Project description:When considering airborne epidemic spreading in social systems, a natural connection arises between mobility and epidemic contacts. As individuals travel, possibilities to encounter new people either at the final destination or during the transportation process appear. Such contacts can lead to new contagion events. In fact, mobility has been a crucial target for early non-pharmaceutical containment measures against the recent COVID-19 pandemic, with a degree of intensity ranging from public transportation line closures to regional, city or even home confinements. Nonetheless, quantitative knowledge on the relationship between mobility-contagions and, consequently, on the efficiency of containment measures remains elusive. Here we introduce an agent-based model with a simple interaction between mobility and contacts. Despite its simplicity, our model shows the emergence of a critical mobility level, inducing major outbreaks when surpassed. We explore the interplay between mobility restrictions and the infection in recent intervention policies seen across many countries, and how interventions in the form of closures triggered by incidence rates can guide the epidemic into an oscillatory regime with recurrent waves. We consider how the different interventions impact societal well-being, the economy and the population. Finally, we propose a mitigation framework based on the critical nature of mobility in an epidemic, able to suppress incidence and oscillations at will, preventing extreme incidence peaks with potential to saturate health care resources.

Project description:There is evidence that adoption of non-pharmaceutical containment measures (NPMs) may have had a major impact on Covid-19 epidemic dynamics, and mitigated its effect on healthcare system. Optimal timing of implementation of these measures however is not known. In Italy, a national lockdown was decided on March 11th 2020 and ended 4th of May. At that time, cumulative incidence (CI) was different in Italian regions which ranged from <5 cases/100,000 to >11 cases/100,000 inhabitants. In this paper, we aim to evaluate how level of incidence in different regions at the time of implementation of NPMs affected CI and had an impact on the healthcare system in terms of ICU bed occupancy and mortality rates. We used regional daily new COVID-19 diagnosed cases as well number of people hospitalized in ICU and number of deaths for period February 24-May 11 from all the 19 Italian regions and two autonomous provinces. For each region we calculated: temporal daily trend of cumulative cases of Covid-19/100,000 inhabitants, daily trend of ICU bed occupancy and mortality rate at the end of period. We found that the epidemic curves show similar trends for all regions and all tend to flatten between 11-32 days. However, after 2 months, regions with lower CI at lockdown remained at substantially lower CI (<265 cases/100,000), had a peak of percentage of cases hospitalized in ICU which did not exceed 79.4% and a mortality<0.27/1,000. On the other hand, in regions with higher incidence at lockdown, CI reached 382-921 cases/100,000, the peak of percentage of cases hospitalized in ICU and mortality rate reached 270%, and 1.5/1,000, respectively. Our data suggests that level of CI at the moment of lockdown is important to control the subsequent spread of infection so NPMs should be adopted very early during the course of Covid-19 epidemic, in order to mitigate the impact on the healthcare system and to reduce related mortality.

Project description:The recent outbreak of coronavirus disease 2019 (COVID-19) in mainland China was characterized by a distinctive subexponential increase of confirmed cases during the early phase of the epidemic, contrasting with an initial exponential growth expected for an unconstrained outbreak. We show that this effect can be explained as a direct consequence of containment policies that effectively deplete the susceptible population. To this end, we introduce a parsimonious model that captures both quarantine of symptomatic infected individuals, as well as population-wide isolation practices in response to containment policies or behavioral changes, and show that the model captures the observed growth behavior accurately. The insights provided here may aid the careful implementation of containment strategies for ongoing secondary outbreaks of COVID-19 or similar future outbreaks of other emergent infectious diseases.

Project description:As of writing this paper, COVID-19 (Coronavirus disease 2019) has spread to more than 220 countries and territories. Following the outbreak, the pandemic's seriousness has made people more active on social media, especially on the microblogging platforms such as Twitter and Weibo. The pandemic-specific discourse has remained on-trend on these platforms for months now. Previous studies have confirmed the contributions of such socially generated conversations towards situational awareness of crisis events. The early forecasts of cases are essential to authorities to estimate the requirements of resources needed to cope with the outgrowths of the virus. Therefore, this study attempts to incorporate the public discourse in the design of forecasting models particularly targeted for the steep-hill region of an ongoing wave. We propose a sentiment-involved topic-based latent variables search methodology for designing forecasting models from publicly available Twitter conversations. As a use case, we implement the proposed methodology on Australian COVID-19 daily cases and Twitter conversations generated within the country. Experimental results: (i) show the presence of latent social media variables that Granger-cause the daily COVID-19 confirmed cases, and (ii) confirm that those variables offer additional prediction capability to forecasting models. Further, the results show that the inclusion of social media variables introduces 48.83%-51.38% improvements on RMSE over the baseline models. We also release the large-scale COVID-19 specific geotagged global tweets dataset, MegaGeoCOV, to the public anticipating that the geotagged data of this scale would aid in understanding the conversational dynamics of the pandemic through other spatial and temporal contexts.

