Dietary Pattern Associated with the Risk of Hyperuricemia in Chinese Elderly: Result from China Nutrition and Health Surveillance 2015-2017.

ABSTRACT: Our current study aimed to estimate the relationship between dietary patterns and hyperuricemia among the Chinese elderly over 60 years old. All the data were obtained from China Nutrition and Health Surveillance during 2015-2017. A total of 18,691 participants who completed the whole survey were included in our statistical analysis. The definition of hyperuricemia was 420 μmmol/L (7 mg/dL) for male and 360 μmmol/L (6 mg/dL) for female. Exploratory factor analysis was applied to explore posterior dietary patterns in our samples, and five dietary patterns were recognized, namely "Typical Chinese", "Modern Chinese", "Western", "Animal products and alcohol", and "Tuber and fermented vegetables". After multiple adjusted logistic regression, participants in the highest quartile of "typical Chinese" (Q4 vs. Q1, OR = 0.32, 95% CI: 0.28-0.37, p-trend < 0.0001), "modern Chinese" (Q4 vs. Q1, OR = 0.81, 95% CI: 0.71-0.93, p-trend = 0.0021) and "tuber and fermented vegetables" (Q4 vs. Q1, OR = 0.78, 95% CI: 0.69-0.88, p-trend < 0.0001) showed a lower risk of hyperuricemia, while animal products and alcohol was positively associated with hyperuricemia (Q4 vs. Q1, OR = 1.49, 95% CI: 1.31-1.7, p-trend < 0.0001). We also found that participants who mainly ate a modern Chinese diet tended to meet the RNI/AI of nutrients we discuss in this paper, which may supply some information for hyperuricemia prevention and management by dietary methods.

Project description:Objectives: This study investigated regional dietary patterns and their association with hypertensive dyslipidemia multimorbidity among older Chinese adults. Methods: Data from 13,809 individuals (aged ≥ 65 years) were extracted from the 2015-2017 China Nutrition and Health Surveillance. Hypertensive dyslipidemia multimorbidity was defined as concurrent hypertension and dyslipidemia. Four dietary patterns were identified via exploratory factor analysis using 3-day 24 h dietary records. Results: Four dietary patterns were extracted: traditional southern, diverse, high oil and salt, and animal oil-other animal meat-coarse grain pattern. The national prevalence of hypertensive dyslipidemia multimorbidity was 31.9%, with significant regional variation (highest in North China: 36.4%; lowest in Central China: 27.9%; p < 0.0001). Regional dietary dominance included: the traditional southern pattern in South China (75.9%); the traditional southern (35.8%) and diversified (28.8%) patterns in East China; animal oil-other animal meat-coarse grain (38.4%) and traditional southern (37.9%) patterns in Southwest China; high oil and salt (36.8%) and traditional southern (25.9%) in Central China; diversified (47.7%) and high oil and salt (26.3%) patterns in North China; high oil and salt (44.9%) and diversified (37.9%) patterns in Northwest China; and the diversified (46.9%) pattern in Northeast China. In the highest scoring group of the animal oil-other animal meat-coarse grain pattern, the risk of hypertensive dyslipidemia multimorbidity was 1.179 times greater compared with the lowest scoring group (Q4 vs. Q1, OR = 1.179, 95% CI: 1.032-1.316, p < 0.001). Region-specific analyses highlighted: increased risks with the traditional southern pattern in South/East China (Q4 vs. Q1, OR = 2.080, 95% CI: 1.036-4.175, p < 0.001) and (Q4 vs. Q1, OR = 1.734, 95% CI: 1.052-2.859, p < 0.001); protective effects of the diversified pattern in Northwest China (Q4 vs. Q1, OR = 0.377, 95% CI: 0.173-0.824, p < 0.001); elevated risks with the high oil and salt pattern in Central/North China (Q4 vs. Q1, OR = 2.343, 95% CI: 1.107-4.955, p < 0.001) and (Q4 vs. Q1, OR = 1.837, 95% CI: 1.019-3.312, p < 0.001); and higher risks with the animal oil-other animal meat-coarse grain pattern in Southwest China (Q4 vs. Q1, OR = 2.009, 95% CI: 1.004-4.021, p < 0.001). Conclusions: In conclusion, dietary patterns among elderly Chinese are diversified and should be optimized based on local food resources to control hypertensive dyslipidemia multimorbidity.

Project description:Regional dietetic cultures were indicated in China, but how dietary patterns geographically varied across China is unknown. Few studies systematically investigated the association of dietary patterns with overweight/obesity and hypertension and the potential mechanism with a national sample. This study included 34,040 adults aged 45 years and older from China Nutrition and Health Surveillance (2015-2017), who had complete outcome data, reliable intakes of calorie and cooking oils, unchanged diet habits, and without diagnosed cancer or cardiovascular disease. Outcomes were overweight/obesity and hypertension. By using the Gaussian finite mixture models, four dietary patterns were identified-common rice-based dietary pattern (CRB), prudent diversified dietary pattern (PD), northern wheat-based dietary pattern (NWB), and southern rice-based dietary pattern (SRB). Geographic variations in dietary patterns were depicted by age-sex standardized proportions of each pattern across 31 provinces in China. We assessed the association of these dietary patterns with outcomes and calculated the proportion mediated (PM) by overweight/obesity in the association of the dietary patterns with hypertension. Evident geographic disparities in dietary patterns across 31 provinces were observed. With CRB as reference group and covariates adjusted, the NWB had higher odds of being overweight/obese (odds ratio (OR) = 1.44, 95% confidence interval (CI): 1.36-1.52, p < 0.001) and hypertension (OR = 1.07, 95%CI: 1.01-1.14, p < 0.001, PM = 43.2%), while the SRB and the PD had lower odds of being overweight/obese (ORs = 0.84 and 0.92, 95%CIs: 0.79-0.89 and 0.85-0.99, p < 0.001 for both) and hypertension (ORs = 0.93 and 0.87, 95%CIs: 0.87-0.98 and 0.80-0.94, p = 0.038 for SRB and p < 0.001 for PD, PMs = 27.8% and 9.9%). The highest risk of overweight/obesity in the NWB presented in relatively higher carbohydrate intake (about 60% of energy) and relatively low fat intake (about 20% of energy). The different trends in the association of protein intake with overweight/obesity among dietary patterns were related to differences in animal food sources. In conclusion, the geographic distribution disparities of dietary patterns illustrate the existence of external environment factors and underscore the need for geographic-targeted dietary actions. Optimization of the overall dietary pattern is the key to the management of overweight/obesity and hypertension in China, with the emphasis on reducing low-quality carbohydrate intake, particularly for people with the typical northern diet, and selection of animal foods, particularly for people with the typical southern diet.
