs was 26 per 1000 patient encounters per week (95% CI [23‰ -28‰]). Incidents were three times more frequently related to the organization of healthcare than to knowledge and skills of health professionals, and especially to the workflow in the GPs' offices and to the communication between providers and with patients. Among GP characteristics, three were related with an increased incidence in the final multivariable model: length of consultation higher than 15 minutes, method of receiving radiological results (by fax compared to paper or email), and being in a multidisciplinary clinic compared with sole practitioners.ConclusionsPatient safety incidents (PSIs) occurred in mean once every two days in the sampled GPs and 2% of them were associated with a definite possibility for harm. Studying the association between organizational features of general practices and PSIs remains a major challenge and one of the most important issues for safety in primary care.

Project description:BackgroundFire is a major contributor to global fatalities and disabilities and remains a prevalent threat to individuals in Australia. New South Wales (NSW) experiences over one-third of all structure fires, including residential ones, in the country and one-quarter of all associated deaths and hospital admissions. In an atmosphere of rising healthcare cost and heightened demand for its services, obtaining total cost data for residential fires is essential. The study aims to estimate the cost of health service utilisation, including ambulance, emergency, and hospital admission, resulting from residential fire incidents in NSW, Australia.MethodThis population-based cohort study uses response and health service administration data from 1 January 2005 to 31 December 2014. The data sources are Australian Computer Aided Dispatch system, Fire and Rescue NSW Australian Incident Reporting System, NSW Ambulance datasets, NSW Emergency Department Data Collections, and NSW Admitted Patient Data Collection which are administrative datasets that have been linked. Cost figures were expressed in constant 2023 Australian dollars.ResultsThe estimated annual average cost for ambulance use was AU$75 thousand, with the average cost per use of AU$945. The average annual cost for emergency department visits was AU$137 thousand, and the average cost per visit was AU$890. The average total length of stay in hospital was nine days. The average annual cost of episodes of care in hospital was estimated at AU$4 million, with the average cost per episode of care of AU$16,140. The study assessed the total cost for health service use relevant to residential fire incidents and associated injuries at AU$46 million over time, averaging approximately AU$5 million annually.ConclusionThis study provided, for the first time, total and average costs of health service utilisation per type for ambulance use, ED visits, and hospital admissions associated with residential fire incidents and related injuries using linked administrative data in NSW. Our study will assist the government and stakeholders in making informed decisions that prioritise funding for healthcare service and improve the quality of overall fire safety and public health outcomes.