Project description:Background: COVID-19 seriously hit Myanmar between June and August 2021, a few months after the military coup, though the first and second waves in 2020 were managed effectively by the government. People in Myanmar experienced serious consequences of the COVID-19 pandemic precipitated by the disorganized health system under the military junta. This study aimed to analyse the situation of COVID-19 occurrence and death proportions during its third wave in Myanmar. Methods: An online survey was conducted using a Google form. People with the symptoms of COVID-19 and those who died from COVID-19 between June and August 2021 were eligible to participate. The Google form was extracted into an Excel datasheet and analysed using Stata v16.1. Results: Among the 29,171 participants, 76.7% were over 30 years old and 56.4% were female. A majority of participants were from highly populated regions: Yangon (17,220; 59%) (Business capital), Mandalay (3,740; 12.8%) and Sagaing (1,546; 5.3%). Participants sought health care from telegram/other online services (34%), home care by health care providers (22%), private clinics (13%) and public hospitals run under the military junta (5%). Overall, 15% of participants died, of which, 72% occurred at home and 17% at public hospitals. Significantly higher proportions of deaths were seen among participants over 60 years than other age groups and males (p<0.001). Death proportions at different weeks from June to August 2021 ranged from 12.4% to 17.3%, much higher than the military junta's reports. Overall, 25% of participants received oxygen therapy. Conclusions: Death proportions in different weeks were consistently over 12%. The majority of participants received tele/online and home treatment services. Most deaths occurred at home. Findings indicated the high COVID-19 case fatality rates with limited access to public hospital care during the third wave. The data from this study suggest that the outcomes were adversely impacted by the military coup.

Project description:During the COVID-19 pandemic, several countries have resorted to self-adaptive mechanisms that tailor non-pharmaceutical interventions to local epidemiological and health care indicators. These mechanisms reinforce the mutual influence between containment measures and the evolution of the epidemic. To account for such interplay, we develop an epidemiological model that embeds an algorithm mimicking the self-adaptive policy mechanism effective in Italy between November 2020 and March 2022. This extension is key to tracking the historical evolution of health outcomes and restrictions in Italy. Focusing on the epidemic wave that started in mid-2021 after the diffusion of Delta, we compare the functioning of alternative mechanisms to show how the policy framework may affect the trade-off between health outcomes and the restrictiveness of mitigation measures. Mechanisms based on the reproduction number are generally highly responsive to early signs of a surging wave but entail severe restrictions. The emerging trade-off varies considerably depending on specific conditions (e.g., vaccination coverage), with less-reactive mechanisms (e.g., those based on occupancy rates) becoming more appealing in favorable contexts.

Project description:Modelling how a pandemic is spreading over time is a challenging issue. The new coronavirus disease called COVID-19 does not escape this rule as it has embraced over two hundred countries. As for previous pandemics, several studies have attempted to model the occurrence of cases caused by COVID-19. However, no study has succeeded in accurately modelling the impact of the infectious agent. Here we show that COVID-19 daily case distribution in humans obeys a Gamma law, which two new parameters can describe without any adjustment. Though the Gamma law has been exploited for nearly two centuries to describe the statistical distribution of spatial or temporal quantities, the goodness-of-fit rationale using two or three parameters has remained enigmatic. The new Gamma law approach we demonstrate here emerges from actual data and sheds light on the underlying mechanisms of the observed phenomenon. This finding has promising applicability in the epidemiological domain and in all disciplines involving branching systems, for which our Gamma law approach may bring a solution to hitherto unsolved problems.

