Insights into Predictors of Vaccine Hesitancy and Promoting Factors in Childhood Immunization Programs-A Cross-Sectional Survey in Cameroon.

ABSTRACT: Vaccination is claimed to be a key intervention against the COVID-19 pandemic. A major challenge today is to increase vaccine acceptance as vaccine hesitancy has delayed the eradication of polio. This study aimed to identify predictors associated with vaccine acceptance in the context of the Expanded Program on Immunization among parents of children between the ages of 12 to 23 months in the Foumbot district, Cameroon. The design of this study is a cross-sectional survey. A total of 160 mothers of children between the ages of 12 to 23 months were selected using a simple random sampling technique. A pre-tested structured questionnaire was used for data collection. Incomplete vaccination status was considered "vaccine hesitancy". Data was analyzed along with 95% confidence intervals and the p-value < 0.05. The results showed 60% vaccine acceptance and 40% vaccine hesitancy. Factors such as age-appropriate vaccination, knowledge of vaccine-preventable diseases (VPD), and religion were associated with vaccine acceptance. Poor knowledge of VPDs is a matter of concern as it contributes to vaccine hesitancy. The study findings provide the basis to heighten health education, the public perceived threat of the VPDs, and the consequences if no measures are taken to ensure health.

Project description:Immunization is the most cost-effective health intervention in the world yet, vaccination uptake is still low with less than 50% of children aged 12-23 months fully vaccinated in Cameroon by 2018. The objective of this study was to estimate the burden of vaccine hesitancy associated with routine vaccines in Yaounde-Cameroon. A two-stage cross-sectional cluster survey was conducted in Yaoundé in November 2021, targeting parents/guardians of children 0-59 months. Clusters were selected with probability proportionate to size (PPS) and household's selection done using a restricted sampling method. Data collection was done using an interviewer-administered questionnaire, "Core Closed Questions" and "Likert Scale Questions" proposed by the WHO Vaccine Hesitancy Technical Working Group in 2014. Vaccine hesitancy was analyzed as proportions of parent's/guardian's self-reported vaccine refusal or delay in vaccination with 95% confidence interval. This was stratified by household wealth level and tested using Chi-Square test to appreciate the effect of household wealth on vaccine hesitancy. A total of 529 participants were enrolled out of 708 visited, giving a non-response rate of 25%. In total, vaccine hesitancy was reported in 137(25.90[22.35-29.80] %), and oral polio vaccine(OPV) was the most affected vaccine with hesitancy of 10%. Vaccine hesitancy prevalence did not vary significantly across different households' wealth levels (p-value = 0.3786). However, in wealthy households' refusal of vaccines (14%) was less than in poorer households (20%). Lack of trust was reported as the leading cause of vaccine refusal (43%). Vaccine hesitancy prevalence in Yaounde is high and oral polio vaccine(OPV) was the most affected vaccine. The level of weath does not affect vaccine hesitancy and lack of trust was the leading cause of vaccine hesitancy related to routine immunization in Yaounde-Cameroon. We, recommend that the burden of vaccine hesitancy should be assessed at national scale and root causes investigated.

Project description:Background: During the ongoing COVID-19 pandemic, trust within a community in the projected schemes or strategies to combat COVID-19 depends on the confidence generated and launched by the government and medical employees toward the public. The "vaccination intention" within a community is determined by a range of factors, which include sociodemographic features, personal beliefs, and attitude toward vaccination. Methods: A nationwide survey was conducted involving 2000 people using a Tencent questionnaire platform. One-way ANOVA was conducted for age, education, and occupation with vaccination intention for the COVID-19 vaccine. Correlation analysis was conducted between sources, trust, and vaccination intention indicating both types of sources (official and unofficial sources) and both types of trust (trust in the social environment and in vaccines). Results: The reception of the sources of information on the COVID-19 vaccine was significantly higher from official sources (M = 5.54, SD = 1.37) and government officials (M = 5.68, SD = 1.499) compared with that from experts in medicine (M = 5.39, SD = 1.511). Among the unofficial sources, "chatting and communicating with family and friends" scored the highest (M = 4.84, SD = 1.649). In the statistics on people's trust in all aspects involved in vaccines, the level of trust in the social environment was significantly higher and more concentrated than in vaccines (M = 5.67, SD = 1.129). The level of trust in government (M = 5.80, SD = 1.256) was slightly higher than in medical personnel (M = 5.53, SD = 1.199). People's willingness to be vaccinated was generally high (M = 78.15, SD = 22.354). The demographic factors were not influential in vaccination intention. Both sources (official and unofficial sources) and trust (trust in the social environment and in vaccines) are significantly and positively correlated with vaccination intention. Information receptions from official and unofficial sources were significant positive predictors of trust in the social environment, but they were not significant predictors of trust in vaccines. The mediating effect of trust in vaccines on the relationship between receiving information from official and unofficial sources and vaccination intention was insignificant. Conclusions: This study revealed that trust in the environment is an important channel linking people's information reception and vaccination intention, explores a new path for health information communication, and attempts to provide new ideas for health information dissemination and promotion.
