Development of a New Ankle Joint Hybrid Assistive Limb.

ABSTRACT: Foot and ankle disabilities (foot drop) due to common peroneal nerve palsy and stroke negatively affect patients' ambulation and activities of daily living. We developed a novel robotics ankle hybrid assistive limb (HAL) for patients with foot drop due to common peroneal nerve palsy or stroke. The ankle HAL is a wearable exoskeleton-type robot that is used to train plantar and dorsiflexion and for voluntary assistive training of the ankle joint of patients with palsy using an actuator, which is placed on the lateral side of the ankle joint and detects bioelectrical signals from the tibialis anterior (TA) and gastrocnemius muscles. Voluntary ankle dorsiflexion training using the new ankle HAL was implemented in a patient with foot drop due to peroneal nerve palsy after lumbar surgery. The time required for ankle HAL training (from wearing to the end of training) was approximately 30 min per session. The muscle activities of the TA on the right were lower than those on the left before and after ankle HAL training. The electromyographic wave of muscle activities of the TA on the right was slightly clearer than that before ankle HAL training in the resting position immediately after ankle dorsiflexion. Voluntary ankle dorsiflexion training using the novel robotics ankle HAL was safe and had no adverse effect in a patient with foot drop due to peroneal nerve palsy.

Project description:IntroductionRobotic therapy has drawn attention in the rehabilitation field including home-based rehabilitation. A previous study has reported that home-based therapy could be more effective for increasing upper limb activity than facility-based therapy. The single-joint hybrid assistive limb (HAL-SJ) is an exoskeleton robot developed according to the interactive biofeedback theory, and several studies have shown its effectiveness for upper limb function in stroke patients. A study of home-based robotic therapy has shown to enhance rehabilitation effectiveness for stroke patient with a paretic upper limb. However, home-based therapy involving a HAL-SJ in stroke patients with paretic upper limbs has not been investigated. The present study aimed to investigate paretic upper limb activity and function with home-based robotic therapy involving a HAL-SJ in stroke patients.Materials and methodsA home-based robotic therapy program involving a HAL-SJ was performed for 30 min per session followed by standard therapy for 30 min per session, 2 times a week, for 4 weeks (i.e., completion of all 8 sessions involved 8 h of rehabilitation), at home. After the intervention, patients were followed up by telephone and home visits for 8 weeks. The paretic upper limb activity and function were assessed using the Motor Activity Log (MAL; amount of use (AOU)), arm triaxial accelerometry (laterality index (LI)), the Fugl-Meyer assessment (FMA), and the action research arm test (ARAT), at baseline and week 4 and week 12 after the start of training.ResultsThe study included 10 stroke patients (5 men; mean age, 61.1 ± 7.1 years). The AOU scores and LI significantly improved at week 4 after the start of training (p<0.05). However, no significant changes were observed in the LI at week 12 (p=0.161) and the FMA scores at both week 4 and week 12 (p=0.059 and p=0.083, respectively). The ARAT scores significantly improved at both week 4 and week 12 (p<0.05).ConclusionHome-based robotic therapy combined with conventional therapy could be a valuable approach for increasing paretic upper limb activity and maintaining paretic upper limb function in the chronic phase of stroke.

Project description:Shoulder elevation, defined here as arm raising, being essential for activities of daily living, dysfunctions represent a substantial burden in patients' lives. Owing to the complexity of the shoulder joint, the tightly coordinated muscular activity is a fundamental component, and neuromuscular impairments have devastating effects. A single-joint shoulder type version of the Hybrid Assistive Limb (HAL) allowing motion assistance based on the intention of the user via myoelectric activation has recently been developed, and its safety was demonstrated for shoulder rehabilitation. Yet, little is known about the physiological effects of the device. This study aims to monitor the changes in muscle activity and motion during shoulder HAL rehabilitation in several patients suffering from shoulder elevation dysfunction from cervical radicular origin. 8 patients (6 males, 2 females, mean age 62.4 ± 9.3 years old) with weakness of the deltoid muscle resulting from a damage to the C5 nerve root underwent HAL-assisted rehabilitation. We combined surface electromyography and three-dimensional motion capture to record muscular activity and kinematics. All participants showed functional recovery, with improvements in their Manual Muscle Testing (MMT) scores and range of motion (ROM). During training, HAL decreased the activity of deltoid and trapezius, significantly more for the latter, as well as the coactivation of both muscles. We also report a reduction of the characteristic shrugging compensatory motion which is an obstacle to functional recovery. This reduction was notably demonstrated by a stronger reliance on the deltoid rather than the trapezius, indicating a muscle coordination tending toward a pattern similar to healthy individuals. Altogether, the results of the evaluation of motion and muscular changes hint toward a functional recovery in acute, and chronic shoulder impairments from cervical radicular origin following shoulder HAL rehabilitation training and provide information on the physiological effect of the device.

Project description:IntroductionThe wearable cyborg Hybrid Assistive Limb (HAL) is a therapeutic exoskeletal device that provides voluntary gait assistance using kinematic/kinetic gait data and bioelectrical signals. By utilizing the gait data automatically measured by HAL, we are developing a system to analyze the wearer's gait during the intervention, unlike conventional evaluations that compare pre- and post-treatment gait test results. Despite the potential use of the gait data from the HAL's sensor information, there is still a lack of analysis using such gait data and knowledge of gait patterns during HAL use. This study aimed to cluster gait patterns into subgroups based on the gait data that the HAL automatically collected during treatment and to investigate their characteristics.MethodsGait data acquired by HAL, including ground reaction forces, joint angles, trunk angles, and HAL joint torques, were analyzed in individuals with progressive neuromuscular diseases. For each measured item, principal component analysis was applied to the gait time-series data to extract the features of the gait patterns, followed by hierarchical cluster analysis to generate subgroups based on the principal component scores. Bayesian regression analysis was conducted to identify the influence of the wearer's attributes on the clustered gait patterns.ResultsThe gait patterns of 13,710 gait cycles from 457 treatments among 48 individuals were divided into 5-10 clusters for each measured item. The clusters revealed a variety of gait patterns when wearing the HAL and identified the characteristics of multiple sub-group types. Bayesian regression models explained the influence of the wearer's disease type and gait ability on the distribution of gait patterns to subgroups.DiscussionThese results revealed key differences in gait patterns related to the wearer's condition, demonstrating the importance of monitoring HAL-assisted walking to provide appropriate interventions. Furthermore, our approach highlights the usefulness of the gait data that HAL automatically measures during the intervention. We anticipate that the HAL, designed as a therapeutic device, will expand its role as a data measurement device for analysis and evaluation that provides gait data simultaneously with interventions, creating a novel cybernics treatment system that facilitates a multi-faceted understanding of the wearer's gait.
