Tracking human activity in real time and at fine spatial scale is particularly valuable during episodes such as the COVID-19 pandemic. Project description:How does interconnectedness affect the course of a pandemic? What are the optimal containment policies in an economy with connected regions? We embed a spatial SIR model into a multi-sector quantitative trade model. We calibrate it to US states and the COVID-19 pandemic and find that interconnectedness increases the death toll by 146,200 lives. State-level policies that reduce within-state economic activity mitigate welfare losses by more than a uniform national policy or a policy that only reduces mobility between states. The optimal policy in mitigating welfare losses generated by the pandemic combines local within- and between-state restrictions and saves 289,300 lives, despite significantly exacerbating economic losses and imposing mobility restrictions across states. Different timing of policies across states is key to minimize welfare losses. States like South Carolina might have imposed internal lockdowns too early but travel restrictions too late.

Project description:The evidence on the demographics of COVID-19 fatalities points to an overrepresentation of minorities and an underrepresentation of women. We investigate the joint impact of race and gender using individual-level georeferenced death data collected by the Cook County Medical Examiner, which we structure as a cell-level panel at a race, block group, week, and year level. Through an event study approach, we establish that Black individuals are affected more harshly than White and that the effect is driven by Black women. The Black female bias is associated with occupational segregation in the health care and transportation sectors and by commuting on public transport.

Project description:ImportanceMobile smartphones are rapidly emerging as an effective means of communicating with many Americans. Using mobile applications (apps), they can access remote databases, track time and location, and integrate user input to provide tailored health information.ObjectiveA smartphone mobile app providing personalized, real-time sun protection advice was evaluated in a randomized clinical trial.Design, setting, and participantsThe trial was conducted in 2012 and had a randomized pretest-posttest controlled design with a 10-week follow-up. Data were collected from a nationwide population-based survey panel. A sample of 604 non-Hispanic and Hispanic adults from the Knowledge Panel 18 years or older who owned an Android smartphone were enrolled.InterventionsThe mobile app provided advice on sun protection (ie, protection practices and risk of sunburn) and alerts (to apply or reapply sunscreen and get out of the sun), hourly UV Index, and vitamin D production based on the forecast UV Index, the phone's time and location, and user input.Main outcomes and measuresPercentage of days using sun protection and time spent outdoors (days and minutes) in the midday sun and number of sunburns in the past 3 months were collected.ResultsIndividuals in the treatment group reported more shade use (mean days staying in the shade, 41.0% vs 33.7%; P = .03) but less sunscreen use (mean days, 28.6% vs 34.5%; P = .048) than controls. There was no significant difference in number of sunburns in the past 3 months (mean, 0.60 in the treatment group vs 0.62 for controls; P = .87). Those who used the mobile app reported spending less time in the sun (mean days keeping time in the sun to a minimum, 60.4% for app users vs 49.3% for nonusers; P = .04) and using all protection behaviors combined more (mean days, 39.4% vs 33.8%; P = .04).Conclusions and relevanceThe mobile app improved some sun protection. Use of the mobile app was lower than expected but associated with increased sun protection. Providing personalized advice when and where people are in the sun may help reduce sun exposure.

