Project description:ObjectiveTo examine performance of a sepsis surveillance system in a simulated environment where modifications to parameters and settings for identification of at-risk patients can be explored in-depth.Materials and methodsThis was a multiple center observational cohort study. The study population comprised 14 917 adults hospitalized in 2016. An expert-driven rules algorithm was applied against 15.1 million data points to simulate a system with binary notification of sepsis events. Three system scenarios were examined: a scenario as derived from the second version of the Consensus Definitions for Sepsis and Septic Shock (SEP-2), the same scenario but without systolic blood pressure (SBP) decrease criteria (near SEP-2), and a conservative scenario with limited parameters. Patients identified by scenarios as being at-risk for sepsis were assessed for suspected infection. Multivariate binary logistic regression models estimated mortality risk among patients with suspected infection.ResultsFirst, the SEP-2-based scenario had a hyperactive, unreliable parameter SBP decrease >40 mm Hg from baseline. Second, the near SEP-2 scenario demonstrated adequate reliability and sensitivity. Third, the conservative scenario had modestly higher reliability, but sensitivity degraded quickly. Parameters differed in predicting mortality risk and represented a substitution effect between scenarios.DiscussionConfiguration of parameters and alert criteria have implications for patient identification and predicted outcomes.ConclusionPerformance of scenarios was associated with scenario design. A single hyperactive, unreliable parameter may negatively influence adoption of the system. A trade-off between modest improvements in alert reliability corresponded to a steep decline in condition sensitivity in scenarios explored.

Project description:Recent studies have suggested reduced independent and sensitive parameter sets for stabilometry measurements based on correlation and variance analyses. However, the reliability of these recommended parameter sets has not been studied in the literature or not in every stance type used in stabilometry assessments, for example, single leg stances. The goal of this study is to evaluate the test-retest reliability of different time-based and frequency-based parameters that are calculated from the center of pressure (CoP) during bipedal and single leg stance for 30- and 60-second measurement intervals. Thirty healthy subjects performed repeated standing trials in a bipedal stance with eyes open and eyes closed conditions and in a single leg stance with eyes open for 60 seconds. A force distribution measuring plate was used to record the CoP. The reliability of the CoP parameters was characterized by using the intraclass correlation coefficient (ICC), standard error of measurement (SEM), minimal detectable change (MDC), coefficient of variation (CV) and CV compliance rate (CVCR). Based on the ICC, SEM and MDC results, many parameters yielded fair to good reliability values, while the CoP path length yielded the highest reliability (smallest ICC > 0.67 (0.54-0.79), largest SEM% = 19.2%). Usually, frequency type parameters and extreme value parameters yielded poor reliability values. There were differences in the reliability of the maximum CoP velocity (better with 30 seconds) and mean power frequency (better with 60 seconds) parameters between the different sampling intervals.

Project description:The metabolism of all living organisms is dependent on temperature, and therefore, having a good method to predict temperature effects at a system level is of importance. A recently developed Bayesian computational framework for enzyme and temperature constrained genome-scale models (etcGEM) predicts the temperature dependence of an organism's metabolic network from thermodynamic properties of the metabolic enzymes, markedly expanding the scope and applicability of constraint-based metabolic modelling. Here, we show that the Bayesian calculation method for inferring parameters for an etcGEM is unstable and unable to estimate the posterior distribution. The Bayesian calculation method assumes that the posterior distribution is unimodal, and thus fails due to the multimodality of the problem. To remedy this problem, we developed an evolutionary algorithm which is able to obtain a diversity of solutions in this multimodal parameter space. We quantified the phenotypic consequences on six metabolic network signature reactions of the different parameter solutions resulting from use of the evolutionary algorithm. While two of these reactions showed little phenotypic variation between the solutions, the remainder displayed huge variation in flux-carrying capacity. This result indicates that the model is under-determined given current experimental data and that more data is required to narrow down the model predictions. Finally, we made improvements to the software to reduce the running time of the parameter set evaluations by a factor of 8.5, allowing for obtaining results faster and with less computational resources.

Project description:Computational modeling of behavior is increasingly being adopted as a standard methodology in psychology, cognitive neuroscience, and computational psychiatry. This approach involves estimating parameters in a computational (or cognitive) model that represents the computational processes of the underlying behavior. In this approach, the reliability of the parameter estimates is an important issue. The use of hierarchical (Bayesian) approaches, which place a prior on each model parameter of the individual participants, is thought to improve the reliability of the parameters. However, the characteristics of reliability in parameter estimates, especially when individual-level priors are assumed, as in hierarchical models, have not yet been fully discussed. Furthermore, the suitability of different reliability measures for assessing parameter reliability is not thoroughly understood. In this study, we conduct a systematic examination of these issues through theoretical analysis and numerical simulations, focusing specifically on reinforcement learning models. We note that the heterogeneity in the estimation precision of individual parameters, particularly with priors, can skew reliability measures toward individuals with higher precision. We further note that there are two factors that reduce reliability, namely estimation error and intersession variation in the true parameters, and we discuss how to evaluate these factors separately. Based on the considerations of this study, we present several recommendations and cautions for assessing the reliability of the model parameters.

