Coverage of school health monitoring systems in China: a large national cross-sectional survey.

ABSTRACT:

Background

There is growing interest in the role that schools can play in promoting student health. The aim of this study was to describe the coverage of school health monitoring systems for infectious diseases, non-communicable diseases, and school physical environments in China, and to explore differences by geography, regional wealth, and school type.

Methods

A cross-sectional study was performed using data from 2428 schools from 17 provinces in China in 2018. Data were collected using a questionnaire administered by the Ministry of Education through its monitoring system, and included infectious diseases (e.g., reporting system for student infectious diseases), non-communicable diseases (e.g., regular student health examinations), and school physical environments (e.g., monitoring of classroom light, microclimate and drinking water).

Findings

Overall, the coverage rate of full school health monitoring systems was 16·6%. The coverage rates of school health monitoring systems for infectious diseases, non-communicable diseases, and school physical environments were 71·2%, 68·5%, and 24·9%, respectively. Coverage was higher in schools from urban rather than rural areas, in schools from areas with greater wealth, and in senior secondary schools rather than junior secondary and primary schools.

Interpretation

Systems for monitoring infectious diseases in school students have been widely implemented in China. Systems for monitoring non-communicable diseases and physical environments need to be strengthened. Beyond greater attention in poorer and rural areas, increased investment in more comprehensive approaches to school health is indicated.

Project description:The Internet hospital, an innovative approach to providing health care, is rapidly developing in China because it has the potential to provide widely accessible outpatient service delivery via Internet technologies. To date, China's Internet hospitals have not been systematically investigated.The aim of this study was to describe the characteristics of China's Internet hospitals, and to assess their health service capacity.We searched Baidu, the popular Chinese search engine, to identify Internet hospitals, using search terms such as "Internet hospital," "web hospital," or "cloud hospital." All Internet hospitals in mainland China were eligible for inclusion if they were officially registered. Our search was carried out until March 31, 2017.We identified 68 Internet hospitals, of which 43 have been put into use and 25 were under construction. Of the 43 established Internet hospitals, 13 (30%) were in the hospital informatization stage, 24 (56%) were in the Web ward stage, and 6 (14%) were in full Internet hospital stage. Patients accessed outpatient service delivery via website (74%, 32/43), app (42%, 18/43), or offline medical consultation facility (37%, 16/43) from the Internet hospital. Furthermore, 25 (58%) of the Internet hospitals asked doctors to deliver health services at a specific Web clinic, whereas 18 (42%) did not. The consulting methods included video chat (60%, 26/43), telephone (19%, 8/43), and graphic message (28%, 12/43); 13 (30%) Internet hospitals cannot be consulted online any more. Only 6 Internet hospitals were included in the coverage of health insurance. The median number of doctors available online was zero (interquartile range [IQR] 0 to 5; max 16,492). The median consultation fee per time was ¥20 (approximately US $2.90, IQR ¥0 to ¥200).Internet hospitals provide convenient outpatient service delivery. However, many of the Internet hospitals are not yet mature and are faced with various issues such as online doctor scarcity and the unavailability of health insurance coverage. China's Internet hospitals are heading in the right direction to improve provision of health services, but much more remains to be done.

Project description:BACKGROUND:Residential instability during pregnancy has been linked to poor health outcomes. As a first step toward providing better health care to pregnant migrant women, the size and characteristics of this population and factors associated with mobility during pregnancy should be studied. METHODS:Using the "Monitoring Data of Chinese Migrants" for 2012, from the Chinese National Population and Family Planning Commission, this study explored mobility patterns during pregnancy and associated factors among migrants within China. From a library of 158,556 participants, two subsamples were selected. Percentages, with chi-squared tests, and means and standard deviations, with ANOVAs, were adopted to describe mobility patterns during pregnancy (always staying in sending area, mainly staying in sending area, mainly staying in receiving area, and always staying in receiving area) and delivery location choice. Logistic regression analysis was used to explore the associated factors. RESULTS:We found that the percentage of migrants always or mainly staying in receiving areas during pregnancy rose from nearly 40% in 1985 to more than 80% in 2012, while the percentage of migrants who were mobile between receiving and sending areas during pregnancy fluctuated between 30 and 40% before 1995, and between 40 and 45% after 1995, decreasing to around 40% after 2008. The percentage of respondents who chose to deliver in receiving areas fluctuated but increased from 10% in 1985 to more than 50% in 2011. Among respondents who had delivered during the last year of the survey period, families with older pregnant women (OR = 1.09, 95% CI 1.05-1.13), their own housing (OR = 5.66, 95% CI 2.45-13.05), longer time in the receiving area (OR = 1.14, 95% CI 1.09-1.20), and strong will to integrate (OR = 1.32, 95% CI 1.15-1.51) always stayed in the receiving area during pregnancy, rather than the sending area, and families with broadly similar characteristics were inclined to choose the receiving area for their delivery. CONCLUSIONS:The mobility patterns of pregnant migrant women in China have been changing in recent years, with the percentage of them staying in receiving areas during pregnancy and delivering there increasing. Individual and family characteristics were also associated with mobility patterns and delivery location choice.

