Volumetric Measurements in Lung Cancer Screening Reduces Unnecessary Low-Dose Computed Tomography Scans: Results from a Single-Center Prospective Trial on 4119 Subjects.

ABSTRACT: This study aims to compare the low-dose computed tomography (LDCT) outcome and volume-doubling time (VDT) derived from the measured volume (MV) and estimated volume (EV) of pulmonary nodules (PNs) detected in a single-center lung cancer screening trial. MV, EV and VDT were obtained for prevalent pulmonary nodules detected at the baseline round of the bioMILD trial. The LDCT outcome (based on bioMILD thresholds) and VDT categories were simulated on PN- and screenee-based analyses. A weighted Cohen's kappa test was used to assess the agreement between diagnostic categories as per MV and EV, and 1583 screenees displayed 2715 pulmonary nodules. In the PN-based analysis, 40.1% PNs were included in different LDCT categories when measured by MV or EV. The agreements between MV and EV were moderate (κ = 0.49) and fair (κ = 0.37) for the LDCT outcome and VDT categories, respectively. In the screenee-based analysis, 46% pulmonary nodules were included in different LDCT categories when measured by MV or EV. The agreements between MV and EV were moderate (κ = 0.52) and fair (κ = 0.34) for the LDCT outcome and VDT categories, respectively. Within a simulated lung cancer screening based on a recommendation by estimated volumetry, the number of LDCTs performed for the evaluation of pulmonary nodules was higher compared with in prospective volumetric management.

Project description:BackgroundPancreatic perfusion computed tomography (CT) imaging is increasingly used for neoplastic grading, predicting prognosis, and evaluating the response to therapy. To optimize the clinical pancreatic CT perfusion imaging methods, we evaluated 2 different CT scanning protocols concerning pancreas perfusion parameters.MethodsA retrospective study was conducted on 40 patients who underwent whole pancreas CT perfusion scanning in The First Affiliated Hospital of Zhengzhou University. Of these 40 patients, 20 patients in group A underwent continuous perfusion scanning, while 20 patients in group B underwent intermittent perfusion scanning. For group A, continuous axial scanning was performed 25 times, and the total scan time was 50 s. For group B, arterial phase helical perfusion scanning was performed 8 times, and then venous phase helical perfusion scanning was performed 15 times, with a total scan time of 64.6 to 70.0 s. A comprehensive list of perfusion parameters between different parts of the pancreas and the 2 groups were compared. The effective radiation dose for the 2 scanning methods was analyzed.ResultsThe parameter of the mean slope of increase (MSI) at different pancreatic parts in group A differed (P=0.028). The pancreas head had the lowest value, and the tail had the highest (about a 20% difference). In group A compared to group B, the blood volume of the pancreatic head was smaller (15.256±2.925 vs. 16.953±3.602), the positive enhanced integral was smaller (0.307±0.050 vs. 0.344±0.060) and the permeability surface was larger (34.205±9. 612 vs. 24.377±8.413); the blood volume of the pancreatic neck was smaller (13.940±2.691 vs. 17.173±3.918), the positive enhanced integral was smaller (0.304±0.088 vs. 0.361±0.051) and the permeability surface was larger (34.898±11.592 vs. 25. 794±8.149); the blood volume of the pancreatic body was smaller (16.142±4.006 vs. 18.401±2.513), the positive enhanced integral was smaller (0.305±0.093 vs. 0.342±0.048) and the permeability surface was larger (28.861±10.448 vs. 22.158±6. 017); the blood volume of the pancreatic tail was smaller (16.446±3.709 vs. 17.374±3.781), the positive enhanced integral was smaller (0.304±0.057 vs. 0.350±0.073) and the permeability surface was larger (27.823±8.228 vs. 21.509±7.768) (P<0.05). The effective radiation dose in the intermittent scan mode was slightly lower at 16.657±2.259 mSv than in the continuous scan mode (17.973±3.698 mSv).ConclusionsDifferent CT scanning intervals had a significant influence on whole pancreas blood volume, permeability surface, and positive enhanced integral. These demonstrate the high sensitivity of intermittent perfusion scanning for identifying perfusion abnormalities. Therefore, for the diagnosis of pancreatic diseases, intermittent pancreatic CT perfusion may be more advantageous.

Project description:BackgroundProstatic diseases are common and mostly associated with enlargement of the accessory gland. Thus, determining the prostate size has become a main criterion for evaluating prostate health status. Computed tomography (CT) is recommended as a beneficial tool for evaluating prostate size, morphology and surrounding tissues. The purpose of this study was to establish an accurate procedure for volume estimation and afterwards evaluate the prostate volume in CT. Data of 95 dogs were analysed (58 male intact, 37 male neutered) using the slice addition technique with the Amira program. Accuracy of volumetric measurements by CT was validated by comparing them with those of phantoms of known volume. Patients were grouped according to age (< 4 yrs., 4-8 yrs., > 8 yrs) and prostate morphology in CT (H = homogeneous, I = inhomogeneous, C = cystic). The length of the sixth lumbar vertebra was measured to relate prostate volume to body size. This ratio was generated to compare prostate volume between the groups, irrespective of body size (ratio volume = Rv).ResultsA high correlation between the CT-derived and phantom volume was found. Overall, the mean prostate volume was 58.6 cm3. The mean ratio volume was 1.3 in intact male dogs, this being significantly higher than in neutered dogs (0.7). The lowest ratio volume values were found in group H for intact (Rv = 0.9) and neutered dogs (Rv = 0.6), followed by group I (intact: Rv = 1.1; neutered: Rv = 0.7) and C (intact: Rv = 1.4; neutered: Rv = 0.8). The length of the sixth lumbar vertebra was well correlated with the prostate volume (intact: r = 0.63, p < 0.001; neutered: r = 0.48, p = 0.003), while age exhibited a correlation only in intact dogs (r = 0.52, p < 0.001).ConclusionThe present study is pioneering in applying a slice addition technique to volumetric measurements of the prostate gland in CT, resulting in a highly precise method. Volumetric measurements of the canine prostate gland in CT images provide information about the prostate structure, castration status, age and body size of the patients. Therefore, prostate volume is a relevant parameter for evaluating prostate health status.
