Real-time alerting system for COVID-19 and other stress events using wearable data.

ABSTRACT: Early detection of infectious diseases is crucial for reducing transmission and facilitating early intervention. In this study, we built a real-time smartwatch-based alerting system that detects aberrant physiological and activity signals (heart rates and steps) associated with the onset of early infection and implemented this system in a prospective study. In a cohort of 3,318 participants, of whom 84 were infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), this system generated alerts for pre-symptomatic and asymptomatic SARS-CoV-2 infection in 67 (80%) of the infected individuals. Pre-symptomatic signals were observed at a median of 3 days before symptom onset. Examination of detailed survey responses provided by the participants revealed that other respiratory infections as well as events not associated with infection, such as stress, alcohol consumption and travel, could also trigger alerts, albeit at a much lower mean frequency (1.15 alert days per person compared to 3.42 alert days per person for coronavirus disease 2019 cases). Thus, analysis of smartwatch signals by an online detection algorithm provides advance warning of SARS-CoV-2 infection in a high percentage of cases. This study shows that a real-time alerting system can be used for early detection of infection and other stressors and employed on an open-source platform that is scalable to millions of users.

Project description:BackgroundContinuous vital sign monitoring may identify changes sooner than current standard monitoring.ObjectiveTo investigate if the use of real-time digital alerts sent to healthcare staff can improve the time taken to identify unwell patients and those with sepsis.DesignA prospective cohort study design.SettingWest Middlesex University Hospital, UK.ParticipantsFifty acutely unwell surgical patients admitted to hospital.InterventionPatients wore a lightweight wearable sensor measuring heart rate (HR), respiratory rate (RR) and temperature every 2 min whilst standard intermittent ward monitoring of vital signs was performed by nurses. Digital alerts were sent to healthcare staff from the sensor to a smartphone device. All alerts were reviewed for recruited patients to identify the exact time on the sensor in which deterioration occurred. The time to acknowledgement was then reviewed for each action and an average time to acknowledgement calculated.ResultsThere were 50 patients recruited in the pilot study, of which there were vital sign alerts in 18 patients (36%). The total number of vital sign alerts generated in these 18 patients was 51. Of these 51 alerts, there were 7 alerts for high HR (13.7%), 33 for RR (64.7%) and 11 for temperature (21.6%). Out of the 27 acknowledged alerts, there were 2 alerts for HR, 17 for RR and 8 for temperature. The average time to staff acknowledgement of the notification for all alerts was 154 min (2.6 h). There were some patients which had shown signs of deterioration in the cohort. The frequency of routine observation monitoring was increased in 2 cases, 3 patients were referred to a senior clinician and 2 patients were initiated on the sepsis pathway.ConclusionThis study demonstrates the evaluation of digital alerts to nurses in real time. Although not all alerts were acknowledged, deterioration on the ward observations was detected and actions were taken accordingly. Patients were started on the sepsis pathway and escalation to senior clinicians occurred. Further research is required to review why only some alerts were acknowledged and the effects of digital alerting on patient outcomes.Trial registrationClinicalTrials.gov, NCT04638738.

Project description:Health care is one of the least funded sectors in Bangladesh and many other similar developing countries. People living in rural and remote areas do not have access to proper health care, and when they do, it is too expensive. This research aimed to develop a real-time health monitoring system that is cheap, easy to use, and accessible by doctors and patients. The system consists of several Internet of Things (IoT)-based sensors connected to an Arduino microprocessor, which thus measures the vital body signs of the patients. The measured readings are then transmitted to an Android application on a smartphone via a Bluetooth module. The sensors are connected to analog inputs. These sensors measure analog data, which is amplified by the microprocessor after being sorted. Doctors can also carry out the diagnosis of ailments using the data collected remotely from the patient. An Android-based mobile application that interfaces with a web-based application is implemented for efficient patients-doctors dual real-time communication. The Android application, which is connected to a MySQL database, updates the said database, which updates and displays the readings on a website accessible by both doctors and patients. Initially, the health monitor was tested using an Arduino Integrated Development Environment (IDE) monitor and a single user. Once initial simulations were successful, the proposed system was tested on five different real-human test subjects. The testing of the wireless health monitor produced successful results that measured patient vitals with a high level of accuracy. The proposed IoT-based system monitors vital signs such as the patient's body temperature, heart rate, ECG, SpO2 levels, blood pressure, and glucose levels. This system also includes a medical treatment plan by the doctors. The proposed system is novel as it integrates the IoT-based patient monitoring system with telemedicine. This proposed system has different sensors for real-time measuring the vital signs of the human body. A mobile and web application have also been integrated with this system for real-time remote patient monitoring and treatment plan. There are now systems available that only offer a telemedicine facility, where patients and doctors can have discussions, but do not have an IoT-based patient vital sign monitoring system integrated with telemedicine. The proposed system in this paper has the facility of IoT-based patient vital signs monitoring integrated with telemedicine, which makes this research work novel. The proposed system will increase the life expectancy of people throughout the world.