Project description:IntroductionLow fertility rate has become an inevitable problem globally. Although current policies have a certain effect on promoting fertility and raising the birth rate, the overall effect is not obvious to meet the need. Therefore, the exploration of fertility intention and its affecting factors is extremely significant.MethodsThis study collected demographic data and the intention of respondents to have a second children, which focused on the factors that could affect fertility issues. 11,031 respondents were divided into non-fertile group (n = 5062) and fertile group (n = 5969) according to whether they had children or not, and the fertility group (n = 5969) were divided into group with 1-2 children (n = 5293) and group with ≥3 children (n = 676) according to the number of children. Non-fertility respondents aged 26-40 (n = 1369) were divided to explore the factors affecting the second-children intention. Binary logistic regression analysis was used to determine the affecting factors.ResultsIt was revealed that gender [Male: OR: 0.60, 95% CI: 0.54-0.68], age [26-40: OR: 16.0, 95% CI: 13.4-19.1; 41-60: OR: 233.8, 95% CI: 186.7-292.6; >60: OR: 105.6, 95% CI: 77.1-144.6], political status [Partisans: OR: 0.48, 95% CI: 0.42-0.54], highest educational level [Middle school: OR: 0.21, 95% CI: 0.17-0.26; College degree or above: OR: 0.09, 95% CI: 0.08-0.11], whether having chronic disease [Yes: OR: 1.95, 95% CI: 1.60-2.38] and depression [Mild depression: OR: 0.63, 95% CI: 0.56-0.72; Moderate depression: OR: 0.43, 95% CI: 0.36-0.53; Moderate to severe depression: OR: 0.45, 95% CI: 0.35-0.57; Severe depression: OR: 0.50, 95% CI: 0.33-0.74] were important factors affecting fertility intention. We found that age [26-40: OR: 0.11, 95% CI: 0.08-0.15; 41-60: OR: 0.15, 95% CI: 0.12-0.18; >60: 0.81, 95% CI: 0.66-0.99], region [Central China: OR: 1.49, 95% CI: 1.20-1.86; Western China: OR: 1.75, 95% CI: 1.41-2.18], resident place [Urban: OR: 0.59, 95% CI: 0.49-0.72], per capita monthly household income [6001-12000: OR: 0.63, 95% CI:0.46-0.83; ≥12,000: OR: 1.83, 95% CI: 1.20-2.80], political status [Non-partisans: OR: 0.24, 95% CI: 0.09-0.69], highest educational level [Middle school: OR: 0.36, 95%CI: 0.27-0.46; College degree or above: OR: 0.22, 95% CI: 0.17-0.30] and anxiety [Moderate anxiety: OR: 1.39, 95% CI: 1.04-1.88; Severe anxiety: OR: 2.19, 95% CI: 1.26-3.80] were the main affecting factors for choosing the number of children. Furthermore, the second-children intention investigation in respondents aged 26-40 showed that gender [Male: OR: 2.06, 95% CI: 1.67-2.53], resident place [Urban: OR: 0.59, 95% CI: 0.49-0.72], per capita monthly household income [≥12,000: OR: 1.86, 95% CI: 1.23-2.82] and pressure [Severe pressure: OR: 0.54, 95% CI: 0.34-0.85] were the important factors.ConclusionRegion, educational level, psychological factors, income, political status and medical insurance were the important factors affecting the intention of fertility and the number of children. The government should take these factors into account when optimizing the existing policy.

