Negative perceptions and emotional impact of striae gravidarum among pregnant women.

ABSTRACT:

Background

The impact of striae gravidarum (SG), or stretch marks of pregnancy, on quality of life (QoL) is unclear.

Objective

The purpose of this study was to investigate how SG affect QoL in pregnant women.

Methods

In this cross-sectional survey study of healthy pregnant women who developed SG during their current pregnancy, we asked about the impact of lesions on emotional, psychological, and life-quality facets. Spearman product-moment correlation coefficients were generated to determine the strength of relationships between variables.

Results

We analyzed 116 valid surveys. Participants reported permanency of SG as the top physical concern (n = 87; 75%). With regard to severity, nearly three-quarters of participants rated their lesions as very prominent (n = 24; 21%) or moderate (n = 57; 49%). Among the life-quality facets queried, embarrassment/self-consciousness was the most frequently associated with SG, with over one-third of participants reporting "a lot" (n = 19; 16%) or a "moderate" (n = 26; 22%) amount of embarrassment/self-consciousness related to having SG. Lesion severity significantly correlated with the degree of embarrassment/self-consciousness (r = .543), as well as the impact of SG on other life-quality facets, including overall QoL (r = .428), clothing choice (r = .423), self-image/self-esteem (r = .417), feelings of anxiety/depression (r = .415), and social activities (r = .313; all p ≤ .001). Nearly one-quarter of participants believed that emotional distress related to SG was similar or greater than that caused by other skin problems, such as acne, psoriasis, or eczema.

Conclusion

SG can be associated with a host of negative reactions reflecting increased psychological and emotional distress, including embarrassment and decreased QoL. These consequences may compound the emotional stress of pregnancy, potentially warranting psychological support and adjustment strategies.

Project description:BackgroundStriae gravidarum (stretch marks) are considered the most common connective tissue/skin change in pregnancy. Though not a health issue they can affect women in different ways, for example, cause stress or be an aesthetic or cosmetic concern. Many women use one or more of the commercially available products to try and prevent their development during pregnancy despite the fact that that there is a lack of high-quality evidence to support their use. There is a dearth of studies on the prevention of striae gravidarum and large, robust trials are lacking. Until such time as more products are investigated, much of the knowledge remains anecdotal. This pilot study will evaluate the feasibility of conducting a study to evaluate the effectiveness of a commercially available moisturising oil compared to no treatment for the prevention and reduction in severity of striae gravidarum.MethodsThe definitive study will be a randomised controlled trial to evaluate the effectiveness of a moisturising oil (commercially available moisturising oil) compared to no treatment for the prevention and reduction in severity of striae gravidarum. This protocol is for a pilot randomised trial to evaluate the feasibility of conducting such a study. The pilot study will be a two-arm, unblinded, pragmatic parallel randomised trial with a 1:1 randomisation ratio between control and intervention groups. Women in the intervention group will be asked to apply a moisturising oil to their abdomen during pregnancy, while women in the control group will not use any treatment. It is proposed to recruit 20 primigravida, who are 12-16 weeks pregnant from an Irish Maternity Hospital, in each arm to assess the feasibility of running such a trial.DiscussionThis pilot trial will evaluate the feasibility of conducting the main study to evaluate the effectiveness of a moisturising oil (commercially available moisturising oil) compared to no treatment for the prevention and reduction in severity of striae gravidarum. It will potentially initiate the generation of high-quality evidence to guide women in their choice of anti-stretch mark product.Trial registrationISRCTN Registry, ISRCTN76992326 . Project description:BackgroundHyperemesis gravidarum is severe nausea and excessive vomiting, starting between 4 and 6 gestational weeks, peak at between 8 and 12 weeks and usually improve and subside by 20 weeks of pregnancy. Identifying the determinants of hyperemesis gravidarum has a particular importance for early detection and intervention to reduce the health, psychosocial and economic impact. In Ethiopia there is low information on determinants of hyperemesis gravidarum.MethodsInstitution based unmatched case-control study design was conducted from April 12- June 12, 2021. A structured face-to-face interviewer administered questionnaire and checklist for document review were used to collect the data from 360 study participants (120 cases and 240 controls). The data were collected by KoBocollect 1.3, and then exported to statistical package for social science version 25 for further analysis. Both bi-variable and multivariable logistic regression analysis were done to identify the determinants and a p-value < 0.05 with a 95% confidence level was used to declare statistical significance.ResultBeing an urban dweller (AOR = 2.1, 95% CI: 1.01, 4.34), having polygamous husband (AOR = 2.92, 95% CI: 1.27, 6.68), having history asthma/ other respiratory tract infections (AOR = 3.56, 95% CI: 1.43, 8.82), saturated fat intake (AOR = 4.06 95% CI: 1.98, 8.3), no intake of ginger (AOR = 3.04 95% CI: 1.14, 8.09), and inadequate intake of vitamin B rich foods (2.2, 95% CI: 1.14-4.2) were the determinants of hyperemesis gravidarum.ConclusionThis study revealed that, urban residence, having polygamous husband, history of asthma/other respiratory tract infections, intake of saturated fat, no intake of ginger, inadequate intake of vitamin B reach foods were found to be independent determinants of hyperemesis gravidarum. It is better if healthcare providers and government authorities exert continual effort to give health education and counselling service concerning to dietary practice and asthma attacks. It is advisable if pregnant women adhere to healthy diets and limit intake of saturated fats and also husband and nearby relatives give care and support for pregnant women.

