ABSTRACT: A 61-year-old woman who underwent hemodialysis presented with heart failure. Echocardiography revealed multiple mobile masses in the left atrium. The masses were excised, and histopathologic examination revealed calcified amorphous tumors. Here, we present several echocardiography images, including 3-dimensional transesophageal echocardiography, demonstrating the revolving masses in the left atrium. Cardiac calcified amorphous tumor: A systematic review of the literature.

Project description:Calcified amorphous tumor (CAT) of the heart is a rare non-neoplastic intracavitary cardiac mass. Several case reports have been published but large series are lacking.To determine clinical features, current management and outcomes of this rare disease.A systematic review of all articles reporting cases of CAT in order to perform a pooled analysis of its clinical features, management and outcomes.An electronic search of all English articles using PUBMED was performed. Further studies were identified by cross-referencing from relevant papers.We restricted inclusion to articles reporting cases of CAT in the English language literature published up to July 2014.One author performed data extraction using predefined data fields.A total of 27 articles, reporting 42 cases of CAT were found and included in this review.In this review, the most frequent presenting symptoms were dyspnea and embolic events. Mitral valve and annulus were the most frequent location of CAT. Surgery was most of the time required to confirm diagnosis, and was relatively safe. Overall outcome after surgical resection was good.

A case of cardiac calcified amorphous tumor complicated with acute myocardial infarction.

Project description:An 80-year-old woman with a history of eosinophilic granulomatosis with polyangiitis, cardiac hypertrophy, and diabetes called for an ambulance after developing chest pain. She was diagnosed with acute myocardial infarction (AMI), and coronary angiography revealed occlusion of the right coronary artery. Coronary aspiration was performed, and coronary aspirate was white with calcified factor. After percutaneous coronary intervention, transthoracic echocardiography performed on day 25 revealed a hyperechoic mobile mass originating from the anterior mitral leaflet. As a mobile or rapidly increasing mass carries a high risk of embolism, we decided to perform surgical resection. Preoperative cerebral magnetic resonance imaging showed asymptomatic cerebral infarction, suggesting embolism by the cardiac mass. Resection of the cardiac mass was performed by cardiac surgeons. Microscopic pathology of cardiac mass revealed nodules of calcification and fibroblasts, leading to diagnosis of calcified amorphous tumor (CAT). Furthermore, the microscopic pathology of the coronary aspirate showed calcification, fibrin, and vascular endothelial cells. The pathological similarity of the cardiac mass and coronary aspirate indicated that the AMI has been caused by CAT. CAT causes systemic embolization; however, only 1 case of MI caused by CAT has been reported. We therefore experienced a rare case in which CAT caused AMI. <Learning objective: This is a very rare case of acute myocardial infarction (AMI) caused by calcified amorphous tumor (CAT). When encountering AMI by embolism, the possibility of the involvement of CAT should be kept in mind. Careful observation of transthoracic echocardiography or transesophageal echocardiography, and a prompt decision concerning the indication for surgical treatment after assessing the risk of embolism are important.>.

Swinging cardiac calcified amorphous tumour arising from a calcified mitral annulus in a patient with normal renal function.

Project description:A 67-year-old man was referred to our institution for the management of a left-sided intracardiac mass discovered following two cardioembolic events. Imaging studies revealed a highly mobile mass attached to the ventricular side of a calcified mitral annulus. The mass had not been present on echocardiography 1 year earlier. Surgical resection was performed. Microscopic examination confirmed the diagnosis of a calcified amorphous tumour. The postoperative course was unremarkable, and the patient remains asymptomatic and without recurrence of a mass or neurological event after a 12-month follow-up.

A rapidly growing cardiac calcified amorphous tumour diagnosed after coronary artery bypass graft surgery: a case report.

Project description:BackgroundA cardiac calcified amorphous tumour (CAT) is a non-neoplastic intracavitary cardiac mass. The most serious complication is systemic embolism. Cardiac CATs tend to be surgically resected immediately after detection; therefore, its progress of growth is rarely reported.Case summaryAn 83-year-old Japanese woman received on-pump beating coronary artery bypass graft surgery (CABG) for angina pectoris. Transthoracic echocardiography (TTE) performed preoperatively and 1 month postoperatively revealed the presence of mitral annular calcification, with no other abnormal findings. However, follow-up TTE performed 5 months after CABG revealed a mobile nodular mass (5.0 × 8.2 mm) in the left ventricular outflow tract. At 1 month after detection, the mass had enlarged to 5.0 × 13.0 mm. Transoesophageal echocardiography revealed that the pedunculated high-echoic mass was adhered to the posterior commissure of the mitral valve and was dynamically swinging towards the non-coronary cusp in the systolic phase. As the mass had grown rapidly in less than 6 months, it was surgically resected to prevent systemic embolism. The histological specimen consisted mainly of fibrin, including calcification and hemosiderin deposition, which lead to a diagnosis of cardiac CAT. The patient had an uneventful postoperative course during her hospital stay and had no evidence of recurrence for 1 year after discharge.DiscussionThis was a rare case in which a rapidly growing cardiac CAT was detected following on-pump CABG. Cardiac CATs may grow very rapidly and therefore early surgery should be considered after initial diagnosis.

Caseous calcification of the mitral annulus-associated calcified amorphous tumour with chronic inflammatory cardiomyopathy: a case report.

Project description:BackgroundCalcified amorphous tumour (CAT) is a rare lesion that forms at various sites of the heart, including the mitral annulus. Since it is a systemic embolic source, it often requires resection. In contrast, caseous calcification of the mitral annulus (CCMA) usually receives conservative follow-up; however, it is sometimes resected to prevent embolism. Herein, we present a rare case of CCMA-associated CAT with chronic inflammatory cardiomyopathy and comprehensively consider the aetiology of CCMA-associated CAT.Case summaryA 52-year-old man with end-stage renal failure requiring haemodialysis underwent annual follow-up echocardiography for aortic valvular dysfunction. Echocardiography revealed a well-defined 22 × 17-mm circular mass on the annulus of the posterior mitral leaflet that had not been detected 2 years previously. Based on imaging characteristics, the mass was suspected to be a CCMA. It had rapidly enlarged and posed a potential risk for embolism; therefore, it was resected concurrently with aortic valve replacement. Although CCMA was suspected based on the intraoperative findings, the histopathological diagnosis was a CAT. Upon histological analysis of the left ventricular myocardial specimen and excised aortic valve, chronic inflammatory cell infiltration was observed; therefore, chronic inflammatory cardiomyopathy was diagnosed.DiscussionIn this case, the timing of decreased left ventricular systolic function and the arising mass coincided, suggesting that chronic inflammation and immunopathological reactions may have influenced the development of CAT from CCMA. Therefore, the rapid progression of CCMA in the development of CAT should especially be considered in patients with inflammatory cardiac comorbidities.
