Antidepressant Monotherapy and Combination Therapy with Acupuncture in Depressed Patients: A Resting-State Functional Near-Infrared Spectroscopy (fNIRS) Study.

ABSTRACT: Depression is a common psychiatric illness affecting over 300 million people globally. Acupuncture has been reported to be a safe complementary treatment for depression. This study is aimed to investigate the efficacy and mechanism of combining acupuncture with antidepressants in treating depression compared to the sole use of antidepressants. Seventy depression patients were randomly assigned to the treatment group (n = 50) and control group (n = 20). The treatment group received acupuncture combined antidepressants treatment for 3 weeks, while the control group took antidepressants monotherapy for 3 weeks. Among the 70 patients, 40 participants (20 control; 20 treatment) were randomized for studying functional connectivity (FC) of the dorsolateral prefrontal cortex (DLPFC) measured by the functional near-infrared spectroscopy. The primary outcome was HAMD-17 and secondary outcomes were PHQ-9, and the relationships of resting-state FC (rsFC) with the depression severity. PHQ-9 and HAMD-17 scores in the treatment group were significantly lower than those in the control group at Week 3 (p = 0.01) with effect sizes of -0.4 and -0.61 respectively. The rsFC in F1, F3, AF3, AF7, FC3, FC5 (left DLPFC, 10-20 system), AF8, and F6 (right DLPFC) in the treatment group had significant temporal correlation (p < 0.05, FDR corrected) in DLPFC compared to the channels in the control group. No significant correlation was found between the changes of rsFC and depression severity. In conclusion, depressed patients receiving acupuncture combined with antidepressants have improvement of depressive symptoms and the stronger rsFC in the DLPFC compared to those using antidepressants alone.

Project description:Functional neuroimaging of speech processing has both research and clinical potential. This work is facilitating an ever-increasing understanding of the complex neural mechanisms involved in the processing of speech. Neural correlates of speech understanding also have potential clinical value, especially for infants and children, in whom behavioural assessments can be unreliable. Such measures would not only benefit normally hearing children experiencing speech and language delay, but also hearing impaired children with and without hearing devices. In the current study, we examined cortical correlates of speech intelligibility in normally hearing paediatric listeners. Cortical responses were measured using functional near-infrared spectroscopy (fNIRS), a non-invasive neuroimaging technique that is fully compatible with hearing devices, including cochlear implants. In nineteen normally hearing children (aged 6 - 13 years) we measured activity in temporal and frontal cortex bilaterally whilst participants listened to both clear- and noise-vocoded sentences targeting four levels of speech intelligibility. Cortical activation in superior temporal and inferior frontal cortex was generally stronger in the left hemisphere than in the right. Activation in left superior temporal cortex grew monotonically with increasing speech intelligibility. In the same region, we identified a trend towards greater activation on correctly vs. incorrectly perceived trials, suggesting a possible sensitivity to speech intelligibility per se, beyond sensitivity to changing acoustic properties across stimulation conditions. Outside superior temporal cortex, we identified other regions in which fNIRS responses varied with speech intelligibility. For example, channels overlying posterior middle temporal regions in the right hemisphere exhibited relative deactivation during sentence processing (compared to a silent baseline condition), with the amplitude of that deactivation being greater in more difficult listening conditions. This finding may represent sensitivity to components of the default mode network in lateral temporal regions, and hence effortful listening in normally hearing paediatric listeners. Our results indicate that fNIRS has the potential to provide an objective marker of speech intelligibility in normally hearing children. Should these results be found to apply to individuals experiencing language delay or to those listening through a hearing device, such as a cochlear implant, fNIRS may form the basis of a clinically useful measure of speech understanding.

Project description:SignificanceAdvances in electronics have allowed the recent development of compact, high channel count time domain functional near-infrared spectroscopy (TD-fNIRS) systems. Temporal moment analysis has been proposed for increased brain sensitivity due to the depth selectivity of higher order temporal moments. We propose a general linear model (GLM) incorporating TD moment data and auxiliary physiological measurements, such as short separation channels, to improve the recovery of the HRF.AimsWe compare the performance of previously reported multi-distance TD moment techniques to commonly used techniques for continuous wave (CW) fNIRS hemodynamic response function (HRF) recovery, namely block averaging and CW GLM. Additionally, we compare the multi-distance TD moment technique to TD moment GLM.ApproachWe augmented resting TD-fNIRS moment data (six subjects) with known synthetic HRFs. We then employed block averaging and GLM techniques with "short-separation regression" designed both for CW and TD to recover the HRFs. We calculated the root mean square error (RMSE) and the correlation of the recovered HRF to the ground truth. We compared the performance of equivalent CW and TD techniques with paired t-tests.ResultsWe found that, on average, TD moment HRF recovery improves correlations by 98% and 48% for HbO and HbR respectively, over CW GLM. The improvement on the correlation for TD GLM over TD moment is 12% (HbO) and 27% (HbR). RMSE decreases 56% and 52% (HbO and HbR) for TD moment compared to CW GLM. We found no statistically significant improvement in the RMSE for TD GLM compared to TD moment.ConclusionsProperly covariance-scaled TD moment techniques outperform their CW equivalents in both RMSE and correlation in the recovery of the synthetic HRFs. Furthermore, our proposed TD GLM based on moments outperforms regular TD moment analysis, while allowing the incorporation of auxiliary measurements of the confounding physiological signals from the scalp.
