ABSTRACT: Respiratory rate (RR) is a marker of critical illness, but during hospital care, RR is often inaccurately measured. The capaciflector is a novel sensor that is small, inexpensive, and flexible, thus it has the potential to provide a single-use, real-time RR monitoring device. We evaluated the accuracy of continuous RR measurements by capaciflector hardware both at rest and during exercise. Continuous RR measurements were made with capaciflectors at four chest locations. In healthy subjects (n = 20), RR was compared with strain gauge chest belt recordings during timed breathing and two different body positions at rest. In patients undertaking routine cardiopulmonary exercise testing (CPET, n = 50), RR was compared with pneumotachometer recordings. Comparative RR measurement bias and limits of agreement were calculated and presented in Bland-Altman plots. The capaciflector was shown to provide continuous RR measurements with a bias less than 1 breath per minute (BPM) across four chest locations. Accuracy and continuity of monitoring were upheld even during vigorous CPET exercise, often with narrower limits of agreement than those reported for comparable technologies. We provide a unique clinical demonstration of the capaciflector as an accurate breathing monitor, which may have the potential to become a simple and affordable medical device.Clinical trial number: NCT03832205 https://clinicaltrials.gov/ct2/show/NCT03832205 registered February 6th, 2019.

Project description:BackgroundTachypnea is among the earliest signs of pulmonary decompensation. Contactless continuous respiratory rate monitoring might be useful in isolated COVID-19 patients admitted in wards. We therefore aimed to determine whether continuous monitoring of respiratory patterns in hospitalized patients with COVID-19 predicts subsequent need for increased respiratory support.MethodsSingle-center pilot prospective cohort study in COVID-19 patients who were cared for in routine wards. COVID-19 patients who had at least one escalation of pulmonary management were matched to three non-escalated patients. Contactless respiratory monitoring was instituted after patients enrolled, and continued for 15 days unless hospital discharge, initiation of invasive mechanical ventilation, or death occurred. Clinicians were blinded to respiratory rate data from the continuous monitor. The exposures were respiratory features over rolling periods of 30 min, 24 h, and 72 h before respiratory care escalation. The primary outcome was a subsequent escalation in ventilatory support beyond a Venturi mask.ResultsAmong 125 included patients, 13 exhibited at least one escalation and were each matched to three non-escalated patients. A total of 28 escalation events were matched to 84 non-escalation episodes. The 30-min mean respiratory rate in escalated patients was 23 breaths per minute (bpm) ranging from 13 to 40 bpm, similar to the 22 bpm in non-escalated patients, although with less variability (range 14 to 31 bpm). However, higher respiratory rate variability, especially skewness over 1 day, was associated with higher incidence of escalation events. Our overall model, based on continuous data, had a moderate accuracy with an AUC 0.81 (95%CI: 0.73, 0.88) and a good specificity 0.93 (95%CI: 0.87, 0.99).ConclusionOur pilot observational study suggests that respiratory rate variability as detected with continuous monitoring is associated with subsequent care escalation during the following 24 h. Continuous respiratory monitoring thus appears to be a valuable increment over intermittent monitoring.Strengths and limitationsOur study was the initial evaluation of Circadia contactless respiratory monitoring in COVID-19 patients who are at special risk of pulmonary deterioration. The major limitation is that the analysis was largely post hoc and thus needs to be confirmed in an out-of-sample population.

Project description:The aim of the present study was to assess the accuracy of heart rate to estimate energy cost during eight resistance exercises performed at low intensities: half squat, 45° inclined leg press, leg extension, horizontal bench press, 45° inclined bench press, lat pull down, triceps extension and biceps curl. 56 males (27.5 ± 4.9 years, 1.78 ± 0.06 m height, 78.67 ± 10.7 kg body mass and 11.4 ± 4.1% estimated body fat) were randomly divided into four groups of 14 subjects each. Two exercises were randomly assigned to each group and subjects performed four bouts of 4-min constant-intensity at each assigned exercise: 12%, 16%, 20% and 24% 1-RM. Exercise and intensity order were random. Each subject performed no more than 2 bouts in the same testing session. A minimum recovery of 24h was kept between sessions. During testing VO2 was measured with Cosmed K4b2 and heart rate was measured with Polar V800 monitor. Energy cost was calculated from mean VO2 during the last 30-s of each bout by using the energy equivalent 1 ml O2 = 5 calorie. Linear regressions with heart rate as predictor and energy cost as dependent variable were build using mean data from all subjects. Robustness of the regression lines was given by the scatter around the regression line (Sy.x) and Bland-Altman plots confirmed the agreement between measured and estimated energy costs. Significance level was set at p≤0.05. The regressions between heart rate and energy cost in the eight exercises were significant (p<0.01) and robustness was: half squat (Sy.x = 0,48 kcal·min-1), 45° inclined leg press (Sy.x = 0,54 kcal·min-1), leg extension (Sy.x = 0,59 kcal·min-1), horizontal bench press (Sy.x = 0,47 kcal·min-1), 45° inclined bench press (Sy.x = 0,54 kcal·min-1), lat pull down (Sy.x = 0,28 kcal·min-1), triceps extension (Sy.x = 0,08 kcal·min-1) and biceps curl (Sy.x = 0,13 kcal·min-1). We conclude that during low-intensity resistance exercises it is possible to estimate aerobic energy cost by wearable heart rate monitors with errors below 10% in healthy young trained males.