Project description:A lack of data reproducibility ("reproducibility crisis") has been extensively debated across many academic disciplines. Although a reproducibility crisis is widely perceived, conclusive data on the scale of the problem and the underlying reasons are largely lacking. The debate is primarily focused on methodological issues. However, examples such as the use of misidentified cell lines illustrate that the availability of reliable methods does not guarantee good practice. Moreover, research is often characterized by a lack of established methods. Despite the crucial importance of researcher conduct, research and conclusive data on the determinants of researcher behavior are widely missing. Meta-research that establishes an understanding of the factors that determine researcher behavior is urgently needed. This knowledge can then be used to implement and iteratively improve measures that incentivize researchers to apply the highest standards, resulting in high-quality data.

Project description:Using reinterview data from the PATH Reliability and Validity (PATH-RV) study, we examine the characteristics of questions and respondents that predict the reliability of the answers. In the PATH-RV study, 524 respondents completed an interview twice, five to twenty-four days apart. We coded a number of question characteristics and used them to predict the gross discrepancy rates (GDRs) and kappas for each question. We also investigated respondent characteristics associated with reliability. Finally, we fitted cross-classified models that simultaneously examined a range of respondent and question characteristics. Although the different models yielded somewhat different conclusions, in general factual questions (especially demographic questions), shorter questions, questions that did not use scales, those with fewer response options, and those that asked about a noncentral topic produced more reliable answers than attitudinal questions, longer questions, questions using ordinal scales, those with more response options, and those asking about a central topic. One surprising finding was that items raising potential social desirability concerns yielded more reliable answers than items that did not raise such concerns. The respondent-level models and cross-classified models indicated that five adult respondent characteristics were associated with giving the same answer in both interviews-education, the Big Five trait of conscientiousness, tobacco use, sex, and income. Hispanic youths and non-Hispanic black youths were less likely to give the same answer in both interviews. The cross-classified model also found that more words were associated with less reliable answers. The results are mostly consistent with earlier findings but are nonetheless important because they are much less model-dependent than the earlier work. In addition, this study is the first to incorporate such personality traits as needed for cognition and the Big Five personality factors and to examine the relationships among reliability, item nonresponse, and response latency.

Project description:BackgroundThere is conflicting evidence regarding reliability criteria for the controlled attenuation parameter (CAP; a marker for hepatic steatosis [HS]). Thus, we assessed the diagnostic performance of CAP according to different reliability criteria based on real-world data from an academic centre.MethodsPatients undergoing measurement of CAP and liver biopsy (±6 months) at the Medical University of Vienna were included. HS was assessed according to SAF score.ResultsIn total 319 patients were included. The main aetiologies were non-alcoholic fatty liver disease (NAFLD, n = 177, 55.5%), viral hepatitis (n = 49, 15.4%), and alcoholic liver disease (ALD, n = 29, 9.1%). Histological steatosis and fibrosis stages were: S0: 93 (29.2%), S1: 100 (31.3%), S2: 67 (21.0%), and S3: 59 (18.5%); F0/F1: 150 (47.0%), F2: 47 (14.7%), and F3/F4: 122 (48.3%). In the overall cohort, the area under the receiver operating characteristic curve (AUC) of CAP was 0.843 (95% confidence interval [CI]: 0.798-0.887) for diagnosing HS ≥ S1), 0.789 (95%CI: 0.740-0.839) for ≥S2, and 0.767 (95%CI: 0.712-0.823) for S3. CAP corrections as suggested by Karlas et al. did not improve the diagnostic performance. Importantly, the AUC of CAP for HS ≥ S1 was numerically highest in patients with CAP-IQR/median<0.10 or <0.20 (obtained in 37.9% and 74.9%), in whom CAP also had better diagnostic performance, as compared with patients not meeting these criteria. Moreover, it was substantially higher in 288 (90.3%) patients with CAP-IQR/median<0.3: 0.856 (95%CI: 0.809-0.903) vs. patients not meeting this criterion (0.530 [95%CI: 0.309-0.751]). In contrast, the previously suggested reliability criterion of CAP-IQR<40 dB/m was not associated with an improved diagnostic performance for HS≥S1 (0.866 [95%CI: 0.812-0.920] vs. 0.799 [95%CI: 0.717-0.881]) and was only obtained in 199 (62.4%) patients.ConclusionCAP-IQR/median<0.1, <0.2, and <0.3 identify reliable measurements for diagnosing any hepatic steatosis (≥S1). Importantly, CAP-IQR/median<0.3 has a considerably higher applicability in clinical practice, as compared with the previously suggested CAP-IQR<40 dB/m criterion.

