In Vivo Super-Resolution Cardiac Diffusion Tensor MRI: A Feasibility Study.

ABSTRACT: A super-resolution (SR) technique is proposed for imaging myocardial fiber architecture with cardiac magnetic resonance. Images were acquired with a motion-compensated cardiac diffusion tensor imaging (cDTI) sequence. The heart left ventricle was covered with three stacks of thick slices, in short axis, horizontal and vertical long axes orientations, respectively. The three low-resolution stacks (2 × 2 × 8 mm³) were combined into an isotropic volume (2 × 2 × 2 mm³) by a super-resolution reconstruction. For in vivo measurements, each slice was acquired during a breath-hold period. Bulk motion was corrected by optimizing a similarity metric between intensity profiles from all intersecting slices in the dataset. The benefit of the proposed approach was evaluated using a numerical heart phantom, a physical helicoidal phantom with artificial fibers, and six healthy subjects. The SR technique showed improved results compared to the native scans, in terms of image quality and cDTI metrics. In particular, the myocardial helix angle (HA) was more accurately estimated in the physical phantom (HA = 41.5° ± 1.1°, with the ground truth being 42.0°). In vivo, it resulted in a sharper rate of change of HA across the myocardial wall (-0.993°/% ± 0.007°/% against -0.873°/% ± 0.010°/%).

Project description:IntroductionMyocardial perfusion MRI is important for diagnosing coronary artery disease, but current clinical methods face challenges in balancing spatial resolution, temporal resolution, and slice coverage. Achieving broader slice coverage and higher temporal resolution is essential for accurately detecting abnormalities across different slice locations but remains difficult due to constraints in acquisition speed and heart rate variability. While techniques like parallel imaging and compressed sensing have significantly advanced perfusion imaging, they still suffer from noise amplification, residual artifacts, and potential temporal blurring due to the rapid transit of dynamic contrast vs. the temporal constraints of the reconstruction.MethodsThis study introduces a conditional diffusion-based generative model for myocardial perfusion MRI super resolution, addressing the trade-offs between spatiotemporal resolution and slice coverage. We adapted Denoising Diffusion Probabilistic Models (DDPM) to enhance low-resolution perfusion images into high-resolution outputs without requiring temporal regularization. The forward diffusion process introduces Gaussian noise incrementally, while the reverse process employs a U-Net architecture to progressively denoise the images, conditioned on the low-resolution input image.ResultsWe trained and validated the model on a retrospective dataset of dynamic contrast-enhanced (DCE) perfusion MRI, consisting of both stress and rest images from 47 patients with heart disease. Our results showed significant image quality improvements, with a 5.1% reduction in nRMSE, a 1.1% increase in PSNR, and a 2.2% boost in SSIM compared to GAN-based super-resolution method (P < 0.05 for all metrics using paired t-test) in retrospective study. For the 9 prospective subjects, we achieved a total nominal acceleration of 8.5-fold across 5-6 slices through a combination of low-resolution acquisition and GRAPPA. PerfGen outperformed GAN-based approach in sharpness (4.36 ± 0.38 vs. 4.89 ± 0.22) and overall image quality (4.14 ± 0.28 vs. 4.89 ± 0.22), as assessed by two experts in a blinded evaluation (P < 0.05) in prospective study.DiscussionThis work demonstrates the capability of diffusion-based generative models in generating high-resolution myocardial perfusion MRI from conditional low-resolution images. This approach has shown the potentials to accelerate myocardial perfusion MRI while enhancing slice coverage and temporal resolution, offering a promising alternative to existing methods.

Project description:PurposeCardiomyocyte organization and performance underlie cardiac function, but the in vivo mobility of these cells during contraction and filling remains difficult to probe. Herein, a novel trigger delay (TD) scout sequence was used to acquire high in-plane resolution (1.6 mm) Spin-Echo (SE) cardiac diffusion tensor imaging (cDTI) at three distinct cardiac phases. The objective was to characterize cardiomyocyte organization and mobility throughout the cardiac cycle in healthy volunteers.Materials and methodsNine healthy volunteers were imaged with cDTI at three distinct cardiac phases (early systole, late systole, and diastasis). The sequence used a free-breathing Spin-Echo (SE) cDTI protocol (b-values = 350s/mm2, twelve diffusion encoding directions, eight repetitions) to acquire high-resolution images (1.6x1.6x8mm3) at 3T in ~7 minutes/cardiac phase. Helix Angle (HA), Helix Angle Range (HAR), E2 angle (E2A), Transverse Angle (TA), Mean Diffusivity (MD), diffusion tensor eigenvalues (λ1-2-3), and Fractional Anisotropy (FA) in the left ventricle (LV) were characterized.ResultsImages from the patient-specific TD scout sequence demonstrated that SE cDTI acquisition was possible at early systole, late systole, and diastasis in 78%, 100% and 67% of the cases, respectively. At the mid-ventricular level, mobility (reported as median [IQR]) was observed in HAR between early systole and late systole (76.9 [72.6, 80.5]° vs 96.6 [85.9, 100.3]°, p<0.001). E2A also changed significantly between early systole, late systole, and diastasis (27.7 [20.8, 35.1]° vs 45.2 [42.1, 49]° vs 20.7 [16.6, 26.4]°, p<0.001).ConclusionWe demonstrate that it is possible to probe cardiomyocyte mobility using multi-phase and high resolution cDTI. In healthy volunteers, aggregate cardiomyocytes re-orient themselves more longitudinally during contraction, while cardiomyocyte sheetlets tilt radially during wall thickening. These observations provide new insights into the three-dimensional mobility of myocardial microstructure during systolic contraction.

