Impact of the COVID-19 Pandemic on Glycemic Control and Blood Pressure Control in Patients with Diabetes in Japan.

ABSTRACT: Objective In this study, we investigated whether and how the COVID-19 pandemic affected glycemic control and blood pressure (BP) control in patients with diabetes mellitus (DM). Methods DM patients whose HbA1c level was measured regularly before and after the declaration of a state of emergency were included in this study. Some patients were given questionnaires about changes in their lifestyle to determine the factors affecting glycemic control and BP control. Results The median HbA1c level of the 804 patients increased significantly from 6.8% before the state of emergency to 7.1% and 7.0% during and after the state of emergency, respectively. This was in contrast to the decrease one year earlier due to seasonal variations. In the 176 patients who responded to the questionnaire, the HbA1c level also increased significantly during and after the state of emergency. The worsening of glycemic control was more pronounced in the group that had achieved HbA1c of <7% before the state of emergency than in those with higher values. Unlike the rise in HbA1c, the BP did not rise during the state of emergency but did rise significantly afterwards. There was no marked decrease in HbA1c or BP after the state of emergency, even in patients who responded that they were much more careful with their diet, ate less, or exercised more. Conclusions The COVID-19 pandemic worsened glycemic control and BP control, even in patients who perceived no marked change in their diet or exercise, suggesting that more active lifestyle guidance is necessary for good treatment of DM patients.

Project description:ImportanceCOVID-19 altered lifestyles and disrupted routine health care. Whether blood pressure (BP) control worsened during COVID-19 is unknown.ObjectiveTo understand whether home BP control worsened during COVID-19 across the United States (US) .Design, setting, and participantsA population-based analysis of home BP data from 72,706 participants enrolled in a digital health hypertension control program. Data was compared before (January 2019 to March 2020) and during (April 2020 to August 2020) COVID-19.Main outcomes and measuresMonthly mean home BP readings, systolic blood pressure (SBP), diastolic blood pressure (DBP), and mean arterial pressure (MAP) were quantified before and during the pandemic. Multivariable adjustments were made for age, sex, race, region, and months enrolled. Home BP readings were also classified based on monthly averages and highest home BP readings into risk groups: Stage 2 HTN: BP> = 135 or DBP> = 85; Uncontrolled HTN: SBP> = 145 or DBP> = 95; or Severely uncontrolled HTN: SBP> = 160 or DBP> = 100).ResultsOverall, 72,706 participants were enrolled in a digital health hypertension program between 1/1/2019 and 8/31/2020. Compared with participants pre-COVID-19 (n = 33,440), those during COVID-19 (n = 39,266) were of similar age (mean 53.0 ± 10.7 years vs 53.3 ± 10.8 years); sex (46% vs 50.6% female) and race (29.1% vs 34.2% non-white). Relative to pre-Covid (Apr-Aug 2019) the mean monthly number of home BP readings rose during COVID-19 (Apr-Aug, 2020), from 7.3 to 9.3 per month (P < .001). During COVID-19, participants had higher monthly adjusted mean SBP (131.6 mmHg vs. 127.5 mmHg, P < .001); DBP (80.2 mmHg vs. 79.2 mmHg, P < .001); and MAP (97.4 mmHg vs. 95.3 mmHg; P < .001). Relative to the pre-pandemic period, during COVID-19 the proportion of participants with a mean monthly BP classified as uncontrolled or severely uncontrolled hypertension also rose, 15% vs 19% and 4% vs 5%, respectively CONCLUSIONS AND RELEVANCE: Based on home BP readings, mean monthly BP rose modestly after COVID-19, despite increased utilization of home monitoring. Further studies are needed to examine the longitudinal effects of the pandemic on cardiovascular disease risk factors, the impact of these on long-term population health.

Project description:Background/ introductionRegional shelter-in-place (SiP) mandates had a substantial impact on physical health and access to care. The impact of SiP on chronic disease management, specifically diabetes mellitus, is unknown. We sought to quantify the impact of California's 2020 SiP order on various health parameters in the Kaiser Permanente Northern California (KPNC) diabetic population.MethodsThis retrospective cohort study included 168,621 diabetic patients, stratified by pre-pandemic HbA1c level. Our primary outcome was the difference in HbA1c, systolic blood pressure (SBP), and diastolic blood pressure (DBP) comparing the pre- (September 1, 2019, to March 31, 2020) to post-SiP period (June 1, 2020, to December 31, 2020). Our secondary endpoints included use of insulin or oral hypoglycemic agents (OHAs) during the post-SiP period and change in body mass index (BMI). This study utilized a paired t-test and chi-squared testing in order to assess for statistically significant differences in pre- versus post-SiP values.ResultsPatients in this cohort were 52.29% male with 37.68% White, 22.35% Hispanic, 9.30% Black and 23.54% Asian and a mean age of 63 years. In this cohort, 44.02% of patients had a HbA1c < 7%, while 29.17%, 13.67%, 6.34% and 6.81% had an HbA1c of 7-7.9%, 8-8.9%, 9-9.9% and ≥ 10%, respectively (range 4.10 to 19.50%). Mean HbA1c, SBP, and DBP increased significantly across all groups; OHA use and insulin utilization also increased overall. Patients with lower pre-SiP HbA1c demonstrated larger increases in HbA1c and OHA utilization, while patients with higher pre-SiP HbA1c demonstrated increased rates of insulin initiation and decreases in their post-SiP HbA1c. Notably, mean BMI decreased in every HbA1c subgroup. Due to the large sample size, all p-values were < 0.0001.ConclusionsAmong the KPNC diabetic population, several metrics for diabetes health management were significantly worsened after California's SiP. However, diabetic patients with the highest HbA1c values showed clinically significant improvement in their HbA1c, indicating a differential effect of the SiP on diabetes management. Our study helps identify higher-risk diabetic patients who require more intensive monitoring in the setting of the recent pandemic and highlights the importance of considering long-term implications of policy decisions on diabetes care.