Project description:Some occupational sectors, such as human health and care, food service, cultural and sport activities, have been associated with a higher risk of SARS-CoV-2 infection than other sectors. To curb the spread of SARS-CoV-2, it is preferable to apply targeted non-pharmaceutical interventions on selected economic sectors, rather than a full lockdown. However, the effect of these general and sector-specific interventions on the virus circulation has only been sparsely studied. We assess the COVID-19 incidence under different levels of non-pharmaceutical interventions per economic activity during the autumn 2020 wave in Belgium. The 14-day incidence of confirmed COVID-19 cases per the Statistical Classification of Economic Activities in the European Community (NACE-BEL) sector is modelled by a longitudinal Gaussian-Gaussian two-stage approach. This is based on exhaustive data on all employees in all sectors. In the presence of sanitary protocols and minimal non-pharmaceutical interventions, many sectors with close contact with others show considerably higher COVID-19 14-day incidences than other sectors. The effect of stricter non-pharmaceutical interventions in the general population and non-essential sectors is seen in the timing of the peak incidence and the width and height of the post-peak incidence. In most sectors incidences returned to higher levels after the peak than before and this decrease took longer for the health and care sector. Sanitary protocols for close proximity occupations may be sufficient during periods of low-level virus circulation, but progressively less with increasing circulation. Stricter general and sector-specific non-pharmaceutical interventions adequately decrease COVID-19 incidences, even in close proximity in essential sectors under solely sanitary protocols.

Project description:The spread of coronavirus disease 2019 (COVID-19) in Italy prompted drastic measures for transmission containment. We examine the effects of these interventions, based on modeling of the unfolding epidemic. We test modeling options of the spatially explicit type, suggested by the wave of infections spreading from the initial foci to the rest of Italy. We estimate parameters of a metacommunity Susceptible-Exposed-Infected-Recovered (SEIR)-like transmission model that includes a network of 107 provinces connected by mobility at high resolution, and the critical contribution of presymptomatic and asymptomatic transmission. We estimate a generalized reproduction number ([Formula: see text] = 3.60 [3.49 to 3.84]), the spectral radius of a suitable next-generation matrix that measures the potential spread in the absence of containment interventions. The model includes the implementation of progressive restrictions after the first case confirmed in Italy (February 21, 2020) and runs until March 25, 2020. We account for uncertainty in epidemiological reporting, and time dependence of human mobility matrices and awareness-dependent exposure probabilities. We draw scenarios of different containment measures and their impact. Results suggest that the sequence of restrictions posed to mobility and human-to-human interactions have reduced transmission by 45% (42 to 49%). Averted hospitalizations are measured by running scenarios obtained by selectively relaxing the imposed restrictions and total about 200,000 individuals (as of March 25, 2020). Although a number of assumptions need to be reexamined, like age structure in social mixing patterns and in the distribution of mobility, hospitalization, and fatality, we conclude that verifiable evidence exists to support the planning of emergency measures.

Project description:BackgroundMyanmar lies in the Greater Mekong Subregion where there is artemisinin-resistant Plasmodium falciparum malaria. As the artemisinin compound is the pillar of effective antimalarial therapies, containing the spread of artemisinin resistance is a national and global priority. The use of insecticide-treated bed nets/long-lasting insecticidal nets (ITNs/LLINs) is the key intervention for ensuring the reduction of malaria transmission and the spread of resistant strains, and for eventually eliminating malaria. This study aimed at assessing household ownership of, access to, and utilization of bed nets in areas of Myanmar with and without artemisinin resistance containment measures.MethodsSecondary data from a nationwide community-based malaria survey conducted by the National Malaria Control Program in 2014 were analyzed. Based on evidence of artemisinin resistance, Myanmar was divided into tiers 1, 2, and 3: townships in tiers 1 and 2 were aggregated as the Myanmar Artemisinin Resistance Containment (MARC) areas and were compared with tier 3 townships, which were defined as non-MARC areas. The chi-square test was used to compare groups, and the level of significance was set at P ≤ 0.05.ResultsOf the 6328 households assessed, 97.2% in both MARC and non-MARC areas had at least one bed net (any type), but only 63% of households had ITNs/LLINs. Only 44% of households in MARC areas and 24% in non-MARC areas had adequate numbers of ITNs/LLINs (one ITN/LLIN per two persons, P < 0.001). Nearly 44% of household members had access to ITNs/LLINs. Regarding the utilization of ITNs/LLINs, 45% of household members used them in MARC areas and 36% used them in non-MARC areas (P < 0.001, desired target = 100%). Utilization of ITNs/LLINs among children aged below five years and pregnant women (high malaria risk groups) was low, at 44% and 42%, respectively.ConclusionsThis study highlights the nationwide shortfalls in the ownership of, access to, and utilization of ITNs/LLINs in Myanmar, which is of particular concern in terms of containing the spread of artemisinin resistance. It highlights the need for priority attention to be paid and mobilization of resources in order to improve bed net coverage and utilization through bed net distribution and/or social marketing, information dissemination, and awareness-raising.
