Glandular Odontogenic Cyst Associated with Odontoma: A Rare Case Report.

ABSTRACT:

Background

Glandular odontogenic cyst (GOC) is an extremely rare developmental jawbone cyst, tending to recurrence owing to its aggressive behavior. There has been no reported case of presence of GOC simultaneous with odontoma. We presented a case of GOC associated with odontoma with special reference to the diagnostic imaging and the histopathological features.

Case presentation

A 42-year-old Chinese man presented with swelling and pain in the anterior mandible for the past 3 months. Panoramic scan showed a large multiocular well-circumscribed radiolucency accompanied by toothlike morphological abnormality. Histological findings confirmed a GOC associated with odontoma.

Conclusion

This case demonstrates GOC with multiple clinical spectrum, and its association with odontoma might enhance the existing knowledge of this rare jawbone cyst.

Project description:A glandular odontogenic cyst (GOC) is a developmental cyst that is a clinically rare and histopathologically unusual type of odontogenic cyst. GOCs are now relatively well-known entities; their importance relates to the fact that they exhibit a propensity for recurrence rates from 21% to 55%, similar to odontogenic keratocysts, and may be confused microscopically with central mucoepidermoid carcinoma. Furthermore, some microscopic features of GOCs may also be found in dentigerous, botryoid, radicular and surgical ciliated cysts. The present case report aims to describe a typical case of GOC, throwing light on its epidemiology and origin, as well as on its clinical, radiographic and microscopic features, which may be helpful for diagnosis in problematic cases, long-term follow-up and to determine the most appropriate treatment.

Multiple chromoanasynthesis in a rare case of sporadic renal leiomyosarcoma: case report

Project description:Renal leiomyosarcomas (LMS) are extremely rare neoplasms with aggressive behaviour and poor survival prognosis. The most frequent somatic events in leiomyosarcomas are mutations in TP53, RB1, ATRX and PTEN genes, chromosomal instability and chromoanagenesis. By using chromosomal microarray analysis we identified monosomy of chromosomes 3 and 11, gain of Xp (ATRX) arm and three chromoanasynthesis regions (6q21-q27, 7p22.3-p12.1 and 12q13.11-q21.2), with MDM2 and CDK4 oncogenes copy number gains, whereas no CNVs or tumor specific SNVs in TP53, RB1 and PTEN genes were observed.

Giant odontoma: A systematic scoping review and case report.

Project description:Introduction and importanceThe most common type of odontogenic tumor is odontoma. Cases with at least one dimension (sagittal, axial, or coronal) ≥30 mm were categorized as giant odontomas. This study aimed to provide a scoping review of giant odontoma and present a case report.Case presentationA 6 years-old boy attended for a routine dental examination without any symptoms. A slight swelling of the right buccal cortex of the posterior maxilla was detected. Radiography views revealed a mixed radiolucent-radiopaque lesion with multiple tooth-like structures. The lesion caused displacement teeth and elevated the floor of the maxillary sinus. The tumor was thoroughly enucleated and the dimensions of the tumor were measured as 32.4 mm × 22.2 mm × 15.3 mm. The specimen was histopathologically examined and was diagnosed as a giant compound odontoma.Clinical discussionThis scoping review indicated that the mean age of patients diagnosed with giant odontomas was 18.73 years, with no difference between the sexes. The posterior jaws were the most prevalent regions of incidence. The most common clinical manifestations are swelling, pain, and a sensation of hardness on palpation. Giant odontomas are usually diagnosed with panoramic radiography, cone-beam computed tomography, and histopathological examinations. Surgical enucleation is the treatment approach for this tumor.ConclusionThe distinction between giant odontoma and non-giant odontoma is solely based on tumor size. Although giant odontomas are benign, their large size necessitates specific precautions for diagnosis and management. Proper treatment involves critical decisions concerning surgical settings, bone grafting techniques, and jaw stabilization methods.

Tailgut cyst, management of a rare perianal mass: a case report.

Project description:BackgroundA perianal mass has a wide range of possible diagnoses, including tailgut cysts. Tailgut cysts are congenital, and their development is slow. Because its most common placement is retro-rectal, it is extremely rare to find it on the perianal level only. It is possible to develop neoplastic or infectious problems. Consequently, proper support is required.Case descriptionWe present a case of perianal tailgut cyst in a 59-year-old man who sought help for painless anal discomfort. Nuclear magnetic resonance was used to conduct this assessment. The radiologist thought it was probably benign, but he was not able to find a precise diagnosis. Thus, the cyst was completely removed using a transanal way with the patient in a lithotomy position. Then, the anatomopathological examination had confirmed the diagnosis and the benignity of the cyst. The postoperative follow-up was uneventful. We had planned to see the patient for a clinical follow-up every six months, but he has refused so far.ConclusionsTailgut cysts are uncommon in the perianal area but should be included in the differential diagnosis of perianal masses and should not be confused with an abscess or a haemorrhoid. The nuclear magnetic resonance appears to be the most appropriate radiological examination for the assessment of these masses. Excision is performed systematically to confirm the diagnosis and exclude a malignant condition thanks to the anatomopathological examination.

