We document changes in U.S. children's family household composition from 1968 to 2017 with regard to the number and types of kin that children lived with and the frequency of family members' household entrances and departures. Data are from the U.S. Panel Study of Income Dynamics (N = 30,412). Project description:BackgroundWomen's social roles (partnership, parenthood, and worker status) are associated with health, with more roles being associated with lower mortality rates. Few studies have examined social roles using a lifecourse perspective to understand how changing role dynamics affect health over time. Sequence analysis is one analytic technique for examining social trajectories.MethodsWork-family trajectories were determined using social sequence analysis. We estimated mortality using age-standardized mortality rates and Poisson regression and examined the impact of personal income as a mediator.ResultsWe identified 5 trajectory types according to probability distributions of work/marriage/child-rearing status and descriptions in previous research: Non-working, married, later-mothers; working divorced mothers; working and non-working, never-married mothers; working, never-married non-mothers; and non-working, married earlier-mothers. Our reference group, non-working, married, later-mothers had the lowest mortality rates (1.47 per 1000 person-years). Adjusting for confounders, timing of childbearing did not impact mortality rates for married, non-working women. Working, never-married non-mothers and working and non-working, never-married mothers had the highest adjusted rates of mortality (RR = 1.81 and 1.57, respectively) these effects were attenuated slightly by the addition of household income in the model. Mortality rates for other trajectory groups were not significantly elevated in adjusted models.ConclusionsMortality rates vary by work-family trajectories, but timing of childbearing does not meaningfully impact risk among women in this population, likely because few of the women who were married and had children also worked full-time. Household income has some mediating effect among those at highest risk of early mortality.

Project description:ObjectiveWe consider the prevalence of family complexity and its association with children's externalizing behavior problems over children's life course and over historical time.BackgroundA growing literature has demonstrated the prevalence and multidimensional nature of family complexity and its association with child behavior. The nature/strength of this association may have changed in recent cohorts as family complexity has become more common.MethodData are from the 1997 and 2014 cohorts of the Panel Study of Income Dynamics Child Development Supplement. Samples represent U.S. children ages 0-12 years born since 1985 (N=5,030). Ordinary least squares regression estimated change in the association between family complexity and behavior between cohorts. Difference-in-difference models estimated baseline and longitudinal differences in children's behavior as linked to family complexity.ResultsThe prevalence of family complexity has stabilized over the last two decades, and the antecedents to parental repartnering and complex sibship organization remain similar. The expectation that increasing family complexity contributes to elevated behavior problem scores was not supported. Instead, children who eventually acquired a step- or half-sibling or who experienced parents' union dissolution had elevated behavior problems prior to those changes.ConclusionThe prevalence of and precursors to complex family organization were stable across recent child cohorts. The observed association between family complexity and child behavior problems may be attributable to selection mechanisms linked to both parents' family formation trajectories and to children's behavior, rather than to family change itself.

Project description:BackgroundStrongyloidiasis is a neglected tropical disease (NTD). It is commonly associated with poverty and poor hygiene. Strongyloidiasis poses an unseen global public health problem. The aim of this study was to assess and analyze peer-reviewed literature on human strongyloidiasis to shed light on the evolution, volume, important topics, and key players in the field of human strongyloidiasis.MethodsA validated bibliometric method was implemented using Scopus database for the study period from 1968 to 2017. The search strategy was developed based on keywords related to strongyloidiasis. Bibliometric indicators and visualization maps were presented.ResultsIn total, 1947 documents were found. Retrieved documents received 32,382 citations, an average of approximately 16.6 per document, and an h-index of 76. The most frequently encountered keywords in the retrieved literature focused on hyperinfection, diagnosis, prevalence, and ivermectin. The USA led with 540 (27.7%) documents followed by Brazil (139; 7.1%) and Japan (137; 7.0%). When research output was standardized by income and population size, India ranked first (12.4 documents per GDP/capita) followed by the USA (9.1 documents per GDP/capita). The most active journal involved in publishing articles was the American Journal of Tropical Medicine and Hygiene (95; 4.8%). In terms of institutions, the University of Ryukyus (Japan) was the most active with 62 (3.2%) publications, followed by the University of Pennsylvania with 54 (2.8%) publications.ConclusionThe volume, growth, and international research collaboration in human strongyloidiasis were inadequate given the long history of the disease, the large number of affected people, and the results obtained for other NTDs. Research in human strongyloidiasis needs to be strengthened and encouraged in endemic regions in Southeast Asia and Latin America. International research networking needs to be established to achieve the goals of Sustainable Development Goals in fighting and eradicating NTDs by 2030.

