A Learning Theory Approach to Attachment Theory: Exploring Clinical Applications.

ABSTRACT: Although clinicians typically acknowledge the importance of insecure attachment as one factor that can contribute to children's psychopathology, translating attachment theory into clinical practice has proved a challenge. By specifying some of the mechanisms through which the child's attachment develops and changes, learning theory can enhance attachment based approaches to therapy. Specifically, interventions building on operant (parent management training) and classical (exposure therapy) learning can be used to stimulate new learning that increases the child's security and confidence in the parent's availability and responsiveness. To explore the clinical application and utility of a Learning Theory of Attachment (LTA), we focus on two attachment-focused interventions: Video-feedback Intervention to promote Positive Parenting and Sensitive Discipline (VIPP-SD) and Middle Childhood Attachment-based Family Therapy (MCAT). VIPP-SD is an evidence-based parent management training designed to promote sensitive parenting and secure attachment in early childhood. MCAT is a recently developed intervention that uses exposure to stimulate secure attachment in middle childhood. LTA sheds light on the mechanisms set in train by VIPP-SD and MCAT facilitating the induction of professionals in clinical applications.

Project description:BackgroundIn recent years, point-of-care ultrasound (POCUS) has become an essential field of medical education. Bedside ultrasound has become a necessary skill for clinical physicians. Previous studies have already discussed the importance of advancements in ultrasound education. However, learning motivations for ultrasound education have seldom been analyzed in the literature. For medical students, learning ultrasound could have a relevance for their future career. The Existence, Relatedness and Growth (ERG) theory extended Maslow's hierarchy of needs through these three concepts. This theory has been widely used in the workplace to analyze employee job performance but has not yet been applied in medical education. In this study ERG theory was applied to analyze pre-clinical medical students' learning motivation toward ultrasound education.MethodThis mixed method study used online questionnaires consisting of open-ended questions as a data collection tool, and based on these results, both qualitative and quantitative analysis were conducted. Participants answered a series of neutral and open-ended questions regarding their motivations to learn ultrasonography. After data collection, a three-step analysis was conducted based on the grounded theory approach. Finally, the results of the thematic coding were used to complete additional quantitative analysis.ResultsThe study involved 140 pre-clinical medical students, and their responses fell into 13 specific categories. The analysis demonstrated that students' motivations toward ultrasound education were unbalanced across the three ERG domains (F = 41.257, p < .001). Pairwise comparisons showed that students mentioned existence motivation (MD = 39.3%; p < .001) and growth motivation (MD = 40.7%; p < .001) more frequently than relatedness motivation. However, there was no significant difference between existence motivation and growth motivation (MD = - 1.4%; p = .830).ConclusionThe results revealed that students placed a high value on existence and growth needs rather than relatedness based on the survey. In addition, the findings suggest that ERG theory can be a useful tool to conduct medical education motivation analysis.

Project description:ObjectivesThe primary objectives of this study were to examine the regulatory processes of medical students as they completed a diagnostic reasoning task and to examine whether the strategic quality of these regulatory processes were related to short-term and longer-term medical education outcomes.MethodsA self-regulated learning (SRL) microanalytic assessment was administered to 71 second-year medical students while they read a clinical case and worked to formulate the most probable diagnosis. Verbal responses to open-ended questions targeting forethought and performance phase processes of a cyclical model of SRL were recorded verbatim and subsequently coded using a framework from prior research. Descriptive statistics and hierarchical linear regression models were used to examine the relationships between the SRL processes and several outcomes.ResultsMost participants (90%) reported focusing on specific diagnostic reasoning strategies during the task (metacognitive monitoring), but only about one-third of students referenced these strategies (e.g. identifying symptoms, integration) in relation to their task goals and plans for completing the task. After accounting for prior undergraduate achievement and verbal reasoning ability, strategic planning explained significant additional variance in course grade (ΔR(2 ) = 0.15, p < 0.01), second-year grade point average (ΔR(2) = 0.14, p < 0.01), United States Medical Licensing Examination Step 1 score (ΔR(2) = 0.08, p < 0.05) and National Board of Medical Examiner subject examination score in internal medicine (ΔR(2) = 0.10, p < 0.05).ConclusionsThese findings suggest that most students in the formative stages of learning diagnostic reasoning skills are aware of and think about at least one key diagnostic reasoning process or strategy while solving a clinical case, but a substantially smaller percentage set goals or develop plans that incorporate such strategies. Given that students who developed more strategic plans achieved better outcomes, the potential importance of forethought regulatory processes is underscored.

