Patellar Tendon Repair With Suture Tape Augmentation for Proximal Patellar Tendon Rupture.

ABSTRACT: Patellar tendon ruptures are severe but uncommon injuries that require surgical treatment. Primary repair for acute patellar tendon ruptures using augmentation techniques has shown good results in terms of biomechanical and clinical outcomes. This Technical Note details patellar tendon repair with suture tape augmentation for proximal patellar tendon rupture. Because this surgical technique does not require harvesting of the hamstring tendon and hardware removal, it is minimally invasive. In addition, it is simple and quick to perform.

Proximal Patellar Tendon Repair: Internal Brace Technique With Unicortical Buttons and Suture Tape.

Project description:Patellar tendon ruptures may be considerably limiting, especially in younger and highly active patients. These injuries ultimately result in a complete inability to maintain extension of the knee, thereby placing strict impediment on physical activity. As a result, a durable repair construct via surgery is necessary to allow patients to return to their preinjury activity level. Because of the inherent difficulty in maintaining patellar tendon position after repair, and to avoid failure of the tendon healing to the patella, we recommend using an internal brace construct. The construct uses bone tunnels in the patella and also cortical buttons on the tibia with suture tape whipstitched through the tendon. We feel that this provides an enhanced fixation construct. The purpose of this Technical Note is to describe our preferred method for proximal patellar tendon repair via an internal brace construct with unicortical buttons and suture tape.

Patellar Tendon Reconstruction Using Semitendinosus Allograft With Suture Tape Augmentation for Recalcitrant Patellar Tendinosis.

Project description:Chronic patellar tendinosis is an overuse pathology affecting both adolescents and adults who participate in jumping sports. It often can be managed with nonoperative modalities. Chronic patellar tendinosis is associated with degenerative changes to the proximal, posterior patellar tendon. Operative treatments include open or arthroscopic debridement of the pathologic tissue. When a debridement is performed, the resulting defect forms a void at the posterior segment of the proximal patellar tendon. The authors assert this defect should be addressed in elite athletes with a graft as opposed to tendon debridement alone to prevent recurrence and maximize tendon strength. Described herein is the senior author's preferred technique for open patellar tendon debridement and allograft reinforcement with suture tape augmentation for chronic patellar tendinosis. This technique creates both a tension band construct with allograft to offload the tendon proximally and an InternalBrace construct longitudinally for patellar tendon load sharing.

Double-Row Suture Anchor Fixation and Achilles Allograft Augmentation for Chronic Patellar Tendon Rupture Repair.

Project description:Patellar tendon ruptures are functionally devastating injuries that result in failure of the knee extensor mechanism and can lead to a loss of ambulation. Chronic patellar tendon injuries are defined as tears greater than 2 weeks old and are typically more complex to manage than acute tears. Recently, the use of double-row suture anchor configurations has been explored as a technique to provide improved strength in addition to tendon-to-bone compression at the anatomic footprint. The purpose of this article is to describe a surgical technique involving chronic patellar tendon rupture repair using a double-row suture construct augmented with Achilles allograft. Our technique offers a variety of benefits and permits early postoperative mobilization.

High-Strength Suture Tape Augmentation Improves Cyclic Gap Formation in Transosseous Patellar Tendon Repair: A Biomechanical Study.

Project description:PurposeTo compare gap displacement at various intervals of cyclic testing and biomechanical load to failure of a Krackow patellar tendon repair augmented with high-strength suture tape versus the standard Krackow transosseous technique for inferior pole patellar tendon rupture.MethodsTwelve matched pairs of cadaveric knees were used (8 males and 4 females; mean age 79.6 years, range 57 to 96). An inferior pole patellar tendon rupture was simulated after random assignment of specimens in each pair to the standard or augmented Krackow technique. Each specimen was then repetitively cycled from 90° to 5° for 1,000 cycles. A differential variable reluctance transducer was used to measure gap displacement. After cyclic loading, load to failure was determined by pulling the tendon at a rate of 15 mm/s until a sudden decrease in load occurred.ResultsCompared with the control repair, specimens with augmented repair demonstrated significantly less displacement at all testing intervals up to 1,000 cycles (P < .05). Two patellar tendons failed before the end of cyclic loading, and 4 specimens had inadequate tendon length for loading. Among the 18 remaining specimens, no significant difference in load to failure was observed between the experimental group (n = 11) and the control group (n = 7) (1,006.5 ± 332.1 versus 932.8 ± 229.1 N, respectively; P = .567).ConclusionsSignificantly greater gap displacement was observed in the standard Krackow repair group compared with the augmented Krackow group at all cyclic loading intervals. This suggests that the Krackow transosseous procedure augmented with high-strength suture tape is biomechanically viable for inferior pole patellar tendon repair.Clinical relevanceThis biomechanical study supports the use of high-strength suture tape augmentation of Krackow transosseous repair for inferior pole patellar tendon rupture.

Anterior Cruciate Ligament Reconstruction Using Bone-Patellar Tendon-Bone Autograft With Suture Tape Augmentation.

Project description:The anterior cruciate ligament (ACL) is the most commonly injured ligament in the knee, with injury usually occurring as a result of multidirectional sports. The incidence of ACL injury has continued to increase, with most patients opting for surgery to improve stability as well as permit a return to sport. Traditional methods of ACL reconstruction can achieve this but are not without their problems, including graft rupture, residual laxity, and donor-site morbidity. There is therefore a requirement for further research into newer, innovative surgical techniques to help improve complication rates. This article describes, with video illustration, ACL reconstruction using a reduced-size bone-patellar tendon-bone autograft with suture tape augmentation. The augmentation acts as a stabilizer during the early stages of graft incorporation while resisting against reinjury during an accelerated recovery. The ability to use a reduced-size graft decreases the donor-site burden, and retention of residual native ACL tissue, when possible, may help with proprioception.

Primary PLC repairs have been associated with a high failure rate in past literature. However, with modern improved arthroscopic instrumentation and devices, there has been a renewed interest in repair of the ligaments around the knee. Anterolateral Ligament Repair With Suture Tape Augmentation.

Project description:Recent insights into the structure and function of the anterolateral ligament (ALL) of the knee has resulted in a recognition of its contribution in rotational control of the knee. Several ALL reconstruction techniques have been described in the literature. This article describes, with video illustration, a percutaneous repair technique using suture tape augmentation. A tendon graft is not needed. This technique allows early mobilization and encourages natural healing of the ligament by protecting the ligament during the healing phase as a secondary stabilizer.

Chronic Midsubstance Patellar Tendon and Retinacular Rupture: Primary Repair Enhancement Using Bioinductive Implant Augmentation.

Project description:Midsubstance tears of the patellar tendon are uncommon and present a difficult injury to treat. If left untreated, these can be debilitating injuries for patients and leave them with an overall lack of function in the injured extremity. Compared to a proximal or distal patellar tendon rupture, midsubstance tears rely on tendon-to-tendon healing instead of bone to tendon healing. Given this situation, specific preoperative planning and the use of a bioinductive scaffolding allows surgeons to enhance the overall construct, while promoting a beneficial healing environment. Although the addition of bioinductive implants has grown in popularity for upper extremity injuries, few cases describe its use in the lower extremity setting. Here, we present a case of midsubstance patellar tendon repair, as well as a medial and lateral retinacular repair using a structural biological implant with Type I collagen for augmentation to enhance our overall final construct.