Project description:Crude oil pipelines are considered as the lifelines of energy industry. However, accidents of the pipelines can lead to severe public health and environmental concerns, in which greenhouse gas (GHG) emissions, primarily methane, are frequently overlooked. While previous studies examined fugitive emissions in normal operation of crude oil pipelines, emissions resulting from accidents were typically managed separately and were therefore not included in the emission account of oil systems. To bridge this knowledge gap, we employed a bottom-up approach to conducted the first-ever inventory of GHG emissions resulting from crude oil pipeline accidents in the United States at the state level from 1968 to 2020, and leveraged Monte Carlo simulation to estimate the associated uncertainties. Our results reveal that GHG emissions from accidents in gathering pipelines (~720,000 tCO2e) exceed those from transmission pipelines (~290,000 tCO2e), although significantly more accidents have occurred in transmission pipelines (6883 cases) than gathering pipelines (773 cases). Texas accounted for over 40% of total accident-related GHG emissions nationwide. Our study contributes to enhanced accuracy of the GHG account associated with crude oil transport and implementing the data-driven climate mitigation strategies.

Project description:The present study examines the development of complex sentences with non-finite clause combining with particular focus on clause chaining, in narratives of 40 Turkish-speaking 4- to 11-year-olds and six adults elicited by a wordless picture book. Results show a gradual increase by age in the variety of clauses combined, the length of the complex sentences and their frequency of use. Clause chains formed with converbal clauses are the earliest and most frequent type of clause combinations, already present in 4-year-olds' complex sentences with 1-non-finite clause. Older children's and adults' 2- or 3-non-finite clause complex sentences consist of some combinations of adverbial, complement, relative and converbal clauses. Developmentally, clause chains establish first, aspectual-temporal continuity, then temporal-causal continuity. Sentence-internal and cross-sentence-boundary referential continuities are present early, from age 4 onwards. These findings are discussed in terms of the demands of narrative organization as well as the syntactic and semantic complexity of the clause combination devices in Turkish.

Project description:The question of what is a perceptual object is one of the most central and also controversial issues in cognitive science. According to the topological approach to perceptual organization, the core intuitive notion of an object--the holistic identity preserved over shape-changing transformations--may be characterized precisely as topological invariance. Here we show that, across a series of multiple-object tracking tasks, performance was not disrupted when the moving items underwent massive featural changes. However, performance was significantly impaired when the items changed their topological properties of holes, demonstrating that topological invariance constrains what counts as an object in the first place. Consistent with previous findings, fMRI studies indicated that the anterior temporal lobe may be involved in the formation of object representation defined by topological constraints.

Project description:To compare the prevalence, resource utilization, and mortality for pediatric severe sepsis identified using two established identification strategies.Observational cohort study from 2004 to 2012.Forty-four pediatric hospitals contributing data to the Pediatric Health Information Systems database.Children 18 years old or younger.We identified patients with severe sepsis or septic shock by using two International Classification of Diseases, 9th edition, Clinical Modification-based coding strategies: 1) combinations of International Classification of Diseases, 9th edition, Clinical Modification codes for infection plus organ dysfunction (combination code cohort); 2) International Classification of Diseases, 9th edition, Clinical Modification codes for severe sepsis and septic shock (sepsis code cohort). Outcomes included prevalence of severe sepsis, as well as hospital and ICU length of stay, and mortality. Outcomes were compared between the two cohorts examining aggregate differences over the study period and trends over time. The combination code cohort identified 176,124 hospitalizations (3.1% of all hospitalizations), whereas the sepsis code cohort identified 25,236 hospitalizations (0.45%), a seven-fold difference. Between 2004 and 2012, the prevalence of sepsis increased from 3.7% to 4.4% using the combination code cohort and from 0.4% to 0.7% using the sepsis code cohort (p < 0.001 for trend in each cohort). Length of stay (hospital and ICU) and costs decreased in both cohorts over the study period (p < 0.001). Overall, hospital mortality was higher in the sepsis code cohort than the combination code cohort (21.2% [95% CI, 20.7-21.8] vs 8.2% [95% CI, 8.0-8.3]). Over the 9-year study period, there was an absolute reduction in mortality of 10.9% (p < 0.001) in the sepsis code cohort and 3.8% (p < 0.001) in the combination code cohort.Prevalence of pediatric severe sepsis increased in the studied U.S. children's hospitals over the past 9 years, whereas resource utilization and mortality decreased. Epidemiologic estimates of pediatric severe sepsis varied up to seven-fold depending on the strategy used for case ascertainment.

