Acceptability of Traditional Chinese Medicine in Chinese People Based on 10-Year's Real World Study With Mutiple Big Data Mining.

ABSTRACT: In the past decades, numerous clinical researches have been conducted to illuminate the effects of traditional Chinese medicine for better inheritance and promotion of it, which are mostly clinical trials designed from the doctor's point of view. This large-scale data mining study was conducted from real-world point of view in up to 10 years' big data sets of Traditional Chinese Medicine (TCM) in China, including both medical visits to hospital and cyberspace and contemporaneous social survey data. Finally, some important and interesting findings appear: (1) More Criticisms vs. More Visits. The intensity of criticism increased by 2.33 times over the past 10 years, while the actual number of visits increased by 2.41 times. (2) The people of younger age, highly educated and from economically developed areas have become the primary population for utilizing TCM, which is contrary to common opinions on the characteristics of TCM users. The discovery of this phenomenon indicates that TCM deserves further study on how it treats illness and maintains health.

Project description:ObjectivesThis study aims to assess the impact of Traditional Chinese Medicine (TCM) on dementia patients, utilizing real-world data. Specifically, it seeks to evaluate how TCM influences clinical outcomes by examining changes in the Clinical Dementia Rating (CDR) and Mini-Mental State Examination (MMSE) scores, as well as its effect on medical expenses over a two-year period. Data from a multi-center research database spanning from 2004 to 2021 will be used to achieve these objectives, addressing the current gap in empirical data concerning intuitive outcomes and cognitive function assessments.MethodsPropensity score matching was adopted to improve comparability among the intervention and control groups. Due to repeated dependent variable measurements, the generalized estimating equation was used to control for socio-demographic characteristics, regional characteristics, and Western medicine treatments for dementia.ResultsAfter propensity score matching, a total of 441 research subjects were included: 90 in the TCM intervention group and 351 in the non-TCM intervention group. The results of multivariate regression analysis showed that compared with the non-TCM intervention group, the MMSE scores in the TCM intervention group increased by 0.608 points each year. The annual change in CDR scores in the TCM intervention group was 0.702 times that of the non-TCM utilization group. After TCM intervention, annual outpatient expenses increased by US$492.2, hospitalization expenses increased by US$324.3, and total medical expenses increased by US$815.9, compared with the non-intervention group.ConclusionsTCM interventions significantly decelerate cognitive decline in dementia patients, evidenced by slower reductions in MMSE scores and mitigated increases in CDR scores. However, these benefits are accompanied by increased medical expenses, particularly for outpatient care. Future healthcare strategies should balance the cognitive benefits of TCM with its economic impact, advocating for its inclusion in dementia care protocols.

Project description:BackgroundGiven the prevalent utilization of integrated traditional Chinese and western medicine (ITCWM) in the management of acute pancreatitis, the majority of studies have concentrated on severe cases, lacking robust evidence-based medical research. Real-world investigations can provide an objective assessment of the clinical effectiveness of combining traditional Chinese medicine with western medicine. Consequently, relying on real-world research, we intend to evaluate the clinical efficacy and safety of the combined approach in treating mild to moderate acute pancreatitis.MethodsA total of 563 AP patients from Henan Provincial Hospital of Traditional Chinese Medicine were collected from January 2019 to October 2023. A propensity score matching (PSM) analysis was conducted to evaluate the clinical efficacy of traditional Chinese medicine (TCM) in treating mild to moderate acute pancreatitis. Patients were divided into a control group (61 cases) and an integrated traditional Chinese and Western medicine (ITCWM) group (120 cases). To further assess the clinical efficacy of TCM enema in the treatment of mild to moderate acute pancreatitis, PSM analysis was conducted across three groups. The patients were categorized into a control group (n = 49), an oral TCM treatment group (OCM group, n = 274), and an oral TCM plus enema treatment group (OCM+E group, n = 131). Logistic regression was used to analyze factors after treatment in each group, and the Kaplan-Meier method compared symptom duration in each group.ResultsCompared with the control group, the ITCWM group significantly decreased C-reactive protein (CRP, mg/L) (17.8 [1.2-59.5] vs. 8.0 [3.3-33.5], P = 0.022), shortened the duration of abdominal distension, abdominal pain, nausea and bitter taste symptoms (P < 0.05), and shortened the length of hospital stay (median 19.0 and 11.5 days, respectively, P = 0.001); Compared with the other two groups, the neutrophil percentage (NEUT%) was lower (74.1 vs. 61.9 vs. 59.5, P < 0.05) and serum prealbumin (PA, mg/L) was higher (116.0 vs. 184.4 vs. 220.0, P < 0.05), the length of hospitalization (days) was shortened (19.0 vs.12.0 vs.10.0, P < 0.05) in the OCM+E group.ConclusionThe combination of traditional Chinese medicine and modern medicine has been shown to effectively decrease inflammatory indicators in patients with mild to moderate acute pancreatitis, leading to a reduction in symptom duration and hospitalization period, as well as promoting disease recovery. Notably, the use of traditional Chinese medicine in conjunction with enema therapy yields more pronounced benefits.

Project description:ObjectiveDepression is a mental disorder characterized by persistent feelings of sadness, decreased interest or pleasure in activities and reduced energy. As a highly prevalent disorder, it seriously endangers the psychosocial functioning of patients. Many scholars have conducted clinical studies on the treatment of depression using different herbal remedies, but there are no studies that integrate these remedies to explore the general medication rule. This study aims to explore the medication pattern of Traditional Chinese Medicine (TCM) treatment for depression through data mining methods, so as to provide scientific theoretical basis and reference for clinical treatment and new prescription development.MethodsBased on the PRISMA principle, 121 articles involving 10810 patients with depression of TCM treatment were collected. We then performed frequency, association rule, and hierarchical clustering analysis of Chinese herbs using Microsoft Excel 2016, SPSS Modeler 18.0 and IBM SPSS Statistics 23.ResultsAmong the 270 herbs collected, the three most frequently occurring herbs are Gancao, Chaihu, and Shaoyao. The categories of high-frequency herbs are mainly deficiency-tonifying, Qi-regulating and blood-activating and stasis-eliminating herbs. Through the Apriori algorithm, we mined 21 herbal groups of association rules, and among which the combination of Chaihu-Shaoyao-Gancao has the highest level of support. Furthermore, five novel clustering combinations were identified, predominantly derived from Xiaoyao-San, Chaihu-Shugan-San, Sini powder, Kaixin-San and Chaihu-Jia-Longgu-Muli Decoction.ConclusionThe current study not only concluded the frequent combinations but also developed five new drug cluster combinations for depression, which can provide evidence-based references for the future clinical treatment and is helpful to understand the potential pharmaceutical mechanism from the properties, tastes, meridian tropisms and categories. The clinical effectiveness of these combinations needs to be verified by future study.