Project description:BackgroundStudies focusing on the occurrence and correlates of hyperemesis gravidarum in Ethiopia have reported varied values in different regions of the country. Additionally, there is no systematic review and meta-analysis summarizing the prevalence of hyperemesis gravidarum and its determinants in Ethiopia. Hence, this systematic review and meta-analysis aimed to estimate the overall prevalence of hyperemesis gravidarum and explore its determinants in Ethiopia.MethodsCross-sectional or case-control studies conducted in Ethiopia, written in English, and reporting the prevalence or the determinant of hyperemesis gravidarum among pregnant women were included in the review. International databases (PubMed, Scopus, Cochrane Library, Google Scholar, Science Direct, African Journal Online, Directory of Open Access Journal, and African Index Medicus) and Ethiopian university repositories (Jimma, Addis Ababa, Haramaya, Hawassa, and Gondar Universities) were searched from September 4-15, 2023, to identify articles published on the topic. The pooled prevalence of hyperemesis gravidarum with a 95% confidence interval was presented using the forest plots. The heterogeneity of the studies was checked by I2 with its corresponding p-values and the Galbraith plot. Subgroup analysis and meta-regression were performed to identify sources of heterogeneity. Funnel plot, Egger, and Begg's tests were used to assess publication bias.ResultsA total of 11 articles with a 3510-sample size were included in this systematic review and meta-analysis. The pooled prevalence of hyperemesis gravidarum among pregnant women in Ethiopia was 7.12% with a 95% CI (4.09-10.15) and a high level of heterogeneity (I2 = 86.5%, p<0.001). Subgroup analyses revealed the overall prevalence of hyperemesis gravidarum was highest in the Amhara region with 11.30%, 95% CI (8.20-14.40), and lowest in Oromia with 3.40%, 95% CI (1.94-4.85). Having a previous history of hyperemesis gravidarum (POR = 3.828, 95% CI: 1.673-5.983), being in the first trimester of pregnancy (POR = 8.476, 95% CI: 5.047-11.905), and Helicobacter pylori infection (POR = 3.924, 95% CI: 2.027-5.821) were found to be significantly associated with hyperemesis gravidarum in Ethiopia.ConclusionThe prevalence of hyperemesis gravidarum among pregnant women in Ethiopia is high. Targeting pregnant women in the first trimester, with a previous history of hyperemesis gravidarum, and those with Helicobacter pylori infection during prenatal counseling on how to manage and reduce hyperemesis gravidarum is very helpful to avert related complications.RegistrationThe review was registered in the International Prospective Register of Systematic Reviews (PROSPERO) with the registration number "CRD42023461808", on September 19, 2023.

Project description:ObjectivesHyperemesis gravidarum is a severe form of nausea and vomiting during pregnancy sufficiently enough to produce weight loss greater than 5%, dehydration, ketosis, alkalosis, and hypokalemia. Several studies have investigated risk factors for hyperemesis gravidarum in Ethiopia, but the studies have reported conflicting results attributed to study design, lack of proper sample size, and the selection of variables. This study aimed to assess the determinants of hyperemesis gravidarum among pregnant women in public hospitals of Guji, West Guji, and Borana zones, Southern Ethiopia, 2022.MethodsAn institutional-based case-control study design was conducted from April 15 to June 15, 2022 with a ratio of 1:2 (103 cases and 206 controls). Cases were all pregnant women admitted with a diagnosis of hyperemesis gravidarum by a clinician while controls were pregnant women who were visiting antenatal care services at the same time. Cases were selected consecutively until the required sample size is attained, while controls were selected by a simple random sampling technique. Data were collected using structured questionnaires with face-to-face interviews. The collected data were cleaned, coded, and entered into EpiData version 3.1, and then exported to SPSS version 25 for analysis. Frequency distribution for categorical variables, median, and interquartile range for continuous variables was computed. Backward stepwise logistic regression analyses were done. A significant association was declared with a 95% confidence interval at a p value less than 0.05.ResultsThose mothers who had antenatal follow-up (adjusted odds ratio = 0.082, 95% confidence interval: 0.037-0.180), pregnancy with multiple gestations (adjusted odds ratio = 3.557, 95% confidence interval: 1.387-9.126), previous history of hyperemesis gravidarum (adjusted odds ratio = 6.66, 95% confidence interval: 2.57-17.26), family history of hyperemesis gravidarum (adjusted odds ratio = 2.067, 95% confidence interval: 1.067-4.015), and those women had exercised before pregnancy (adjusted odds ratio = 0.352, 95% confidence interval: 0.194-0.639) were determinants of hyperemesis gravidarum.ConclusionAntenatal follow-up, number of the fetus, previous and family history of hyperemesis gravidarum, and exercise before pregnancy were significantly associated with outcome. Lifestyle modification, early treatment, and early ultrasound scans for pregnant women are crucial to reducing the burden of hyperemesis gravidarum.