A Rare Presentation of Pancreatic Lymphoepithelial Cyst: A Case Report and Review.

Project description:Pancreatic lymphoepithelial cyst (LEC) is a rare, benign collection of keratinizing squamous epithelial cells encapsulated by lymphoid tissue. Because of its limited data and nonspecific features that can mimic malignant lesions, LECs can lead to unnecessary operations. A 62-year-old male with a known pancreatic mass presented with abdominal pain. CT scan showed an increased mass in the pancreatic head, and endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) revealed "rare fragments of benign-appearing squamous epithelium in a background of keratin debris, cyst contents, and scattered lymphocytes," consistent with a lymphoepithelial cyst. Pancreatic LEC is an extremely rare lesion that comprises of only 0.5% of all pancreatic cysts. EUS-FNA has become the mainstay for diagnosing pancreatic LECs. Given the slow growing and benign nature, conservative management and observation is adequate for pancreatic LECs with excellent long-term outcome. With increasing number of imaging ordered by clinicians, it is anticipated that there will be a greater number of incidental pancreatic LECs detected. Thus, EUS-FNA should be utilized more frequently to help distinguish benign pancreatic LECs from premalignant or malignant lesions to avoid surgery.

Rare giant complex composite odontoma of mandible in mixed dentition: Case report with 3-year follow-up and literature review.

Project description:Introductionand importance: Complex odontomas are hamartomas representing a conglomeration of all dental tissues. When developed, they rarely become giant and even more rare to become giant in children. This report presented an unusual giant mandibular complex odontoma in very young patient. It also provided a literature analysis and better understanding of clinical features of such giant lesions in the mandible.Case presentationAn 8-year-old boy presented with swelling in the right side of the mandible. After clinical and radiological exams, a provisional diagnosis of giant composite odontoma was made. This lesion led to jaw expansion, asymmetrical face, and teeth eruption alterations. Surgical excision of the lesion was performed, and histopathological examination confirmed the diagnosis. The patient's follow-up showed that normal jaw size and facial symmetry was restored with no evidence of recurrence for 3 years.Clinical discussionA literature review of reported mandibular large odontomas was made. All published reported cases of giant odontoma in the mandible caused bone expansion with or without pain. According to the literature, this paper described the youngest patient with giant complex odontoma in the mandible. This was also the first reported case from Syria.ConclusionGiant complex composite odontoma of the mandible can develop at any age causing facial asymmetry, however, with good prognosis and predictable surgical treatment.

Case Report: A Primordial odontogenic tumor.

Project description:Introduction: Primordial odontogenic tumors are a rare recently described mixed odontogenic tumor composed histopathologically of dental papilla like tissue and enamel organ like tissue. Only nine cases have been documented worldwide and we are reporting the tenth case which is from Egypt. Clinical finding: A 2-year-old Egyptian boy that presented with an asymptomatic swelling of the mandible which appeared with multilocular radiolucency associated with an impacted developing tooth on a computerized tomography (CT) scan. Diagnoses, interventions, and outcomes: The lesion was excised and diagnosed as a primordial odontogenic tumor. The patient was followed up for two years with no recurrence. Conclusion: Differentiation of primordial odontogenic tumors from other odontogenic tumors, which resemble it histopathologically is crucial to avoid unnecessary aggressive treatment.

The Importance of Histopathological Examination to the Final Diagnosis of Peripheral Odontogenic Tumors: A Case Report of a Peripheral Odontoma.

Project description:A 30-year-old Caucasian man presented with an 18-month history of an asymptomatic calcified mass, located on the buccal side of the alveolar ridge. Medical records did not present any underlying conditions. On intraoral examination, the lesion was located on the right side of the maxilla, showing mucosal fenestration with mineralized tissue measuring approximately 1 cm in diameter. Radiographic examination showed multiple radiopaque masses. Incisional biopsy was performed, and histological analysis revealed a presence of enamel matrix, dentin, and cementum, resembling tooth-like structures. Surgical removal was offered after the diagnostic confirmation of peripheral odontoma, but the patient refused because of the asymptomatic nature of the lesion.
