Project description:MitoFish, MitoAnnotator, and MiFish Pipeline are comprehensive databases of fish mitochondrial genomes (mitogenomes), accurate annotation software of fish mitogenomes, and a web platform for metabarcoding analysis of fish mitochondrial environmental DNA (eDNA), respectively. The MitoFish Suite currently receives over 48,000 visits worldwide every year; however, the performance and usefulness of the online platforms can still be improved. Here, we present essential updates on these platforms, including an enrichment of the reference data sets, an enhanced searching function, substantially faster genome annotation and eDNA analysis with the denoising of sequencing errors, and a multisample comparative analysis function. These updates have made our platform more intuitive, effective, and reliable. These updated platforms are freely available at http://mitofish.aori.u-tokyo.ac.jp/.

Project description:IntroductionAn adequate health workforce (HWF) is essential to achieving the targets of the Sustainable Development Goals (SDG), including universal health coverage. However, weak HWF planning and constrained fiscal space for health, among other factors in the WHO Africa Region, has consistently resulted in underinvestment in HWF development, shortages of the HWF at the frontlines of service delivery and unemployment of qualified and trained health workers. This is further compounded by the ever-evolving disease burden and reduced access to essential health services along the continuum of health promotion, disease prevention, diagnostics, curative care, rehabilitation and palliative care.MethodsA stock and flow model based on HWF stock in 2022, age structure, graduation and migration was conducted to project the available stock by 2030. To estimate the gap between the projected stock and the need, a population needs-based modelling was conducted to forecast the HWF needs by 2030. These estimations were conducted for all 47 countries in the WHO African Region. Combining the stock projection and needs-based estimation, the modelling framework included the stock of health workers, the population's need for health services, the need for health workers and gap analysis expressed as a needs-based shortage of health workers.ResultsThe needs-based requirement for health workers in Africa was estimated to be 9.75 million in 2022, with an expected 21% increase to 11.8 million by 2030. The available health workers in 2022 covered 43% of the needs-based requirements and are anticipated to improve to 49% by 2030 if the current trajectory of training and education outputs is maintained. An increase of at least 40% in the stock of health workers between 2022 and 2030 is anticipated, but this increase would still leave a needs-based shortage of 6.1 million workers by 2030. Considering only the SDG 3.c.1 tracer occupations (medical doctors, nurses, midwives, pharmacists and dentists), the projected needs-based shortage is 5.3 million by 2030. In sensitivity analysis, the needs-based shortage is most amenable to the prevalence of diseases/risk factors and professional standards for service delivery CONCLUSIONS: The WHO African Region would need to more than double its 2022 HWF stock if the growing population's health needs are to be adequately addressed. The present analysis offers new prospects to better plan HWF efforts considering country-specific HWF structure, and the burden of disease.

Project description:Current and projected shortages in the US health workforce have prompted policymakers to propose reforms to Medicare Graduate Medical Education (GME) and nursing programs. However, researchers have historically faced challenges in accurately predicting workforce trends; physician and nurse supply and demand all grew faster than expected over the past 2 decades. These discrepancies highlight the need for scenario-based workforce planning and projection models that estimate how a policy intervention would affect the workforce outcome of interest. In addition, policy options modeled should address not only increasing provider-to-population ratios but also improving health outcomes through innovative payment and care models.

Project description:Despite the increasing prevalence of CKD in the United States, there is a declining interest among United States medical graduates in nephrology as a career choice. Effective programs are needed to generate interest at early educational stages when career choices can be influenced. The Kidney Disease Screening and Awareness Program (KDSAP) is a novel program initiated at Harvard College that increases student knowledge of and interest in kidney health and disease, interest in nephrology career paths, and participation in kidney disease research. This model, built on physician mentoring, kidney screening of underserved populations, direct interactions with kidney patients, and opportunities to participate in kidney research, can be reproduced and translated to other workforce-challenged subspecialties.

Project description:The global prevalence of hypertension continues excessively elevated, especially among low- and middle-income nations. Workplaces provide tremendous opportunities as a unique, easily accessible and practical avenue for early diagnosis and treatment of hypertension among the workforce class. The evaluation of such a Workplace Screening Strategy can give insight into its possible effects. Innovative machine learning approaches like k-means clustering are underutilized for such assessments. We set out to use this technology to analyze the results of our university's yearly health checkup of the employees for hypertension. An anonymized dataset including the demographics and blood pressure monitoring information gathered from workers in various departments/units of a learning organization. The overall amount of samples or data values is 1, 723, and the supplied dataset includes six attributes, such as year group (2018, 2019, 2021, 2022), Department/Unit (academic and non-academic), and gender (male and female), with the intended output being the blood pressure status (low, normal, and high). The dataset was analyzed using machine learning approaches. In this longitudinal study, it was discovered that the average age for the workforce is 42. Similarly, it was revealed that hypertension was common among employees over the age of 40, regardless of gender or occupational type (academic or nonacademic). The data also found that there was a consistent drop in the prevalence of hypertension from 2018 to 2022. According to the study findings, the use of machine learning algorithms for periodic evaluations of workplace health status monitoring initiatives (particularly for hypertension) is feasible, realistic, and sustainable in diagnosing and controlling hypertension among those in the workforce.

Project description:BackgroundPublic hospitals are facing a critical shortage of health workers. The area-based network consolidations could be the solution to increase the system capacity for human resources by improving local allocative efficiency.MethodsThis study develops counterfactual simulations for area-based network allocations for the health workforce in 10500 public hospitals in Thailand and examines improvements in allocative efficiency from the health workforce redistribution at different administrative levels such as sub-districts, districts, provinces, and health service areas. The workload per worker is calculated from the output measured by numbers of outpatient and inpatient cases and the input measured by numbers of health workers. Both output and input are weighted with their economic values and controlled for heterogeneity through regression analysis. Finally, this study compares the workload per worker and economic valuation of the area-based networks or ex-ante scenarios with the hospital-level or status quo scenario.ResultsNetwork consolidations of the sub-district primary-level hospitals within the same district could reduce workload per worker by seven percentage points. Another practical policy option is to consolidate similar hospital levels such as primary, first-level secondary, and mid-level secondary hospitals altogether within the same province which could result in the reduction of the workload per worker by 6-7 percentage points. The total economic value gained from consolidating similar hospital levels within the same province is about 15-18 percentage points of total labor cost in the primary hospitals.ConclusionThis study illustrates the improvement in allocative efficiency of the health workforce in public hospitals from the area-based network consolidations. The results provide an insightful example of economic gains from efficiently reallocating the medical workforce within the same local areas. Major reforms are required such that the health care delivery units can automate their resources in corresponding to the population's health needs through a strengthening gatekeeping system.

Project description:Research shows that mindfulness practice can alter the expression of genes associated with energy metabolism, telomere maintenance, inflammatory and stress response. The aim of this study was to determine if mindfulness awareness practice (MAP) or health education program (HEP) might reverse cognitive impairment and/or prevent further cognitive decline in 60 subjects (aged 60-90). We investigated the gene expression changes in subject with mild cognitive impairment randomized to either group after 9 months. We used Affymetrix GeneChip Human Genome U133 Plus 2.0 array to determine the differential gene expression between Pre- and Post-treatment samples from MAP and HEP group.