Project description:High-resolution imaging studies have consistently shown that in cortical tissue water diffuses preferentially along radial and tangential orientations with respect to the cortical surface, in agreement with histology. These dominant orientations do not change significantly even if the relative contributions from microscopic water pools to the net voxel signal vary across experiments that use different diffusion times, b-values, TEs, and TRs. With this in mind, we propose a practical new framework for imaging non-parametric diffusion tensor distributions (DTDs) by constraining the microscopic diffusion tensors of the DTD to be diagonalized using the same orthonormal reference frame of the mesoscopic voxel. In each voxel, the constrained DTD (cDTD) is completely determined by the correlation spectrum of the microscopic principal diffusivities associated with the axes of the voxel reference frame. Consequently, all cDTDs are inherently limited to the domain of positive definite tensors and can be reconstructed efficiently using Inverse Laplace Transform methods. Moreover, the cDTD reconstruction can be performed using only data acquired efficiently with single diffusion encoding, although it also supports datasets with multiple diffusion encoding. In tissues with a well-defined architecture, such as the cortex, we can further constrain the cDTD to contain only cylindrically symmetric diffusion tensors and measure the 2D correlation spectra of principal diffusivities along the radial and tangential orientation with respect to the cortical surface. To demonstrate this framework, we perform numerical simulations and analyze high-resolution dMRI data from a fixed macaque monkey brain. We estimate 2D cDTDs in the cortex and derive, in each voxel, the marginal distributions of the microscopic principal diffusivities, the corresponding distributions of the microscopic fractional anisotropies and mean diffusivities along with their 2D correlation spectra to quantify the cDTD shape-size characteristics. Signal components corresponding to specific bands in these cDTD-derived spectra show high specificity to cortical laminar structures observed with histology. Our framework drastically simplifies the measurement of non-parametric DTDs in high-resolution datasets with mesoscopic voxel sizes much smaller than the radius of curvature of the underlying anatomy, e.g., cortical surface, and can be applied retrospectively to analyze existing diffusion MRI data from fixed cortical tissues.

Project description:Background Cardiac MRI is limited by long acquisition times, yet faster acquisition of smaller-matrix images reduces spatial detail. Deep learning (DL) might enable both faster acquisition and higher spatial detail via super-resolution. Purpose To explore the feasibility of using DL to enhance spatial detail from small-matrix MRI acquisitions and evaluate its performance against that of conventional image upscaling methods. Materials and Methods Short-axis cine cardiac MRI examinations performed between January 2012 and December 2018 at one institution were retrospectively collected for algorithm development and testing. Convolutional neural networks (CNNs), a form of DL, were trained to perform super resolution in image space by using synthetically generated low-resolution data. There were 70%, 20%, and 10% of examinations allocated to training, validation, and test sets, respectively. CNNs were compared against bicubic interpolation and Fourier-based zero padding by calculating the structural similarity index (SSIM) between high-resolution ground truth and each upscaling method. Means and standard deviations of the SSIM were reported, and statistical significance was determined by using the Wilcoxon signed-rank test. For evaluation of clinical performance, left ventricular volumes were measured, and statistical significance was determined by using the paired Student t test. Results For CNN training and retrospective analysis, 400 MRI scans from 367 patients (mean age, 48 years ± 18; 214 men) were included. All CNNs outperformed zero padding and bicubic interpolation at upsampling factors from two to 64 (P < .001). CNNs outperformed zero padding on more than 99.2% of slices (9828 of 9907). In addition, 10 patients (mean age, 51 years ± 22; seven men) were prospectively recruited for super-resolution MRI. Super-resolved low-resolution images yielded left ventricular volumes comparable to those from full-resolution images (P > .05), and super-resolved full-resolution images appeared to further enhance anatomic detail. Conclusion Deep learning outperformed conventional upscaling methods and recovered high-frequency spatial information. Although training was performed only on short-axis cardiac MRI examinations, the proposed strategy appeared to improve quality in other imaging planes. © RSNA, 2020 Online supplemental material is available for this article.

Sessions in healthy subjects-standard ddMRI (30 directions), standard DTI (15 and 30 directions), and nested cubes scans (15 and 30 directions). Sessions in epilepsy patients-two 30 direction (standard ddMRI, 10 nested cubes) or two 15 direction scans (standard DTI, 5 nested cubes).AssessmentFifteen direction DTI was repeated twice for within-session test-retest measurements in healthy subjects. Bland-Altman analysis computed bias and limits of agreement for DTI metrics using test-retest scans and standard 15 direction vs. 5 nested cubes scans. Correlation of white matter fractional anisotropy was also strong (ICC > 0.95) and significant. ICCs of the CoV of dynamic ADC measured using repeated cubes and nested cubes acquisitions were modest (ICC >0.60), but significant in gray matter.ConclusionA nested cubes diffusion gradient table produces tensor-based and dynamic diffusion measurements in a single acquisition.Level of evidence2 TECHNICAL EFFICACY: Stage 1.
