Project description:Very little is known about how cellular osmosensors monitor changes in osmolarity of the environment. Here, we report that in yeast, Sln1 osmosensor histidine kinase monitors changes in turgor pressures. Reductions in turgor caused by either hyperosmotic stress, nystatin, or removal of cell wall activate MAPK Hog1 specifically through the SLN1 branch, but not through the SHO1 branch of the high osmolarity glycerol pathway. The integrity of the periplasmic region of Sln1 was essential for its sensor function. We found that activity of the plant histidine kinase cytokinin response 1 (Cre1) is also regulated by changes in turgor pressure, in a manner identical to that of Sln1, in the presence of cytokinin. We propose that Sln1 and Cre1 are turgor sensors, and that similar turgor-sensing mechanisms might regulate hyperosmotic stress responses both in yeast and plants.

Project description:Background. Previous research has examined the individual effects of uncertainty, time pressure, perceived stress, and team support on decision making. However, scant research has investigated how team support and perceived stress collectively influence providers' perception of decision conflict and satisfaction with decision. Objectives. The present study aims to fill this void by examining the potential mediating role of perceived stress and team support in the relationship between time pressure, uncertainty, decision satisfaction, and decision conflict. Methods. Obstetrics and gynecology (Obs and Gynae) physicians (N = 347) working in tertiary care hospitals were approached through snowball and purposive convenient sampling. Self-reported data were collected in the form of questionnaires. Results. Structural equation modeling was used to uncover the complex linkages. Perceived stress was found to be a significant mediator between uncertainty and decision conflict (b = -0.033, P < 0.05). In addition, team support was also found to be a significant mediator between uncertainty and decision satisfaction (b = 0.082, P < 0.05) as well as between time pressure and decision satisfaction (b = 0.086, P < 0.05). Conclusion. Team support acts as a bridge between uncertainty and decision satisfaction and also between time pressure and decision satisfaction, underscoring its critical role in provider perceptions of decision making in the Obs and Gynae context. Implications. This study highlights the significance of managing stress, enhancing team support, and giving priority to patient-centered care. These findings provide insights into risk and uncertainty management in medical decision making, advancing patient-centered care, and optimizing health care outcomes.HighlightsStressors in hospital settings such as the complexity and uncertainty of tasks create stress among physicians, potentially leading to decision conflicts.Team support plays a fundamental role in mitigating the negative effects of stressors such as time pressure and uncertainty.Implementing stress management and team support interventions such as cognitive-behavioral therapy and mindfulness may enhance decision making among Obs and Gynae physicians.

Project description:BACKGROUND:Ground golf is a popular sport among the elderly in Japan. Several types of exercise can reduce the body's mean arterial pressure (MAP), but little is known about how ground golf affects the MAP. We investigated the effects of ground golf on the MAP and the oxygen uptake ([Formula: see text]) in a healthy elderly population. PARTICIPANTS AND METHODS:Thirteen elderly Japanese people (3 males and 10 females, mean age of 66 years) participated. All participants played 8 holes of ground golf 6 times, as game (G)1 to G6. The MAP, heart rate (HR), and [Formula: see text] were measured at rest and every 5 min during each game. RESULTS:A linear trend analysis revealed that participants' MAP values progressively decreased as each game proceeded with marginal differences (p = 0.054). There were no significant differences in HR between at rest and any of the games. The [Formula: see text] during the games (except for G6) were significantly higher than that at-rest (p < 0.05). The resting MAP values were negatively associated with the ground golf-induced changes in MAP (r = 0.786, p = 0.001). The participants with greater changes in [Formula: see text] during the games showed significantly greater reductions in MAP (r = 0.276, p = 0.043). CONCLUSIONS:Playing ground golf reduced the participants' MAP and increased their [Formula: see text]. Participants with higher resting MAP experienced greater reductions in MAP by playing ground golf, which suggests that ground golf can be a useful recreational sport for the elderly.

Project description:Luminance and contrast are two major attributes of objects in the visual scene. Luminance and contrast information received by visual neurons are often updated simultaneously. We examined the temporal response properties of neurons in the primary visual cortex (V1) to stimuli whose luminance and contrast were simultaneously changed by 50 Hz. We found that response tuning to luminance changes precedes tuning to contrast changes in V1. For most V1 neurons, the onset time of response tuning to luminance changes was shorter than that to contrast changes. Most neurons carried luminance information in the early response stage, while all neurons carried both contrast and luminance information in the late response stage. The early luminance response suggests that cortical processing for luminance is not as slow as previously thought.