Project description:BackgroundPosterior tibial slope (PTS) exhibits considerable variability among individuals and is anticipated to influence the accuracy of radiographic measurements related to the knee. Despite this potential impact, there is a lack of prior research investigating how PTS affects the accuracy of these measurements. Therefore, this study aimed to investigate the effect of PTS on the measurement reliability regarding the radiographic parameter of the knee.MethodsThe medical records of patients who took full-length anteroposterior radiographs of the lower limb between January 2020 and June 2022 were evaluated retrospectively. Radiographic parameters related to the knee joint characteristics such as osteoarthritis grade, hip-knee-ankle angle, weight-bearing line ratio, medial proximal tibial angle (MPTA), lateral distal femoral angle, joint-line convergence angle (JLCA), and PTS were measured. Subjects were classified into 3 groups according to PTS (group A, PTS < 4°; group B, PTS ≥ 4° and < 8°; group C, PTS ≥ 8°), and the measurement reliability for the radiographic variables was compared between groups. The intra- and inter-observer agreements were assessed using the kappa coefficients, intra-class correlation coefficients (ICC), and Bland-Altman plots.ResultsA total of 175 limbs (86 patients) were included in this study. As the intra- and inter-observer reliability for PTS ranged over 0.9, grouping was performed based on the average of the measured PTSs. The inter-observer reliability of the MPTA and JLCA decreased as the PTS increased (ICCs for MPTA in Groups A, B, and C: 0.889, 0.796, and 0.790, respectively; ICCs for JLCA in Groups A, B and C: 0.916, 0.859, and 0.843, respectively), whereas there were no remarkable differences in other variables. Similar trends were observed in the comparisons of intra-observer reliability and Bland-Altman plots also showed consistent results.ConclusionThe larger the PTS, the lower the measurement reliability regarding the radiographic parameters of the knee that includes the joint line, such as MPTA and JLCA. Given the occasional challenge in accurately identifying the knee joint line in patients with a relatively large PTS, careful measurement of radiographic parameters is crucial and utilizing repetitive measurements for verification may contribute to minimizing measurement errors.

Project description:The optimum temperature is commonly determined in enzyme characterization. A search in the PubMed database for "optimum temperature" and "enzymes" yielded more than 1,700 manuscripts reporting this parameter over the last five years. Here, we show that the optimum temperature is not a constant. The catalytic activity of the mesophylic β-glucosidase Sfβgly was determined at different temperatures using different assay times and enzyme concentrations. We observed that the optimum temperature for Sfβgly changed by 5°C simply by modifying the assay length, and it was inversely correlated with enzyme concentration. These observations rely on the fact that close to the melting temperature, thermal denaturation continuously decreases the active enzyme concentration as the assay progresses. Thus, as the denaturation rate increases with increasing temperature, the bell-shaped curves observed in "activity versus temperature plots" occur only if the enzyme is denatured at and above the optimum temperature, which was confirmed using the thermostable β-glucosidase bglTm. Thus, the optimum temperature hardly reflects an intrinsic enzyme property and is actually a mere consequence of the assay condition. Thus, adoption of the "optimum temperature" determined under bench conditions for large-scale uses, which differ in assay length and enzyme concentration, may result in lower yields and financial losses.

Project description:Running is the basis of many sports and has highly beneficial effects on health. To increase the understanding of running, DSPro® insoles were developed to collect running parameters during tasks. However, no validation has been carried out for running gait analysis. The aims of this study were to assess the test-retest reliability and criterion validity of running gait parameters from DSPro® insoles compared to a motion-capture system. Equipped with DSPro® insoles, a running gait analysis was performed on 30 healthy participants during overground and treadmill running using a motion-capture system. Using an intraclass correlation coefficient (ICC), the criterion validity and test-retest reliability of spatiotemporal parameters were calculated. The test-retest reliability shows moderate to excellent ICC values (ICC > 0.50) except for propulsion time during overground running at a fast speed with the motion-capture system. The criterion validity highlights a validation of running parameters regardless of speeds (ICC > 0.70). This present study validates the good criterion validity and test-retest reliability of DSPro® insoles for measuring spatiotemporal running gait parameters. Without the constraints of a 3D motion-capture system, such insoles seem to be helpful and relevant for improving the care management of active patients or following running performance in sports contexts.
