Ovarian vein thrombosis: a rare but not to be forgotten cause of abdominal pain.

ABSTRACT: A healthy 38-year-old woman, with no significant medical or surgical history, was referred to the gynaecology emergency department with acute low quadrant abdominal pain and nausea. On examination, painful palpation of right iliac fossa, abdominal guarding and positive Blumberg's sign were noted. An abdominal-pelvic CT scan showed a hypodense tubular structure with peripheral enhancement to the right of the inferior vena cava, suggestive of thrombosis of the right ovarian vein. The only risk factor identified was the use of the combined contraceptive pill.

Cancer-Associated Abdominal Vein Thrombosis.

Project description:Cancer is associated with an increased risk of developing venous thromboembolism, due to its direct influence on the three pillars of Virchow's triad (e.g., compression on the blood vessels by the tumour, blood vessels invasion, and cytokine release), together with the effect of exogenous factors (such as chemotherapy, radiotherapy, surgery). In cancer patients, the risk of thrombosis at unusual sites, such as splanchnic, ovarian and renal vein thrombosis, is also increased. Abdominal vein thromboses are frequently incidental findings on abdominal imaging performed as part of the diagnostic/staging workup or the follow-up care of malignancies. There is little evidence on the management of unusual site venous thromboembolism in cancer patients since there are only a few specific recommendations; thus, the management follows the general principles of the treatment of cancer-associated deep vein thrombosis and pulmonary embolism. This narrative review summarises the latest evidence on cancer-associated abdominal vein thrombosis, i.e., thrombosis of the splanchnic, ovarian and renal veins.

Cystic Adventitial Disease of the Popliteal Vein, a Rare Cause of Lower Limb Deep Vein Thrombosis.

Project description:IntroductionCystic adventitial disease (CAD) is characterised by the accumulation of gelatinous fluid within the adventitial layer of a blood vessel. Over 90% of CAD occurs in the arterial system. Venous CAD most commonly involves the iliofemoral rather than the popliteal segments.ReportThis is the report of a 49 year old female patient with a previous right leg deep vein thrombosis (DVT). She presented to a vascular outpatient appointment with recurrent right lower extremity swelling. Venous duplex ultrasound showed an ectatic and incompetent right popliteal vein. Computed tomography (CT) venography showed focal ectasia of the right popliteal vein resulting from an eccentric low density cyst with a diameter of 15 mm. Under general anaesthesia, the patient was placed in the prone position. A lazy S incision was performed in the right popliteal fossa. The popliteal vein had an eccentrically thickened lateral bulge. After heparinisation, a longitudinal venotomy, endophlebectomy, and en bloc cyst removal were performed sequentially. Popliteal patch venoplasty was performed subsequently using the ipsilateral small saphenous vein. After six months, the patient remains on rivaroxaban. A follow up venous duplex ultrasound showed vein reflux through a standard calibre popliteal vein without evidence of cyst recurrence.ConclusionVenous CAD is a rare disease and should be considered if previous DVT or symptoms mimicking DVT occur. Cyst resection and reconstruction with vein patch, venous or synthetic graft is the most commonly used strategy and has lower rates of cyst recurrence and need for re-operation.

Diagnosis and management of right external iliac vein "sandwich": A rare cause of iliofemoral deep venous thrombosis.

Project description:Several anatomic abnormalities predispose patients to iliofemoral deep venous thrombosis, the most common of which is compression of the left iliac vein between the right common iliac artery and lumbar vertebrae, or May-Thurner syndrome. Other areas of venous compression can occur but are rare. This case report describes the presentation, diagnosis, and management of a patient with compression of the right iliac vein "sandwiched" between the right internal and external iliac arteries. After treatment, the patient demonstrated significant improvement in symptoms.

Splenic vein thrombosis in cirrhosis of the liver: A rare case.

Project description:Splenic Vein Thrombosis (SpVT) in a young patient with non-hepatitis B and C liver cirrhosis is an infrequent case generating hemorrhagic manifestations. Herein we report a 28-year-old man presenting with hematemesis, melena, and features of liver cirrhosis. Hematemesis, melena, and ascites resolve following a conservative treatment. Abdominal ultrasound confirmed portal hypertension. Serial endoscopy on day 14, 17 and 1-month evaluation showed grade II-III esophageal varices and severe hypertensive portal gastropathy. Abdominal CT scan with contrast within 1 week after discharge revealed thrombus along ± 5.8 cm, splenomegaly with dilated splenic vein, dilatation and tourtosity of the left gastric vein and visualized distal esophageal vein. Liver biopsy 2 months after hospitalization showed hepatocytes with extensive hydropic degeneration with fibrosis (F3).

Foetal death due to extensive extra-abdominal umbilical vein Varix with umbilical vein thrombosis: a case report.