Project description:BackgroundThe integration of machine learning (ML) in predicting asthma-related outcomes in children presents a novel approach in pediatric health care.ObjectiveThis scoping review aims to analyze studies published since 2019, focusing on ML algorithms, their applications, and predictive performances.MethodsWe searched Ovid MEDLINE ALL and Embase on Ovid, the Cochrane Library (Wiley), CINAHL (EBSCO), and Web of Science (core collection). The search covered the period from January 1, 2019, to July 18, 2023. Studies applying ML models in predicting asthma-related outcomes in children aged <18 years were included. Covidence was used for citation management, and the risk of bias was assessed using the Prediction Model Risk of Bias Assessment Tool.ResultsFrom 1231 initial articles, 15 met our inclusion criteria. The sample size ranged from 74 to 87,413 patients. Most studies used multiple ML techniques, with logistic regression (n=7, 47%) and random forests (n=6, 40%) being the most common. Key outcomes included predicting asthma exacerbations, classifying asthma phenotypes, predicting asthma diagnoses, and identifying potential risk factors. For predicting exacerbations, recurrent neural networks and XGBoost showed high performance, with XGBoost achieving an area under the receiver operating characteristic curve (AUROC) of 0.76. In classifying asthma phenotypes, support vector machines were highly effective, achieving an AUROC of 0.79. For diagnosis prediction, artificial neural networks outperformed logistic regression, with an AUROC of 0.63. To identify risk factors focused on symptom severity and lung function, random forests achieved an AUROC of 0.88. Sound-based studies distinguished wheezing from nonwheezing and asthmatic from normal coughs. The risk of bias assessment revealed that most studies (n=8, 53%) exhibited low to moderate risk, ensuring a reasonable level of confidence in the findings. Common limitations across studies included data quality issues, sample size constraints, and interpretability concerns.ConclusionsThis review highlights the diverse application of ML in predicting pediatric asthma outcomes, with each model offering unique strengths and challenges. Future research should address data quality, increase sample sizes, and enhance model interpretability to optimize ML utility in clinical settings for pediatric asthma management.

Project description:IntroductionPeer assessment enables students to evaluate peers, deepening their understanding of course objectives and boosting engagement. However, its dependability, especially in summative contexts, is often questioned. This study examines the dependability of peer assessments, and the optimal number of items and raters needed for a reliable assessment in project-based learning (PtBL).MethodsIn a PtBL class, 95 third-year pre-clinical students, grouped into ten teams, created 5-min videos on cardiovascular lifestyle modifications for the community, followed by a 5-min presentation on the video's information and relevance to the citizen. Over three weeks, each group, guided by three advisors, refined their video. During presentations, peers (from nine non-presenting groups) and ten teachers (five Doctor of Medicines (MDs) and five health professors) evaluated the video presentation using a 6-item rubric covering three domains: interdisciplinary data integration, interpersonal skills, and video quality/effectiveness. Messick's validity framework was utilized to guide the collection of validity evidence.ResultsFive sources of validity evidence were collected: 1) Content: Three professors confirmed the rubric's content validity. 2) Response process: Scores from students, MDs, and health professors are similar at 54.00 ± 4.03, 53.24 ± 4.18, and 54.16 ± 4.16, respectively (F = 0.75, p = 0.472). 3) Internal structure: A fully crossed design (p × i × r) generalizability theory analysis showed that achieving a Phi-coefficient ≥ 0.70 on a six-item rubric requires 27 students (Phi-coefficient = 0.70), 7 MDs (Phi-coefficient = 0.70), or 5 health professors (Phi-coefficient = 0.73). A nested design (r:(p × i)) demonstrated superior reliability, requiring only 9 students, 5 MDs, and 4 health professors for acceptable reliability. The confirmatory factor analysis indicated a good model fit. 4) Relations to other variables: On average, peer and teacher ratings scored 54.00 ± 2.22 and 53.70 ± 2.78, respectively, with an inter-rater reliability of r = 0.73 (p = 0.016). 5) Consequences: Most groups found peer assessment beneficial for gaining insightful feedback (8/10), enhancing engagement (7/10), refining their work (5/10), and learning structured feedback (3/10), though there were concerns about potential bias (5/10).ConclusionDependability evidence for peer assessment in the PtBL context was successfully gathered. In PtBL, students contribute to grading due to their diverse expertise. While peer assessment cannot replace teacher evaluations, it enhances engagement, enriches the learning environment, and improves assessment quality through valuable feedback.
