COVID-19 Exposure, Stress, and Mental Health Outcomes: Results From a Needs Assessment Among Low Income Adults in Central North Carolina.

ABSTRACT: This study focuses on identifying COVID-19 related exposure, stress, and mental health concerns in the larger Charlotte, North Carolina region, an area with many low-income and under resourced communities. A community-academic partnership conducted a regional COVID-19 needs assessment. Low-income adults (N = 156) completed an online-administered survey of demographic information, COVID-19 exposure, stress, coping-related factors, and mental health. Frequency data showed that common COVID-19 related stressors included job exposure, lost job/income, and increased home responsibilities. Frequency data further showed elevated screening risk rates for mental health concerns were observed for post-traumatic stress (83.3%), depression (52.2%), problematic drinking (50.0%), generalized anxiety (43.0%), and suicide (40.4%). Bivariate correlation and multivariate regression models identified robust mental health risk factors including COVID-19 related stress affecting close persons, fear/worry reaction to the pandemic, and use of venting as a coping strategy; protective factors included active coping and problem-focused coping beliefs. Findings are discussed with respect to informing regional public health efforts during the pandemic.

Project description:BackgroundThe increasing incidence of extreme heat due to climate change poses a significant threat to maternal mental health in the U.S. We examine the association of acute exposure to heatwaves with maternal mental health conditions in North Carolina from 2011 to 2019.MethodsWe incorporate a matched analysis design using NC Hospital Discharge Data to examine emergency department admissions for psychiatric conditions during the warm season (May to September), matching heatwave periods with non-heatwave unexposed periods at the zip code tabulation area (ZCTA) level. We stratify the sample to examine effect modification across the rural-urban continuum, physiographic regions, measurements of neighborhood racial and economic inequality, and individual-level sociodemographic factors (e.g., age, race/ethnicity, and insurance type).FindingsOur sample of 324,928 emergency department visits by pregnant individuals has a mean age of 25.8 years (SD: 5.84), with 9.3% (n = 30,205) identifying as Hispanic. Relative risk (RR) estimates and 95% confidence intervals (CI) indicate significant increases in maternal mental health burdens following heatwave exposure. Acute heatwave periods were associated with a 13% higher risk of severe mental illness (RRSMI: 1.13, CI: 1.08-1.19, p: <0.0001), while prolonged exposure to moderate-intensity heatwaves was associated with 37% higher risk (RRSMI: 1.37, CI: 1.19-1.58, p: <0.001). Individual factors (e.g., advanced maternal age and insurance providers) and neighborhood-level characteristics, like low socioeconomic status, racialized and economic segregation, rurality, and physiographic region, further modified the risk of adverse maternal mental health outcomes.InterpretationOur results add to the growing evidence of the impact of extreme heat on maternal mental health, particularly among vulnerable subpopulations. Project description:BackgroundMany studies have explored the heat-mortality relationship; however, comparability of results is hindered by the studies' use of different exposure methods.ObjectiveThis study evaluated different methods for estimating exposure to temperature using individual-level data and examined the impacts on the heat-mortality relationship.MethodsWe calculated different temperature exposures for each individual death by using a modeled, gridded temperature dataset and a monitoring station dataset in North Carolina for 2000-2016. We considered individual-level vs. county-level averages and measured vs. modeled temperature data. A case-crossover analysis was conducted to examine the heat-mortality risk under different exposure methods.ResultsThe minimum mortality temperature (MMT) (i.e., the temperature with the lowest mortality rate) for the monitoring station dataset was 23.87 °C and 22.67 °C (individual monitor and county average, respectively), whereas for the modeled temperature dataset the MMT was 19.46 °C and 19.61 °C (individual and county, respectively). We found higher heat-mortality risk while using temperature exposure estimated from monitoring stations compared to risk based on exposure using the modeled temperature dataset. Individual-aggregated monitoring station temperature exposure resulted in higher heat mortality risk (odds ratio (95% CI): 2.24 (95% CI: 2.21, 2.27)) for a relative temperature change comparing the 99th and 90th temperature percentiles, while modeled temperature exposure resulted in lower odds ratio of 1.27 (95% CI: 1.25, 1.29).SignificanceOur findings indicate that using different temperature exposure methods can result in different temperature-mortality risk. The impact of using various exposure methods should be considered in planning health policies related to high temperatures, including under climate change. IMPACT STATEMENT: (1) We estimated the heat-mortality association using different methods to estimate exposure to temperature. (2) The mean temperature value among different exposure methods were similar although lower for the modeled data, however, use of the monitoring station temperature dataset resulted in higher heat-mortality risk than the modeled temperature dataset. (3) Differences in mortality risk from heat by urbanicity varies depending on the method used to estimate temperature exposure.

Project description:BackgroundPreterm birth (PTB) and term low birth weight (LBW) have been associated with pollution and other environmental exposures, but the relationship between these adverse outcomes and specific characteristics of polluted sites is not well studied.ObjectivesWe conducted a retrospective cohort study to examine relationships between residential proximity to polluted sites in North Carolina (NC) and PTB and LBW. We further stratified exposure to polluted sites by route of contaminant emissions and specific contaminants released at each site.MethodsWe created an integrated exposure geodatabase of polluted sites in NC from 2002 to 2015 including all landfills, Superfund sites, and industrial sites. Using birth certificates, we assembled a cohort of 1,494,651 singleton births in NC from 2003 to 2015. We geocoded the gestational parent residential address on the birth certificate, and defined exposure to polluted sites as residence within one mile of a site. We used log-binomial regression models to estimate adjusted risk ratios (aRR) and 95% confidence intervals (CI). Binomial models were used to estimate adjusted risk differences (aRD) per 10,000 births and 95% CIs for associations between exposure to polluted sites and PTB or LBW.ResultsWe observed weak associations between residential proximity to polluted sites and PTB [aRR(95% CI): 1.07(1.06,1.09); aRD(95% CI): 61(48,74)] and LBW [aRR(95% CI): 1.09(1.06,1.12); aRD(95% CI): 24(17,31)]. Secondary analyses showed increased risk of both PTB and LBW among births exposed to sites characterized by water emissions, air emissions, and land impoundment. In analyses of specific contaminants, increased risk of PTB was associated with proximity to sites containing arsenic, benzene, cadmium, lead, mercury, and polycyclic aromatic hydrocarbons. LBW was associated with exposure to arsenic, benzene, cadmium, lead, and mercury.SignificanceThis study provides evidence for potential reproductive health effects of polluted sites, and underscores the importance of accounting for heterogeneity between polluted sites when considering these exposures.Impact statementWe documented an overall increased risk of both PTB and LBW in births with gestational exposure to polluted sites using a harmonized geodatabase of three site types, and further examined exposures stratified by site characteristics (route of emission, specific contaminants present). We observed increased risk of both PTB and LBW among births exposed to sites with water emissions or air emissions, across site types. Increased risk of PTB was associated with gestational proximity to sites containing arsenic, benzene, cadmium, lead, mercury, and polycyclic aromatic hydrocarbons; increased risk of LBW was associated with exposure to arsenic, benzene, cadmium, lead, and mercury.