Project description:BackgroundPolice officers work under stressful conditions, and the resulting occupational stress may impact their health and well-being through changes in positive affect (PA) and negative affect (NA). It is therefore important to assess officers' stress, PA, and NA while it is experienced. This study evaluates police officers' momentary stress and affect in real-world settings using an ecological momentary assessment, and examines the within-person (W) and between-person (B) factors that influence momentary affect.MethodsEighty-nine police officers were recruited in South Korea. Participants completed questionnaires about their momentary stress and affect using a smartphone application. The associations between momentary stress, momentary contextual environment, momentary PA, and momentary NA were examined using mixed modeling.ResultsSocial overload (W: -.37), work discontent (W: -.45, B: -.73), social tension (W: -.79, B: -.67), and pressure to perform (W: -.29, B: -.49) were significantly associated with lower PA. Work overload (B: .33) and social isolation(W: .48, B: .31) were significantly associated with higher PA. Being with family (W: .71, B: .91) and friends (W: 1.89, B: 2.45) were significantly associated with higher PA. Being at home or other places away from the work place were significantly associated with higher PA (W: 1.01) and when patrolling or investigating were associated with lower PA (B: - 1.13). Lack of social recognition (W: 1.74, B: 2.33), work discontent (W: 1.59, B: 1.88), social tension (W: 1.74, B: 2.92), and pressure to perform (W: .78, B: 1.92) were significantly associated with higher NA. Being with colleagues (W: - 1.43), family (W: -1.38, B: - 2.66) and friends (W: -1.78, B: - 2.45) were associated with lower NA.ConclusionsMomentary within-person and between-person stress factors and contextual factors influenced police officers' momentary affect. These factors should be considered when developing interventions to mitigate stress and improve affect in police officers.

Project description:BackgroundType 1 diabetes (T1D) care requires multiple daily self-management behaviors (SMBs). Preliminary studies on SMBs rely mainly on self-reported survey and interview data. There is little information on adult T1D SMBs, along with corresponding compensation techniques (CTs), gathered in real-time.ObjectiveThe article aims to use a patient-centered approach to design iDECIDE, a smartphone application that gathers daily diabetes SMBs and CTs related to meal and alcohol intake and exercise in real-time, and contrast patients' actual behaviors against those self-reported with the app.MethodsTwo usability studies were used to improve iDECIDE's functionality. These were followed by a 30-day pilot test of the redesigned app. A survey designed to capture diabetes SMBs and CTs was administered prior to the 30-day pilot test. Survey results were compared against iDECIDE logs.ResultsUsability studies revealed that participants desired advanced features for self-tracking meals and alcohol intake. Thirteen participants recorded over 1,200 CTs for carbohydrates during the 30-day study. Participants also recorded 76 alcohol and 166 exercise CTs. Comparisons of survey responses and iDECIDE logs showed mean% (standard deviation) concordance of 77% (25) for SMBs related to meals, where concordance of 100% indicates a perfect match. There was low concordance of 35% (35) and 46% (41) for alcohol and exercise events, respectively.ConclusionThe high variability found in SMBs and CTs highlights the need for real-time diabetes self-tracking mechanisms to better understand SMBs and CTs. Future work will use the developed app to collect SMBs and CTs and identify patient-specific diabetes adherence barriers that could be addressed with individualized education interventions.

Project description:Background: Sedentary behavior is associated with various adverse health outcomes. Medical students often experience high academic demands, leading to increased sedentary time. This study aimed to evaluate the effectiveness of a mobile app providing real-time feedback in reducing total sedentary time and prolonged sedentary bouts and in promoting physical activity among medical students. Methods: Seventy-seven medical students from Jazan University (mean age: 21.4 years; range: 20-25 years) participated in this study. Participants were assigned to either the control group (n = 40) or the intervention group (n = 37). The intervention group received real-time vibration feedback via a mobile app, prompting movement every 30 min of sedentary time, while the control group received no intervention. Sedentary behavior and physical activity levels were assessed using the Activities Completed Over Time in 24 h. Paired t-tests were conducted to examine within-group changes, and a two-way ANOVA was used to assess the interaction effect of time and group on sedentary time and physical activity. Results: After six weeks, the intervention group showed significant within-group reductions in their total sedentary time of 1.82 h (p = 0.01) and prolonged sedentary bouts of 1.91 h (p = 0.001), while the control group had no significant changes. Physical activity levels did not significantly change within either group. The two-way ANOVA revealed that there was no significant change over time between the two groups in their total sedentary time F (1, 75) = 1.590, p = 0.21, prolonged sedentary bouts F (1, 75) = 3.499, p = 0.06, or physical activity F (1, 75) = 0.565, p = 0.45. Conclusions: Real-time feedback from mobile apps resulted in significant within-group reductions in total and prolonged sedentary time among medical students in the intervention group. Low-cost mobile apps providing real-time feedback may be an effective intervention for reducing sedentary behavior among medical students, potentially improving their health and well-being.