Project description:BackgroundAt the start of the COVID-19 pandemic, guidance was needed more than ever to direct frontline healthcare and national containment strategies. Rigorous guidance based on robust research was compromised by the emergence of the pandemic and the urgency of need for guidance. Rather than aiming to "get guidance right", guidance developers needed to "get guidance right now".AimTo examine how guidance developers have responded to the need for credible guidance at the start of the COVID-19 pandemic.MethodsAn exploratory mixed-methods study was conducted among guidance developers. A web-based survey and follow-up interviews were used to examine the most pertinent challenges in developing COVID-19 guidance, strategies used to address these, and perspectives on the implications of the COVID-19 pandemic on future guidance development.ResultsThe survey was completed by 46 guidance developers. Survey findings showed that conventional methods of guidance development were largely unsuited for COVID-19 guidance, with 80% (n = 37) of respondents resorting to other methods. From the survey and five follow-up interviews, two themes were identified to bolster the credibility of guidance in a setting of extreme uncertainty: (1) strengthening end-user involvement and (2) conjoining evidence review and recommendation formulation. 70% (n = 32) of survey respondents foresaw possible changes in future guidance production, most notably shortening development time, by reconsidering how to balance between rigour and speed for different types of questions.Conclusion"Getting guidance right" and "getting guidance right now" are not opposites, rather uncertainties are always part of guidance development and require guidance developers to balance scientific robustness with usability, acceptability, adequacy and contingency. This crisis points to the need to acknowledge uncertainties of scientific evidence more explicitly and points to mechanisms to live with such uncertainty, thus extending guidance development methods and processes more widely.

Project description:The impact of the host immune environment on parasite transcription and fitness is currently unknown. It is widely held that hookworm infections have an immunomodulatory impact on the host, but whether the converse is true remains unclear. Immunity against adult-stage hookworms is largely mediated by Type 2 immune responses driven by the transcription factor Signal Transducer and Activator of Transcription 6 (STAT6). This study investigated whether serial passage of the rodent hookworm Nippostrongylus brasiliensis in STAT6-deficient mice (STAT6 KO) caused changes in parasites over time. After adaptation to STAT6 KO hosts, N. brasiliensis increased their reproductive output, feeding capacity, energy content, and body size. Using an improved N. brasiliensis genome, we found that these physiological changes corresponded with a dramatic shift in the transcriptional landscape, including increased expression of gene pathways associated with egg production, but a decrease in genes encoding neuropeptides, proteases, SCP/TAPS proteins, and transthyretin-like proteins; the latter three categories have been repeatedly observed in hookworm excreted/secreted proteins (ESPs) implicated in immunosuppression. Although transcriptional changes started to appear in the first generation of passage in STAT6 KO hosts for both immature and mature adult stages, downregulation of the genes putatively involved in immunosuppression was only observed after multiple generations in this immunodeficient environment. When STAT6 KO-adapted N. brasiliensis were reintroduced to a naive WT host after up to 26 generations, this progressive change in host-adaptation corresponded to increased production of inflammatory cytokines by the WT host. Surprisingly, however, this single exposure of STAT6 KO-adapted N. brasiliensis to WT hosts resulted in worms that were morphologically and transcriptionally indistinguishable from WT-adapted parasites. This work uncovers remarkable plasticity in the ability of hookworms to adapt to their hosts, which may present a general feature of parasitic nematodes.

Project description:We focus on exploratory decisions across disorders of compulsivity, a potential dimensional construct for the classification of mental disorders. Behaviors associated with the pathological use of alcohol or food, in alcohol use disorders (AUD) or binge-eating disorder (BED), suggest a disturbance in explore-exploit decision-making, whereby strategic exploratory decisions in an attempt to improve long-term outcomes may diminish in favor of more repetitive or exploitatory choices. We compare exploration vs exploitation across disorders of natural (obesity with and without BED) and drug rewards (AUD). We separately acquired resting state functional MRI data using a novel multi-echo planar imaging sequence and independent components analysis from healthy individuals to assess the neural correlates underlying exploration. Participants with AUD showed reduced exploratory behavior across gain and loss environments, leading to lower-yielding exploitatory choices. Obese subjects with and without BED did not differ from healthy volunteers but when compared with each other or to AUD subjects, BED had enhanced exploratory behaviors particularly in the loss domain. All subject groups had decreased exploration or greater uncertainty avoidance to losses compared with rewards. More exploratory decisions in the context of reward were associated with frontal polar and ventral striatal connectivity. For losses, exploration was associated with frontal polar and precuneus connectivity. We further implicate the relevance and dimensionality of constructs of compulsivity across disorders of both natural and drug rewards.