Project description:This study examines intergenerational continuity (mean level similarity) and stability (maintenance of rank ordering of individuals) in age and marital status at the time of becoming a young parent using prospective data from 3 generations of 585 families. G2 participants were recruited at the age of 5 years and followed until the age of 28, by which time 227 had become parents themselves. The findings suggest that despite dramatic intergenerational discontinuities with young adults, on average, now being more likely to be unmarried and older at the time of becoming parents than in previous generations, intergenerational stability in age and marital status at the time of becoming a young parent is still substantial. This intergenerational stability was, for the most part, not moderated by demographic, familial, or behavioral factors, suggesting that a developmental, multigenerational perspective is necessary to understand what has previously been considered a largely demographic issue. Project description:Understanding the determinants of sedentary time during childhood contributes to the development of effective intervention programmes.To examine family and home-environmental determinants of 1-year change in objectively measured sedentary time after-school and at the weekend.Participants wore accelerometers at baseline and 1 year later. Longitudinal data for after-school and weekend analyses were available for 854 (41.5%male, mean ± SD age 10.2 ± 0.3 years) and 718 (41.8%male, age 10.2 ± 0.3 years) participants. Information on 26 candidate determinants, including socioeconomic status (SES), availability of electronic media and parental rules for sedentary behaviours was self-reported by children or their parents at baseline. Change in the proportion of registered time spent sedentary was used as the outcome variable in multi-level linear regression models, adjusted for age, sex, body mass index and baseline sedentary time. Simple and multiple models were run and interactions with sex explored.Children from higher socioeconomic status families exhibited greater increases in after-school (beta; 95% CI for change in % time spent sedentary 1.02; 0.37, 1.66) and weekend (1.42; 0.65, 2.18) sedentary time. Smaller increases in after-school sedentary time were observed in children with more siblings (-1.00; -1.69, -0.30), greater availability of electronic media (-0.81; -1.29, -0.33) and, for boys, more frequent family visits to the park (-1.89; -3.28, -0.51) and family participation in sport (-1.28; -2.54, -0.02). Greater maternal weekend screen-time (0.45; 0.08, 0.83) and, in girls, greater parental restriction on playing outside (0.91; 0.08, 1.74) were associated with larger increases in weekend sedentary time. The analytical sample was younger, more likely to be female, had lower BMI and was of higher SES than the original baseline sample.Intervention strategies aimed at reducing parents' weekend screen-time, increasing family participation in sports or recreation (boys) and promoting freedom to play outside (girls) may contribute towards preventing the age-related increase in sedentary time.

Project description:In the United States, the gold standard for malaria diagnosis is microscopic blood smear examination. Because malaria is not endemic in the United States, diagnostic capabilities may be limited, causing delays in diagnosis and increased morbidity and mortality. A survey of the malaria diagnostic practices of U.S. laboratories was conducted from June to July 2017; members of the American Society for Microbiology's listserv received a questionnaire inquiring about malaria diagnostic test availability, techniques, and reporting. Results were assessed using the Clinical and Laboratory Standards Institute (CLSI) guidelines for malaria diagnostics. After excluding incomplete and duplicate responses, responses representing 175 laboratories were included. Most labs (99%) received at least one specimen annually for malaria diagnosis, and 31% reported receiving only 1 to 10 specimens. The majority (74%) diagnosed five or fewer cases of malaria per year. Most (90%) performed blood smears on-site. Two-thirds (70%) provided initial blood smear results within 4 h. Although diagnostic testing for malaria was available 24/7 at 74% (141) of responding laboratories, only 12% (17) met criteria for analysis and reporting of malaria testing, significantly more than reported in a similar survey in 2010 (3%; P < 0.05). The majority of laboratories surveyed had the capability for timely diagnosis of malaria; few comply with CLSI guidelines. Inexperience may factor into this noncompliance; many laboratories see few to no cases of malaria per year. Although reported adherence to CLSI guidelines was higher than in 2010, there is a need to further improve laboratory compliance with recommendations.
