Magnitude and determinants of intimate partner violence against women in East Africa: multilevel analysis of recent demographic and health survey.

ABSTRACT:

Background

Violence against women is a significant public health problem, and human rights abuse, and is associated with multiple adverse physical, mental, sexual, and reproductive health effects. The current study aimed to determine the magnitude of intimate partner violence (IPV) and its determinant factors in East African countries.

Methods

We utilized the most recent demographic and health survey data from 11 East African countries, which was comprised of a weighted sample of 55,501 ever-married women. A multilevel multivariable logistic regression analysis was applied. We used an adjusted odds ratio with a 95% CI and a p value ≤ 0.05 in the multilevel logistic model to declare significant factors associated with IPV.

Results

The overall prevalence of all forms of IPV in East African countries was 32.66% [95% CI 32.27, 33.05], with the highest IPV occurring in Uganda (14.93%) and the lowest IPV recorded in Comoros (0.87%). In the multivariable multilevel logistic regression model, women's education, residence, sex of household head, current pregnancy, husband drinking alcohol, attitude towards wife-beating husband controlling behavior, and women's decision-making autonomy were significantly associated with IPV.

Conclusion

The risk factors noted above increase the likelihood of a woman experiencing IPV. Therefore, we recommend establishing effective health and legal response services for IPV, raising awareness of the existing legislation service and improving its application, strengthening legislations on purchasing and selling of alcohol, strengthening joint (both husband and wife) decision-making power by empowering women, improving the educational level of women, and establishing measures to break the culture of societal tolerance towards IPV.

Project description:BackgroundGlobally, intimate partner violence (IPV) during pregnancy is the most common and major public health problem. It has a negative effect on the lives of both mother and fetus. Despite its prominence, many countries in East Africa have paid little attention to this issue. This study assessed the prevalence and associated factors of intimate partner violence among pregnant women in East African countries.MethodsThe study adopted a secondary method data analysis that utilized recent Demographic and Health Surveys of 10 countries in East Africa between 2012 and 2018. A total of 23,521 women who gave birth in the 5 years preceding the survey were included. A multilevel mixed-effect logistic regression model was fitted to identify factors associated with IPV. Variables with a p-value < 0.05 were declared as significant factors associated with IPV.ResultsThe overall prevalence of IPV in East Africa was 37.14 (95% CI 36.53, 37.76). Women with age 25-34 (AOR = 1.20;95%CI; 1.06, 1.36), 35-39 (AOR = 1.40;95%CI; 1.24, 1.58), and 40-49 (AOR = 1.66;95%CI; 1.43, 1.95), women with no education (AOR = 1.27;95%CI; 1.16, 1.39), women with no occupation (AOR = 1.36; 95%CI; 1.27, 1.47), women from households with the poorest (AOR = 1.51; 95%CI: 1.33, 1.71), poorer (AOR = 1.40;95% CI:1.24, 1.58), middle (AOR = 1.32;95%CI:1.17, 1.48), and richer (AOR = 1.26;95%CI: 1.13, 1.40), husband drinks alcohol (AOR = 2.54; 95%CI 2.39, 2.71), ≥ 5 number of living children (AOR = 1.28; 95%CI: 1.31, 2.57) and rural areas (AOR = 1.14; 95%CI: 1.03, 1.25) were significantly associated with IPV.ConclusionMore than one-third of pregnant women experienced intimate partner violence in East Africa. Promoting the educational status of women, the economic capacity of women, and the healthy behavior of the husband by reducing alcohol consumption, with particular attention to rural women and violence during pregnancy, is vital to reduce the prevalence of IPV.

Project description:ObjectivesIntimate partner violence (IPV) threatens women's health and safety. Support services can mitigate the impact, yet few survivors seek services in part due to social norms that discourage use. Little agreement exists on how to measure norms and attitudes related to IPV help-seeking. The objectives were to (1) refine an IPV Help-Seeking Attitudes Scale and examine its psychometric properties, (2) explore differences in attitudes between young men and young women, and (3) examine associations of past 6-month IPV among young women with the scale at the individual level (individual attitudes) and the by-gender community-aggregated averages (community norms) among youth in Nairobi, Kenya.DesignThis cross-sectional, secondary analysis used data from a phone-based survey with a cohort of young men and young women recruited via respondent-driven sampling from April to May 2021. Cross-sectional exploratory factor analysis assessed underlying latent constructs. Multilevel mixed-effects models assessed associations with IPV experience for young women.Setting and participantsA youth cohort of 586 men and 591 women aged 15-24 years in Nairobi, Kenya.Primary outcome measurePast 6-month physical and/or sexual IPV among young women.ResultsThe IPV Help-Seeking Attitudes Scale had acceptable internal reliability (Cronbach's alpha >0.60). IPV prevalence was 17.5%, among whom 21.7% had sought any help for the past 6-month IPV. A one-unit increase in the community aggregate IPV Help-Seeking Attitudes Scale among young women was associated with reduced odds of IPV (aOR: 0.17, 95% CI: 0.03-0.98). However, the individual-level attitudes scale was not associated with IPV nor was the men's community aggregate scale.ConclusionsThe IPV Help-Seeking Attitudes Scale had adequate psychometric properties. The results suggest that young women living in a community where the norm among women enables IPV response may have a reduced risk of IPV. Community norms change to better enable IPV response among young people may help reduce IPV and increase help-seeking.

