Building an Open Health Data Analytics Platform: a Case Study Examining Relationships and Trends in Seniority and Performance in Healthcare Providers.

ABSTRACT: With a movement toward an open data-driven approach, governments worldwide are releasing public data in an effort to increase transparency. Despite the availability of this data, many factors make it difficult for anyone to extract knowledge from it. The relationship between seniority and performance is a controversial issue in many fields; using an open and reproducible framework, we investigate the relationship in open healthcare data. Using data from the Center for Medicare and Medicaid Services covering 895,000 practitioners and 3000 hospitals, weak but statistically significant correlations between graduation year, a proxy for seniority, and the hospital value-based performance score were found in 29 of 74 specialties (Spearman rank correlation values < 0.164, p value < 0.05). This result represents 7% of US healthcare practitioners and over 75% of medical practitioners in several specialties. With 5 years of data (2009-2014) from the New York Statewide Planning and Research Cooperative System (SPARCS), we found weak but statistically significant correlations between graduation year and cardiac surgery outcome measures (Spearman rank correlation value - 0.096, p value < 0.0005). An unsupervised clustering K-means-based algorithm for finding outliers was also applied to these datasets. It captured a unique trend in the number of nurse practitioners which was increasing rapidly since 2010. It also revealed consistencies in practitioner placement throughout hospitals. Our findings suggest that the training of healthcare professionals appears to be robust and positions them for long-lasting and consistent careers across the majority of specialties.

Project description:BackgroundAlthough the vast majority of cancer patients use natural health products (NHPs), 59% of oncology healthcare providers (HCP) report not receiving any education on NHPs. KNOWintegrativeoncology.org (KNOW) is a web-based educational platform that provides up-to-date evidence on NHPs used in cancer care with a user-friendly interface. KNOW is a database of human studies systematically gathered from MEDLINE and EMBASE. We surveyed HCPs before and after accessing KNOW to identify their information needs regarding NHPs in cancer care, their preferred way to receive information, barriers they face accessing NHP information, and to obtain feedback on the website.MethodsRecruitment was done through Beaumont Health Systems, the Society for Integrative Oncology, and the Andrew Weil Centre for Integrative Medicine, University of Arizona. HCPs who consented completed an initial survey and then a follow-up survey after being given access to KNOW for 4-6 weeks. Participants were required to access KNOW at least three times before completion of the follow-up survey.ResultsA total of 65 participants completed the initial survey, with 60% (n = 39) from the conventional medical community, 33% (n = 21) from the integrative medicine community, and 7% (n = 5) from the research community. The majority of participants (82%; n = 53) preferred educational websites to email updates, podcasts/webinars, in-house experts, PubMed searches and smartphone apps. The most common barriers identified to accessing information on NHPs were time, accessibility at point-of-care, and credibility of sources. A high number of participants were lost to follow up, with 18 participants demographically representative of the initial sample of 65 completing the follow-up survey. Half (n = 9) of participants stated accessing the KNOW website changed their clinical practice. Close to 90% (n = 16) reported they would recommend KNOW to a colleague.ConclusionOncology HCPs reported preferring to use, and already relying on, numerous web-based educational platforms to gather information on NHPs, with time, accessibility, and credibility being common barriers to obtaining information. Our study findings highlight the promise of the KNOW web-based educational platform in reducing barriers to accessing up-to-date information on NHPs in busy cancer care settings.

Project description:BackgroundBurnout and compassion fatigue are closely related concepts. Burnout is thought to develop from occupational stress while compassion fatigue results from being in a caregiver role leading to inability to get engaged in a caring relation. The objective of the current study was to investigate research trends, themes, citations, and key players of publications on burnout and compassion fatigue among healthcare providers.MethodsA validated search query was developed and implemented in the Scopus database. The study period was all times up to 2019.ResultsResearch query found 4416 publications. Publications started in 1978. Steep growth in the number of publications was observed in the last decade. There were seven research themes in the retrieved publications; six for burnout and one for compassion fatigue. Approximately 36% of the retrieved publications were about nurses; 36% were about physicians, 10% were about medical residents and the remaining did not include a specific profession. The retrieved publications received an average of 22.2 citations per article. Four of the top 10 active journals were in the field of nursing and two in the field of general medicine, one in public health, one in neurology, one in psychology, and one was multidisciplinary. The USA ranked first with 1292 (29.3%) articles followed by Spain (n = 248; 5.6%) and the UK (n = 247; 5.6%). Mayo Clinic was the most active institution (n = 93; 2.1%) followed by Harvard University (n = 46; 1.0%) and University of Washington, Seattle (n = 45; 1.0%). A total of 16,108 authors participated in publishing the retrieved documents, an average of 3.6 authors per article. Shanafelt, T.D. was the most active author (n = 78; 1.8%) followed by Dyrbye, L.N. (n = 43; 1.0%), and West, C.P. (n = 37; 0.8%). A total of 472 (10.7%) articles declared funding.ConclusionThe current study was carried out to draw attention to the wellbeing of healthcare providers. Retrieved literature was dominated by high-income countries. Lack of information from low- and middle-income countries will hinder planning for interventional strategies and will negatively affect the health system and the patients. Health researchers in low- and middle-income countries need to focus on burn out and compassion fatigue.