Project description:Pregnant women and their newborns have an increased risk of developing severe influenza and influenza-related complications. In Germany, seasonal influenza vaccination is recommended for pregnant women since 2010. However, little is known about pregnant women's vaccination-related knowledge and attitudes, as well as their risk perceptions. This study therefore assessed pregnant women's vaccination-related knowledge, risk perceptions related to influenza disease and influenza vaccination during pregnancy, and aimed to identify determinants of influenza vaccination uptake during pregnancy in Germany.Between 2012 and 2014, a nationwide web-based prospective cohort study with follow-up interviews was conducted in initially pregnant women who gave birth over the study period. Control groups were set up in a cross-sectional fashion during the follow-up interviews. Women who participated in both, the baseline interview before giving birth and in the 1st interview after giving birth were included in the analysis. Univariate and multiple logistic regression were used to identify associations between influenza vaccination uptake and sociodemographic characteristics as well as items assessing attitude and knowledge.In total, 838 women were included in the analyses. Pregnant women had a positive attitude towards vaccination in general, but only modest vaccination knowledge. Overall, 10.9 % of women were vaccinated against seasonal influenza during pregnancy. While pregnant women perceived classical childhood diseases to be more risky than the respective vaccinations, this relation reversed for influenza: The risk of vaccination was perceived higher than the risk of the disease. These two types of risk perceptions independently determined influenza vaccination uptake-higher perception of disease risk and lower perceptions of vaccination-related risks increased uptake. Additionally, knowledge about the vaccination recommendation for pregnant women and a positive gynaecologist's attitude towards vaccination during pregnancy influenced the uptake significantly.Influenza vaccination uptake in pregnant women is low in Germany. Tailored communication strategies for pregnant women should focus especially on changing the perceptions of personal risks regarding influenza and influenza vaccination during pregnancy. Gynaecologists should be made aware about their crucial role in supporting vaccination decision-making of pregnant women and the need to provide relevant information to counteract misconceptions.

Project description:BackgroundAdequate gestational weight gain affects birth outcomes and increases the risk of non-communicable diseases later in life. Weight gain in pregnant Ethiopian women with hyperemesis gravidarum has not been investigated comprehensively.ObjectiveTo assess the determinants of weight gain in pregnant women with hyperemesis gravida in Dire Dawa Administration, Eastern Ethiopia.DesignUnmatched case-control studyMethods:Pregnant women who visited health facilities for ante-natal care were recruited into this study using random sampling techniques. The World Health Organization Global Physical Activity Questionnaire, FANTA Version III Women's Dietary Diversity Score Questionnaire, and anthropometric measurements were used for the assessment of physical activities, dietary diversity, and gestational weight gain, respectively. Bivariate and multivariate logistic regression analyses with a 95% confidence interval (CI) and a Hosmer-Lemeshow goodness model were used to identify the determinants of gestational weight gain in the study population. A P value of <0.05 was considered significant.ResultsA total of 657 pregnant women (219 patients and 438 controls) were included in this study. Among them, 44.3% and 47% of those in the patient and control groups, respectively, gained adequate weight during pregnancy (Std. 0.499: 95% CI: 42-49.8). Gravidity (adjusted odds ratio (aOR): 0.43, 95%CI: 0.22-0.81), planned pregnancy (aOR: 0.28, 95%CI: 0.11-0.69), and pre-pregnancy weight (aOR: 0.16, 95%CI: 0.72-0.74) were determinants of weight gain in the patient group, whereas the secondary level of education (aOR: 0.59, 95%CI: 0.36-0.97) and pre-pregnancy weight (aOR: 0.20, 95%CI: 0.08, 0.50) were determinants of gestational weight gain in the control group.ConclusionThe main determinants of gestational weight gain in women with hyperemesis gravidarum are gravidity, planned pregnancy, and pre-pregnancy weight. However, educational status and pre-pregnancy weight are the main determinants of gestational weight gain in women without hyperemesis gravidarum. Strengthening the nutritional life-cycle approach during pregnancy is important for ensuring that mothers maintain a healthy gestational weight.