Project description:BackgroundWorld Health Organization estimated that 779 million people are at risk of getting schistosomiasis (SCH) and 240 million people were infected worldwide. SCH due to Schistosoma mansoni (S. mansoni) is a wide public health problem in Ethiopia. The aim of the survey was to quantify national and district disaggregated treatment coverage status for SCH and compare validated coverage with the one reported.MethodsCommunity based cross-sectional survey was conducted in April 2019 among households with school age children (SAC) 5-14 years in seven purposively selected districts of the country. Segments to be surveyed were randomly selected and households to be interviewed from each segment were determined using systematic sampling technique. A total of 3378 households visited and 5679 SAC (5-14 years) were interviewed.ResultsOverall reported treatment coverage of Praziquantel (PZQ) against SCH was 4286 (75.5%). Males were 27% more likely to swallow the drug (AOR = 1.27; 95% CI: 1.09, 1.47) than females. SAC with age 10-14 years were 45% more likely to swallow the drug compared with their counter parts (5-9 years), (AOR =1.45; 95% CI: 1.25, 1.69). There is statistically significant association between PZQ swallowing status with school enrollment. (AOR = 20.90, 95% CI: 17.41, 25.08). Swallowing status of PZQ against SCH significantly higher for SAC treated in districts applied integrated treatment approach (87.5%) compared with SAC treated in vertical treatment approach (72.5%); P-value < 0.001. SACs were asked for reasons for not taking the drug and the main reported reason for not swallowing PZQ in the present study was none attending of the school.ConclusionsOver all treatment coverage of PZQ against SCH in the present study was 75.5%. Although it is in accordance with WHO recommendation for Ethiopia, national programmatic improvements are necessary to achieve higher coverage in the future. To increase treatment coverage for PZQ against SCH in Ethiopia, school based training should target all schools. Moreover, mobilization, sensitization and implementation of the community wide treatment need to be improved.

Project description:ObjectivesApproximately 10 years ago, China introduced an education plan to improve the overall quality of medical education and to better serve the population's health needs. Many medical schools were then recognised and financed by China's Ministry of Education to develop and operationalise new pilot programmes (PPs) aligned with this plan. These ran in parallel with the traditional programmes (TPs). One way to achieve the plan's first aim, improving the quality of medical education, is to select academically stronger candidates. We, thus, examined and compared who were selected into PPs and TPs.DesignCross-sectional study.SettingData were collected from 123 medical schools across China via the 2021 China Medical Student Survey.ParticipantsParticipants were undergraduate clinical medicine students across all year groups.Primary and secondary outcome measuresMedical school selection was via the National College Entrance Examination (NCEE). Medical students' NCEE performance and their sociodemographics were used as the primary and secondary outcome measures. Mann-Whitney or χ2 tests were used to compare the means between educational programmes (PPs vs TPs) and various selection outcomes. Multilevel mixed-effects regressions were employed to account for school idiosyncratic selection results.ResultsOf the 204 817 respondents, 194 163 (94.8%) were in a TP and 10 654 (5.2%) a PP. PP respondents (median=75.2, IQR=69.5-78.8) had significantly higher NCEE scores than their TP counterparts (median=73.9, IQR=68.5-78.7). Holding constant their NCEE score, PP respondents were significantly more likely to come from urban areas, not be first-generation college students, and have parents with higher occupational status and income.ConclusionsAssuming quality can be indicated by prior academic achievement at the point of selection, PPs achieved this mission. However, doing so limited medical students' diversity. This may be unhelpful in achieving the Education Plan's goal to better serve China's health needs.

Project description:BackgroundIn response to the increasing cancer prevalence and the evolving health service landscape across the public and private health sectors in Australia, this study aimed to map cancer services and identify factors associated with service provision and important service gaps.MethodsA prospective, cross-sectional survey was conducted throughout 2016. Extensive search strategies identified Government or privately-owned, hospital or community-based healthcare organisations with dedicated cancer services. One nominated staff member from each organisation answered a purpose specific online/paper questionnaire. Descriptive statistics, standardised rates, and single level and multilevel multinomial logistic regression were used to analyse the data. Analysis was augmented with a qualitative descriptive analysis of open-ended questions.ResultsFrom the 295 eligible organisations with a cancer service in Australia, 93.2% participated in the survey. After adjusting for remoteness, for-profit companies were significantly more likely than Government operated services to provide only one or two types of cancer services (e.g. radiotherapy) in a limited range of settings (e.g. day hospital with no in-patient or home care) (p < 0.001) and less likely to provide comprehensive cancer services (p < 0.001). After adjusting for ownership and the respondent's role in the organisation, respondents located in remote regions of Australia were more likely to identify cancer services that are dependent upon specialist medical practitioners as the most important service gaps in their region (p = 0.003). Despite 76.0% of organisations across Australia offering some type of supportive care or survivorship services, providers identified this group of services as the most pressing service gaps in major cities, rural and remote regions alike (standardised rate: 47.9% (95%CI: 43.6-57.4%); p < .000). This included the need for improved integration, outreach and affordability.ConclusionsThe broad range of cancer services, settings and ownership identified by this survey highlights the complexity of the Australian healthcare system that cancer survivors must navigate and the challenges of providing comprehensive cancer care particularly in rural and remote regions. Whilst the significant role of supportive care and survivorship services are increasingly being recognised, the findings from this survey support calls for innovative service models and funding mechanisms that expand the focus from preventing and treating cancer to supporting cancer survivors throughout the cancer continuum and promoting the delivery of integrated and equitable cancer care across the public and private sectors.
