What Can Make the Difference Between Chronotypes in Sleep Duration? Testing the Similarity of Their Homeostatic Processes.

ABSTRACT: The two-process conceptualization of sleep-wake regulation suggests that the biological underpinnings of the differences between morning and evening types in sleep timing and duration might be related to either the circadian process or the homeostatic process or both. The purpose of this report was to test whether morning and evening types might have similar homeostatic processes to achieve such ultimate goal of homeostatic sleep regulation as taking an adequate amount of sleep on free days. Weekend and weekday rise- and bedtimes reported for 50 paired samples of morning and evening types were averaged and simulated with a model of sleep-wake regulation. In morning and evening types of the same age, the homeostatic components of the sleep-wake regulation were found to be identical. Therefore, the difference in the circadian process between chronotypes of similar age can account for the observed differences between them in sleep timing and duration on weekdays and weekends. It was also demonstrated that the model-based simulations might have practical implications for informing an individual about the extent of unrecoverable reduction of his/her sleep on weekdays.

Project description:RationaleHome sleep apnea testing (HSAT) typically does not include electroencephalogram (EEG) monitoring for sleep assessment. In patients with insomnia and low sleep efficiency, overestimation of the sleep period can result from absence of EEG, which will reduce sleep disordered breathing (SDB) indices and may lead to a false-negative result.ObjectiveTo validate a single channel frontal EEG for scoring sleep versus wake against full EEG during polysomnography, and then to examine the utility of adding this single channel EEG to standard HSAT to prevent false-negative results.MethodsEpoch-by-epoch validation for sleep scoring of single channel EEG versus full PSG was first performed in 21 subjects. This was followed by a separate retrospective analysis of 207 consecutive HSATs in adults performed in a university-affiliated sleep center using the Somte (Compumedics) HSAT with one frontal EEG as well as chin EMG, nasal airflow, oxyhemoglobin saturation, respiratory effort, pulse rate, and body position. Each study was scored twice, with (HSATEEG) and without the EEG signal visible (HSATPolygraphy), to calculate AHI4 and RDI and the effect on OSA diagnosis and severity. Analyses were repeated in 69 patients with poor sleep suggesting insomnia plus Epworth Sleepiness Scale < 7 as well as in 38 patients ultimately shown to have sleep efficiency < 70% on HSAT with EEG.Measurements and main resultsSingle channel and full EEG during polysomnography agreed on sleep versus wake in 92-95% of all epochs. HSAT without EEG overestimated the sleep period by 20% (VST = 440 ± 76 min vs TST = 356 ± 82 min), had a false-negative rate of 8% by AHI4 criteria, and underestimated disease severity in 11% of all patients. Sub-group analysis of patients with subjective poor sleep suggesting insomnia did not change the results. Patients later shown to have low sleep efficiency had lower SDB indices and a 20.8% false negative rate of sleep apnea diagnosis.ConclusionsAlthough overall false negative rates using HSATPolygraphy were moderate, suggesting utility for ruling out OSA, there was a specific subgroup in whom there were significant missed diagnoses. However, we were unable to identify this subgroup a priori.

Project description:ImportanceAn association between sleep duration and the trajectory of cognitive decline has not been conclusively demonstrated.ObjectiveTo investigate the association between sleep duration and cognitive decline by a pooled analysis of 2 nationally representative aging cohorts.Design, setting, and participantsA pooled cohort study using data from waves 4 to 8 (2008-2009 to 2016-2017) in the English Longitudinal Study of Ageing and waves 1 to 3 (2011 to 2015) in the China Health and Retirement Longitudinal Study in a population-based setting. Participants were 2 randomly enrolled cohorts comprising 28 756 individuals living in England who were 50 years or older and those living in China who were 45 years or older.ExposureSelf-reported sleep duration per night according to face-to-face interviews.Main outcomes and measuresGlobal cognitive z scores were calculated according to immediate and delayed recall test, an animal fluency test, the serial sevens test, an intersecting pentagon copying test, and a date orientation test.ResultsData were analyzed from 20 065 participants, including 9254 from the English Longitudinal Study of Ageing (mean [SD] age, 64.6 [9.8] years; 55.9% [5174 of 9254] women; median follow-up duration, 8 [interquartile range, 6-8] years) and 10 811 from the China Health and Retirement Longitudinal Study (mean [SD] age, 57.8 [9.0] years; 50.2% [5425 of 10 811] men; median follow-up duration, 4 [interquartile range, 4-4] years). During 100 000 person-years of follow-up, global cognitive z scores in individuals with 4 hours or less (pooled β = -0.022; 95% CI, -0.035 to -0.009 SD per year; P = .001) and 10 hours or more (pooled β = -0.033; 95% CI, -0.054 to -0.011 SD per year; P = .003) of sleep per night declined faster than in the reference group (7 hours per night) after adjusting for a number of covariates. An inverted U-shaped association between sleep duration and global cognitive decline was also observed.Conclusions and relevanceIn this pooled cohort study, an inverted U-shaped association between sleep duration and global cognitive decline was found, indicating that cognitive function should be monitored in individuals with insufficient (≤4 hours per night) or excessive (≥10 hours per night) sleep duration. Future studies are needed to examine the mechanisms of the association between sleep duration and cognitive decline.