Project description:ObjectiveTo describe changes in atherosclerotic cardiovascular disease (ASCVD) risk over time among people living with HIV (PLHIV).MethodsWe used data from the TREAT Asia HIV Observational Database (TAHOD) and the Australian HIV Observational Database (AHOD). Five-year ASCVD risk was calculated using the D:A:D equation. Individuals were eligible for inclusion if they were aged ≥18 years, had started ART, had no previous history of ASCVD and had complete ASCVD risk factor data available within the first 5 years of ART initiation.ResultsA total of 3368 adults contributed data, 3221 were from TAHOD and 147 were from AHOD. The median age at ART initiation was 36 [IQR 31-43] years for TAHOD participants, and 42 [IQR 35-50] years for AHOD participants. Most TAHOD (70.4%) and AHOD (91.8%) participants were male. Overall, ASCVD risk increased from 0.84% (95% CI 0.81%-0.87%) at ART initiation to 1.34% (95% CI 1.29%-1.39%) after 5 years on ART. After adjusting for traditional and HIV-associated ASCVD risk factors, ASCVD risk increased at a similar rate among sub-populations defined by HIV exposure (heterosexuals, men who have sex with men, people who inject drugs), race/ethnicity (Caucasian and Asian) and nadir CD4 at ART initiation (<200 and ≥200 cells/mm3).ConclusionsThese findings emphasize the growing burden of ASCVD risk among PLHIV and the need to develop interventions that are effective across a broad range of HIV sub-populations.

Project description:ObjectiveAcute compartment syndrome (ACS) is a serious medical condition that can be encountered in tibial plateau fractures. However, no studies of compartment pressure changes in patients with tibial plateau fractures compared to patient without fractures have been reported. To obtain a comprehensive understanding of the pressure changes in patients with fractures, we monitored and recorded the compartment pressure and attempted to reveal the potential pressure release function of the human fascia.Materials and methodsCohorts of 43 normal individuals and 23 patients (initial 33, 10 were excluded due to inclusion criteria) and include the number of patients who completed the study with closed tibial fractures (the fracture group, FG, which comprised 6 men and 17 women) were included in this retrospective research. Compartment pressures were measured with Icare, a device that is traditionally used to measure intraocular pressure. Results of measurements at 6 different locations in the lower limb were recorded and compared for three days (days 2, 3, and 4 post fracture) between normal cohort (CG) and fracture cohort (FG) patients.ResultsThe compartment pressures were comparable at each pressure measurement site (upper, middle and lower) in patients of the CG and the FG. Compared with the CG patients, there was a significant increase in compartment pressure at the upper lateral location in 18-45-year-old patients in the FG (P = 0.013) and at the upper lateral (P = 0.004) and medial locations (P = 0.005) in 46-69-year-old patients, and the values tended to normalize over time. Compared with the contralateral normal limb of patients in the FG, there was a significant increase in compartment pressure at the upper lateral location in 18-45-year-old patients (P = 0.009) and at the upper lateral (P = 0.015) and medial locations (P = 0.016) in 46-69-year-old patients on the fractured side. Based on different fracture classifications, there were no significant differences in compartment pressure at the medial (upper, middle and lower) locations when compared with pressures at the corresponding lateral sites of measurement.ConclusionThe results of this study revealed that the fascial compartment as a whole can release the increased intracompartment pressure after fracture to prevent complications such as acute compartment syndrome caused by a continued increase in pressure. The Icare as a portable device, is potentially useful in compartmental pressure measurement especially in emergency room.

Project description:ObjectiveThis study examined the application, feasibility, and validity of supervised learning models for text classification in appraisals for rare disease treatments (RDTs) in relation to uncertainty, and analyzed differences between appraisals based on the classification results.MethodsWe analyzed appraisals for RDTs (n = 94) published by the National Institute for Health and Care Excellence (NICE) between January 2011 and May 2023. We used Naïve Bayes, Lasso, and Support Vector Machine models in a binary text classification task (classifying paragraphs as either referencing uncertainty in the evidence base or not). To illustrate the results, we tested hypotheses in relation to the appraisal guidance, advanced therapy medicinal product (ATMP) status, disease area, and age group.ResultsThe best performing (Lasso) model achieved 83.6 percent classification accuracy (sensitivity = 74.4 percent, specificity = 92.6 percent). Paragraphs classified as referencing uncertainty were significantly more likely to arise in highly specialized technology (HST) appraisals compared to appraisals from the technology appraisal (TA) guidance (adjusted odds ratio = 1.44, 95 percent CI 1.09, 1.90, p = 0.004). There was no significant association between paragraphs classified as referencing uncertainty and appraisals for ATMPs, non-oncology RDTs, and RDTs indicated for children only or adults and children. These results were robust to the threshold value used for classifying paragraphs but were sensitive to the choice of classification model.ConclusionUsing supervised learning models for text classification in NICE appraisals for RDTs is feasible, but the results of downstream analyses may be sensitive to the choice of classification model.
