ABSTRACT: Thoracoscopic segmentectomy and subsegmentectomy have been widely accepted for the treatment of peripheral small lung cancers. Thoracoscopic basal subsegmentectomy, especially when performed through a uniportal procedure, is extremely technically challenging, and therefore there are seldom reports of its technical details. In this article, we present a uniportal thoracoscopic left S10a+ci subsegmentectomy following the single-direction strategy through the inferior pulmonary ligament approach.

Uniportal thoracoscopic left posterior basal segmentectomy using a posterior approach.

Project description:Posterior basal (S10) segmentectomy is one of the most challenging (and uncommon) types of pulmonary segmentectomy. Here, we present two key tips for facilitating a uniportal operation. The first is a full understanding of the relative locations of the pulmonary vessels and bronchi (as revealed by preoperative three-dimensional computed tomography/broncho-angiography), and the other is the use of "suction-guided stapling" to dissect and divide the peripheral pulmonary vessels and bronchi. We describe the successful postoperative course of a patient who was so treated.

A combined subsegmentectomy of right S9 + 10b with single direction thoracoscopic surgery.

Project description:Segmentectomy is widely used to treat pulmonary nodules and more functional lungs can be preserved in patients. For pulmonary nodules deep near the intersegmental border, only one single segmentectomy may not achieve adequate surgical margins, and combined subsegmental resection becomes the most suitable treatment option. Thoracoscopic combined anatomical resections involving both of right S9 and S10b are one of the most challenging cases, especially in the right chest. We previously reported a case of combined subsegmental resection of the left complex basal segment (LS9b + 10b). To our knowledge, there has been no report of combined subsegmental resection of the right S9 and S10b. Here, we aim to introduce a different technique named as "open-gate", which means that the intersegmental border between S7 and S10 was cut open along the intersegmental septa, to deal with complex combined basal subsegmental resections.

Feasibility of Uniportal Thoracoscopic Right Medial-Basal (S7) Segmentectomy.

Project description:Thoracoscopic right medial-basal (S7) segmentectomy is technically challenging due to its small size, depth, and anatomical complexity, especially through a uniportal thoracoscopic approach because of the limited angulation of the surgical instruments or staplers used. Herein, we report a successful case of right S7 segmentectomy through a uniportal approach with sufficient surgical margin. Key aspects are to mobilize the lung by dissecting the pulmonary ligament, dividing a fissure, and exposing the pulmonary vein branches to the periphery, which allowed us to perform smooth and safe stapling through the single skin incision.

Uniportal video-assisted thoracoscopic left upper lobectomy under spontaneous ventilation.

Project description:Intubated general anesthesia with one-lung ventilation was traditionally considered necessary for thoracoscopic major pulmonary resections. However, non-intubated thoracoscopic lobectomy can be performed by using conventional and uniportal video-assisted thoracoscopic surgery (VATS). These non-intubated procedures try to minimize the adverse effects of tracheal intubation and general anesthesia but these procedures must only be performed by experienced anesthesiologists and skilled thoracoscopic surgeons. Here we present a video of a uniportal VATS left upper lobectomy in a non-intubated patient, maintaining the spontaneous ventilation.

Left sleeve pneumonectomy via uniportal video-assisted thoracoscopic approach.

Project description:Left sleeve pneumonectomy is a rarer intervention compared with right sleeve pneumonectomy. It is a challenging surgical therapeutic strategy even when performed through open thoracotomy. Here, we report a case of uniportal video-assisted thoracoscopic surgery (VATS) left sleeve pneumonectomy in a patient with non-small cell lung cancer. The tumor, located at the opening of left upper lobe bronchus, submucosally invaded the orifice of lower lobe extending upward to 4 to 5 cartilage rings of the left main bronchus and to the level of the carina. Left sleeve pneumonectomy and airway reconstruction was performed through video-assisted thoracoscopic completely with an incision of 4 cm. The total operative time was 220 minutes and the estimated intraoperative blood loss was 300 mL. Chylothorax occurred after surgery, which was well handled, and no other severe complication was observed. Three months after the surgery, the follow-up bronchoscopy revealed good healing of the anastomosis. No signal of tumor recurrence was observed by follow-up examination 1 year after the surgery. To our knowledge, this is the first reported uniportal VATS left sleeve pneumonectomy in the world. It was indicated that uniportal VATS might be a feasible approach for left sleeve pneumonectomy, with less surgical trauma compared with other approaches.

Left lower sleeve lobectomy by uniportal video-assisted thoracoscopic approach.

Project description:Endobronchial tumours requiring sleeve resection have been usually considered a contraindication for video-assisted thoracoscopic surgery (VATS). However, with new technical advances and the experience gained in VATS, sleeve lobectomy has been performed by thoracoscopy in experienced VATS centres. Right-sided sleeve anastomoses are easier to perform by VATS than left-sided ones because of the presence of the pulmonary artery and aortic arch on the left side. Most surgeons use a 3 to 4 incision VATS technique for sleeve anastomosis but the surgery can be performed by using only one incision. This is the first report of a left-sided sleeve lobectomy by uniportal approach.