Project description:Recordings of smartphone use for contacts are increasingly being used as alternative or supplementary measurement methods for social interactions and social relations in the health sciences. Less work has been done to understand how these measures compare with widely used survey-based information. Using data from the Copenhagen Network Study, we investigated whether derived survey and smartphone measures on two widely studied concepts; Social integration and Tie strength were associated. The study population included 737 college students (mean age 21.6 years, Standard deviation: 2.6), who were followed with surveys and continuous recordings of smartphone usage over a one-month period. We derived self-reported and smartphone measures of social integration (social role diversity, social network size), and tie strength (contact frequency, duration and tie reciprocity). Logistic regression models were used to assess the associations between smartphone derived and self-reported measures adjusting for gender, age and co-habitation. Larger call and text message networks were associated with having a high self-reported social role diversity, and a high self-reported social contact frequency was likewise associated with having both frequent call and text message interactions, longer call duration and a higher level of reciprocity in call and text message communication. Self-reported aspects of social relations and smartphone measures of social interactions have considerable overlap supporting a measurement of similar underlying concepts.

Project description:Due to the suspension of in-person classes in response to the COVID-19 pandemic, students at universities with earlier spring breaks traveled and returned to campus while those with later spring breaks largely did not. We use variation in academic calendars to study how travel affected the evolution of COVID-19 cases and mortality. Estimates imply that counties with more early spring break students had a higher growth rate of cases than counties with fewer early spring break students. The increase in case growth rates peaked two weeks after spring break. Effects are larger for universities with students more likely to travel through airports, to New York City, and to popular Florida destinations. Consistent with secondary spread to more vulnerable populations, we find a delayed increase in mortality growth rates. Lastly, we present evidence that viral infection transmission due to college student travel also occurred prior to the COVID-19 pandemic.

Project description:We argue that occupations are a key explanatory variable for understanding the early transmission of COVID-19 in New York City, finding that they play a larger role than other key demographics such as race or income. Moreover, we find no evidence that commuting patterns are significant after controlling for occupations. On the other hand, racial disparities still persist for Blacks and Hispanics compared with Whites, although the disparities' magnitudes are economically small. We perform our analysis over a range of several weeks to evaluate how different channels interact with the progression of the pandemic and the stay-at-home order. While the coefficient magnitudes of many occupations and demographics decrease, we find evidence consistent with higher intra-household contagion over time. Finally, our results also suggest that crowded spaces play a more important role than population density in the spread of COVID-19.

Project description:Out-of-hospital cardiac arrest (OHCA) is recognized as a global mortality challenge, and digital strategies could contribute to increase the chance of survival. In this paper, we investigate if cardiopulmonary resuscitation (CPR) quality measurement using smartphone video analysis in real-time is feasible for a range of conditions. With the use of a web-connected smartphone application which utilizes the smartphone camera, we detect inactivity and chest compressions and measure chest compression rate with real-time feedback to both the caller who performs chest compressions and over the web to the dispatcher who coaches the caller on chest compressions. The application estimates compression rate with 0.5 s update interval, time to first stable compression rate (TFSCR), active compression time (TC), hands-off time (TWC), average compression rate (ACR), and total number of compressions (NC). Four experiments were performed to test the accuracy of the calculated chest compression rate under different conditions, and a fifth experiment was done to test the accuracy of the CPR summary parameters TFSCR, TC, TWC, ACR, and NC. Average compression rate detection error was 2.7 compressions per minute (±5.0 cpm), the calculated chest compression rate was within ±10 cpm in 98% (±5.5) of the time, and the average error of the summary CPR parameters was 4.5% (±3.6). The results show that real-time chest compression quality measurement by smartphone camera in simulated cardiac arrest is feasible under the conditions tested.