Project description:BackgroundFoetal anaemia and umbilical vein thrombosis are rare pregnancy complications that can increase the risk of perinatal adverse events, which, in severe cases, can lead to foetal death. During pregnancy, umbilical vein varix (UVV) commonly occurs in the intra-abdominal part of the umbilical vein and is associated with an increased risk of foetal anaemia and umbilical vein thrombosis. However, UVV occurring in the extra-abdominal part of the umbilical vein is rare, especially when accompanied by thrombosis. In this case report, we describe a rare case of an extensive extra-abdominal umbilical vein varix (EAUVV), which ultimately resulted in foetal death due to umbilical vein thrombosis.Case presentationIn this report, we describe a rare case of an extensive EAUVV that was discovered at 25 weeks and 3 days of gestation. During the examination, there were no abnormalities in foetal haemodynamics. The estimated weight of the foetus was only 709 g. In addition to refusing to be hospitalized, the patient refused close monitoring of the foetus. As a result, we were limited to choosing an expectant therapy. The foetus died 2 weeks after diagnosis and was confirmed to have EAUVV with thrombosis after the induction of labour.ConclusionIn the case of EAUVV, lesions are extremely rare, and it is very easy for thrombosis to form, which may result in the death of the child. When determining the next step in the treatment of the condition, the degree of UVV, possible complications, gestational age, foetal haemodynamics, and other relevant factors are strongly connected to the clinical therapy decision, and these factors should be considered comprehensively when making a clinical decision. We recommend close monitoring with hospital admission (to facilities capable of handling extremely preterm foetuses) after variability in delivery for worsening haemodynamic status.

A unique cause of chronic abdominal pain.

Project description:The images presented here demonstrated how we arrived at the diagnosis of Ascaris lumbricoides infection in a 54-year-old man with chronic abdominal pain by capsule endoscopy (CE). In this case, computed tomography (CT) images were not representative, and further investigation with CE was required to confirm the diagnosis. The combination of CT and CE was useful for diagnosing Ascaris lumbricoides infection in this patient.

Hematocrit Sign Elucidates Cause of Abdominal Pain.

Project description:A 78-year-old male with chronic kidney disease on peritoneal dialysis developed unprovoked bilateral pulmonary embolisms. He was started on IV unfractionated heparin, but shortly thereafter developed severe pain and a small firm abdominal nodule near his dialysis catheter site. The diagnosis was unknown, and the initial plan was watchful waiting, until point-of-care ultrasound (POCUS) was used. POCUS revealed an ovoid mass with hyperdensity in the gravity dependent regions with spontaneous movement. This appearance was classic for the hematocrit sign. When combined with the clinical presentation, this was concerning for a rectus sheath hematoma. An urgent CT of the abdomen confirmed this several hours later. POCUS allowed for rapid bedside diagnosis, which expedited appropriate care in a potentially life-threatening situation.

Portal vein thrombosis.

Project description:Portal vein thrombosis is an important cause of portal hypertension. PVT occurs in association with cirrhosis or as a result of malignant invasion by hepatocellular carcinoma or even in the absence of associated liver disease. With the current research into its genesis, majority now have an underlying prothrombotic state detectable. Endothelial activation and stagnant portal blood flow also contribute to formation of the thrombus. Acute non-cirrhotic PVT, chronic PVT (EHPVO), and portal vein thrombosis in cirrhosis are the three main variants of portal vein thrombosis with varying etiological factors and variability in presentation and management. Procoagulant state should be actively investigated. Anticoagulation is the mainstay of therapy for acute non-cirrhotic PVT, with supporting evidence for its use in cirrhotic population as well. Chronic PVT (EHPVO) on the other hand requires the management of portal hypertension as such and with role for anticoagulation in the setting of underlying prothrombotic state, however data is awaited in those with no underlying prothrombotic states. TIPS and liver transplant may be feasible even in the setting of PVT however proper selection of candidates and type of surgery is warranted. Thrombolysis and thrombectomy have some role. TARE is a new modality for management of HCC with portal vein invasion.

Deep vein thrombosis in a nonobstructive azoospermia male taking tamoxifen: a rare case report.

Project description:Tamoxifen is used for male infertility and nonobstructive azoospermia (NOA). Although thrombosis complication of tamoxifen on the treatment of breast cancer has been reported repeatedly, there was no literature about the thrombosis complication of tamoxifen treatment on NOA. A 32-year-old man was admitted to hospital for severe swelling of left lower limb, with difficulty walking. He had been diagnosed with NOA 5 months ago and had been taking tamoxifen 20 mg daily for 4 months continuously. After admission, the patient was finally diagnosed of deep vein thrombosis (DVT) with elevated D-dimer level and Doppler ultrasound of the deep venous system. After a series of effective treatments, especially the operation of percutaneous venous thromboembolism aspiration, the patient recovered rapidly and the abnormal laboratory results of coagulopathy returned to normal. Clinicians should warn about the possibility of thromboembolic complications with tamoxifen when treating male infertility.

Project description:Rationale Aggressive fibromatosis is a rare and locally infiltrative monoclonal fibroblastic proliferation with lack of metastatic potential. We describe a rare case of intra-abdominal aggressive fibromatosis on young female with hyperemesis. Patient concerns A 23-year-old female was admitted with hyperemesis and loss of weight. Diagnoses According to imaging findings and immunohistology findings, a diagnosis of intra-abdominal aggressive fibromatosis was formulated. Outcomes After the surgery, no evidence of local recurrence was noted during the 6 months of follow-up. Lessons AF may explain why pregnant women may have severe hyperemesis.