Project description:IntroductionIntimate partner sexual violence (IPSV) is sexual violence against women by a current or former sexual partner. Sexual violence among pregnant women is higher in low-income countries, particularly in sub-Saharan Africa (SSA), which has numerous negative impacts for both the mother and the growing fetus. There is no recent and distinct information on the magnitude and factors associated with IPSV among pregnant women in SSA.ObjectiveThis study aimed to assess the prevalence of IPSV and its determinants among pregnant women in SSA.DesignA cross-sectional study design using demographic health survey data from 2015 to 2022 in 20 sub-Saharan African countries was used.MethodsThe analysis was conducted using a weighted sample of 10,775 women who were pregnant during the survey period (2015-2022). A multilevel logistic regression model was used to identify the determinants of IPSV among pregnant women. The adjusted odds ratio at 95% confidence interval (95% CI) was computed to assess the strength and significance of the association between explanatory and outcome variables. Factors with a p-value of <0.05 are declared statistically significant.ResultIn this study, the pooled prevalence of IPSV among pregnant women in SSA was 11.06% (95% CI: 10.48-11.67). Respondents currently working, male-headed households, presence of justified beating, respondent mother's history of abuse, respondent having no media access, respondent having self-reported sexually transmitted diseases (STIs), partner alcohol use, and residing in East Africa had increased odds of IPSV during pregnancy. On the other hand, nulliparous respondents, respondents involved in healthcare decision-making, households with a high wealth index, and residents of West Africa had decreased odds of IPSV among pregnant women.ConclusionThis study revealed that a significant proportion (11%) of pregnant women experienced intimate partner sexual violence during pregnancy in SSA. Hence, the government, policymakers, and other relevant authorities should give special attention to women's at-risk of sexual violence, including women who live in households with justified beatings, women with STIs, working women's, women with drunken partners, and non-autonomous women's. Prioritization of the east African region, increasing media access, and promoting a healthy lifestyle in the community shall be done to reduce the burden of the problem.

Project description:IntroductionThe dynamics of intimate partner violence (IPV)-one of the world's leading public health problems-in urban Africa remain poorly understood. Yet, urban areas are key to the future of women's health in Africa.Study objectivesWe explored survivor-, partner-, and household-level correlates of prevalence rates for types of IPV in urban SSA women.MethodThe study uses DHS data from 42,143 urban women aged 15-49 in 27 SSA countries. Associations at the bivariate level were examined using the Pearson Chi-square test. The modified Poisson regression test estimated the relative prevalence of IPV subtypes in the study population at the multivariate level.ResultsApproximately 36% of women in urban SSA experienced at least one form of IPV; 12.8% experienced two types; and 4.6% experienced all three types. SSA urban women who had only primary-level education, had 3 or more living children, were informally employed, were in polygynous unions, or who approved of wife-beating similarly displayed higher adjusted prevalence rates for all three forms of IPV compared respectively to their counterparts without formal education, without a living child, were unemployed, in monogamous unions, or who do not approve of wife-beating. On the other hand, the region's urban women who began cohabiting between ages 25 and 35 years or who lived in higher wealth households showed consistently lower adjusted prevalence rates for all three forms of IPV relative to their counterparts who began cohabiting before 18 years or who lived in lower wealth households. Compared to their counterparts without formal education, without a living child, or whose partners did not have formal education, women with secondary and higher education, with 1-2 living children, or whose partners had only primary level schooling displayed higher adjusted prevalence rates for both IPEV and IPPV, but not for IPSV. However, relative to their counterparts whose partners were aged 25 years or below, living with a partner aged 40 years and above was associated with statistically significant reduced prevalence rates for IPPV and IPSV, but not for IPEV. Only for IPPV did women with partners educated at secondary and above levels display statistically significant higher adjusted prevalence rates relative to their counterparts with uneducated partners. Also, solely for IPPV did women who began cohabiting between ages 18 and 24 years or whose partners were employed (whether formally or informally) show decreased adjusted prevalence rates relative to their counterparts who started cohabiting before 18 years or whose partners were unemployed. In addition, only for IPSV did women aged 40 years and above or living in middle wealth households show statistically significant reduced adjusted prevalence rates relative to their counterparts aged less than 25 years or living in lower wealth households.Discussion and conclusionBy 2030, the majority of SSA women will be urban dwellers. Complexities surround IPV in urban SSA, highlighting the unique dynamics of the problem in this setting. While affirming the link between IPV and marital power inequities and dynamics, findings suggest that the specific correlates of prevalence rates for different IPV sub-types in urban SSA women can, at once, be both similar and unique. The contextual drivers of the differences and similarities in the correlates of the prevalence rates of IPV sub-types among the region's urban women need further interrogation.
